Talks over a fresh deal could be postponed until after the Asian Games when his future will become clear, with the player still currently under contract at the club until June 2020

gives you the information education and expert insight you need to become an
active participant in today's ever changing health care climate here now as
today's program hosts America is seeing a sharp increase in the number of opioid
related deaths nearly half of all overdose deaths involve prescription
opioids this has forced a nationwide push to reduce the number of these types
of deaths today we'll take a look at the issues leading to this nationwide
epidemic and along the way learn what's being done to curtail opioid abuse and
misuse welcome to healthy living for life a show dedicated to helping you do
just that I'm Janet Whitmoyer stay tuned welcome back my first guest
is Dr. Phillip Steele a family practice sports medicine physician here in Helena
thanks for joining us Dr. Steele ok I wanted to start off by asking why are so
many people who are on opioids still in pain while on these powerful drugs well
that's a great question the problem is is that many of these medications
decrease the threshold at which you perceive pain so simply speaking if I
were to pinch my my hand right here and I pinch it lightly I shouldn't feel any
pain but as I pinch it harder and harder and harder at some point I'll perceive
pain what fentanyl and morphine and many of these medications do is lower the
threshold so the amount of pinch that I need to pinch here to create pain
changes and so the more I prescribe an opioid medication the lower my threshold
to perceive pain is and fentanyl is a horrible example of this it's
a medication that's made many many Americans dependent on this stuff
because as physician like oh you poor thing you're in pain let's give you more
and we're actually compounding the problem we're actually making them more
sensitive to pain so that if we just briefly touch something or have pain
shooting down our arm we just barely touch them they're like oh my gosh
we need to help you and as physicians we we got into being physicians because we
want to help people and we don't want you to suffer so we give you more and
that's actually compounding the problem and as a physician we weren't taught
about this when I went to medical school nobody even talked about this I go to
conferences and nobody mentions this so it's really important as a physician and
the public to be aware of this this issue that we're actually making the
problem worse by prescribing these medications great can you tell us a
little bit about what opioid-induced hyperalgesia is that's essentially it
is by using opioid medications for chronic pain we're actually creating
more pain in some of our patients and it's thought to be only a small
percentage that patients initially had this rare complication but we're
learning that more and more that this is a much higher percentage of
the population and my clinical practice I see it all the time people come to me
they're on fentanyl they're on break through oxycontin tablets and they come
in and I just barely touch them or I just try to move them to for an
examination and their pains out of proportion to any of the problems that
they have and so there are physicians keep giving them more and
that's not the answer to this equation the equation is fixed
by getting them off of this medications completely okay so can you talk a little
bit about how you would treat somebody and what the treatment would be to get
them off of that I can't imagine you just say no more well the the problem is
patients come in and they say I'm in pain you want to take this away from me
the initial responses are you crazy and so I need more of this stuff and so
it's a paradigm shift we have to educate our patients that pain is actually okay
you know when you have an operation we say oh you need these pain medications
because you're gonna have pain post procedure well what if we change the
paradigm to say oh pain is good you're gonna have healing pain as part of this
procedure you're going to experience the healing pain symptoms after the
procedure so we need to change how we discuss and talk about pain we can also
there's medications to help get people off pain there's we can
long-acting methadone we can use clonidine for some of the symptoms of
withdrawal there's a narco naloxone which is a medication used for overdose
and for dependency so there's lots of medications that can be used and under
the care of a physician that's familiar with getting someone off this you can
use those medications but the biggest part is just the sales pitch you know I
have to convince my patient that at the end of the day if I get them off of
these medications they'll have less pain than they have currently and
that's a long complicated subject and the first time I meet a patient you know
it's they're like oh there's like no way that's gonna happen but as you continue
to work with the patient that's why I do a lot of nerve blocks and I do other
procedures to try to get them to a point where they're using less and less of it
so we convinced them that if I can get them off they'll actually hurt less and
they start to see that and they start to notice that they're feeling better
realize that opioids decreased testosterone production in women and in men
need to toss their own to heal if you have chronic opioid dependency you're on
chronic pain meds your testosterone level goes down so your your healing
potential decreases your depressed sexual desire goes down lots of things
that depress suicide rate go up depressions huge everything changes by
being on opioids chronically and so once they start coming off is they notice
that they're less depressed they can become more active they can start to go
back to work they could do many things so a lot of people that are in chronic
pain habituate at the point where they just sit at home and do nothing and so
you know a study is looking at just active lifestyle versus antidepressant
medications which wins in terms of being more effective being outside in the Sun
and the babbling brook and the birds singing was more effective than antidepressants
so we need to start looking at changing the paradigm how we speak about opioid
dependency how we talk about pain and then how to get our patients active and
so that they feel better and then convince them that they that by doing so
they're gonna improve the quality of their life great information so on the
flip side of that do you think that there is an option for opioids for some
patients or is that something that we should get away from prescribing
altogether you know there's a difference between acute pain and chronic pain so
you roll down the stairs and you break your leg there's good evidence to
support that short term opioid use 5 to 7 days is not addicting or low potential
for addiction risk and so we need to kind of convince our patients that
we're going to give you this medication and it's only for the acute onset of the
pain and then when we after 5 days we're going to transition you to something
else we're gonna start physical therapy we're gonna recommend acupuncture maybe
chiropractic care we have all these other options tins units there's
lots of modalities that we could do if you just cut them off with nothing to
hope for they're like boy I need another doctor they'll give me a pill awesome
well that is really good information to helps us to know when it's appropriate
and then when others other types of treatment are more appropriate for
treating pain so thank you very much Dr. Steele for being with us we'll be right

that way and once you know certain information and you build yourself up
you kind of look back and go wow I didn't think I needed that but those
things that you learn becomes so ingrained in you and part of your life
that it just helps you get through lots of situations I know and for me you know
living for three years with chronic pain or whatever life throws at you you need
that support network around you when you need it

