Emerging prostate cancer treatment could delay need for hormone therapy
The treatment could potentially offer thousands of men with the illness a reprieve from the prospect of taking testosterone-blocking hormone medication for the rest of their lives.
An estimated 18,000 Australian men will be diagnosed with prostate cancer in 2018, while a further 3,500 men will likely die.
Many men living with the illness will need to take long-term hormone therapy, which slows the growth of prostate cancer by blocking testosterone.
But the treatment is also associated with challenging side effects, including weight gain, loss of muscle mass and physical strength, and menopause-like symptoms, such as hot flushes, loss of libido and osteoporosis.
In an Australian first, scientists at the Peter MacCallum Cancer Centre treated 33 men with advanced prostate cancer who had developed secondary tumours with a high-precision form of radiotherapy.
The one-off dose of stereotactic ablative body radiotherapy (SABR) treats each tumour site with pinpoint accuracy and can be done in an hour during an outpatient visit.
The SABR treatment stopped the growth of the cancer spots in 93 per cent of the men. After two years, about half of the men did not need hormone therapy, and in over a third the cancer did not return.
Tony McDonnell, 77, from Mount Barker in South Australia, travelled to Melbourne to take part in the study.
Mr McDonnell was placed on life-long hormone treatment to inhibit the growth of secondary tumours, after having his prostate removed due to cancer in 2011. However, Mr McDonnell developed secondary tumours on his spine and hip.
Since taking part in the study, the secondary cancer growths have disappeared and he is no longer on hormone therapy. "I got rid of the bone cancer without any pain, without any side effects at all," Mr McDonnell said.
For a select group of patients. Study head radiation oncologist Dr