Halfway through the match between Argentina and Nigeria it looked as if something had begun to take hold as the image of Diego Maradona seemingly asleep in his VIP box appeared on TV screens all over the world.
In reality his appearance had raised questions from the moment he entered the stadium. Memes of his demoniac look, arms crossed across his chest, had gone viral, as did the gif of him dancing with a woman in the crowd.
It looked like a man under the influence.
Maradona's well-documented history of substance abuse, both legal and illegal – an issue he has been openly discussing for decades – gave us a clue as to what was going on but the man himself says he has been clean for years and told reporters afterwards he had "only" been drinking white wine.
The man who has been described by many as the best footballer to have lived has been present at every Argentina match at this World Cup, held firmly in the grasp of some sort of security personnel as he teeters on the edge of the balcony much as he has done on the very edge of life throughout his turbulent existence.
He walks in and commands the chants of thousands, the focus of the world cameras and the response of online communities who faithfully ensure his every move goes viral as he prays, celebrates, applauds, insults, swears and expresses time and again what every man, woman and child feels at some point.
It is a laugh a minute with Maradona, always has been. I first met him when he was planning to launch a union to represent players' rights against Fifa.
Inside a five-star Paris hotel he said, "Let's go and get some pizza," and as we were stepping on to the pavement he warned us, "There's going to be a lot of people here; watch out." No sooner had he said this than he disappeared into the heart of a demented mob chanting his name, hands stretched out, clamouring for a chance to glimpse and touch him.

When he became a celebrity in reality TV dance shows, fat and unfit, he showed once more his hips can also turn right round, and although we have never literally seen his head spinning, metaphorically it clearly does.
He lived and played with a blatant disregard for the rules and laws of man. The cocaine abuse was but one manifestation, a handball-goal and shooting at journalists door-stepping his private home when his daughters were little a couple of other examples.
His final bow on the World Cup stage as a player was when he walked off the pitch led by a nurse in 1994, for a dope test which came back positive.
He re-emerged from the ashes of that disgrace, and became Argentina's 2010 World Cup manager, seducing the world yet again on football's biggest showbiz stage.
People often say he "was clever with his feet" but not with his head, or "great as a player but not as a person", as if one could be separated from the other.
Maradona's popularity worldwide would probably be best explained if we could accept contradiction as truth; Maradona's extremes are not so much a case of someone swaying between good and bad but more the embodiment of both simultaneously.
Excess is his preferred modus operandi – a life as close to the edge as his has landed him in hospitals and emergency rooms more than once.
Those who have had a chance to deal with him, whether doctors, judges or psychiatrists, often like to tell the tale, so we get to hear details such as the fact that admission to a private clinic in Uruguay after a particularly intense New Year's Eve celebration was more a matter of consumption of pizza and champagne in wholesale quantities than recreational use of illegal pharmacology.
Amid the madness, he has always been coherent. His "performance" during the Nigeria-Argentina game in St Petersburg naturally ended with medical professionals to hand.
One of the most controversial and charismatic celebrities alive, whose private life has been public for as long as any of us can remember, infamously fell out with Coppola over financial dealings gone wrong 15 years ago.
They had not met again until St Petersburg, a selfie of them reunited in a hug that went viral.
Maradona is estranged from his first wife and two daughters, for decades the women of steel who held him together and kept him alive, and a poignant tweet from his eldest daughter, Dalma, perhaps best summarises how we all might feel if he were to, unspeakably, one day not walk among us.

Side hustles are awesome because they have the potential to deliver a ton of money to
you on the side and imagine what you could do with an extra hundred, five
hundred even an extra thousand dollars a month?! I mean think about that. $12,000
a year coming in from a side hustle - that is no chump change. The
answer really depends on your situation. I'm gonna start first with two reasons
why it might not be your thing, and first, side hustles take a lot of dedication.
It's super hard to grow your business or hone your craft on the side if you
can't commit to it on a consistent basis. So if you don't have the time to put in
daily, at least, this might not be a good fit for you. Reason number two it's not a
good fit for you if it interferes with your day job. If your moneymaker suffers
because you're spending too much time on your side business, I hate to break it to
you but your job is not paying you to make money for yourself. If you want to
risk getting terminated then that's one thing but just know that a lot of
employers aren't gonna take too kindly if you're working on your side
business during their regular business hours. But of course I want to end this
on a happy note and I have two reasons why a side hustle might be perfect for
you! Number one, it is rejuvenating, and this is something that
we think people don't talk about enough! Sure you hear that side hustles are
super time-consuming and they are because you're working on your day job
you're coming home and you're working right away on your side hustle but we
find that to be super thrilling. It is rejuvenating. Think about it this way:
When you're tired of working on your day job and you can't take it any more
you can just switch gears on work on your passion or something creative or
just something that you love to work on and there's something really
rejuvenating about that. And the best thing is when your Side Hustle starts to
become a little too much and you're like I cannot look at this website anymore
- boom - you're back to work and it's just this way to keep yourself engaged and
rejuvenated all day long. And finally the last reason why this might be the thing
for you is because it's a great way to turn a side hustle a passion a hobby or
a skill that you have into a full-time career opportunity. If you're able to
dedicate consistent time on your side hustle and it starts to grow and you get
more and more clients and you can charge higher and higher rates, then it might be
time for you to turn your side hustle into your full time freelancer income
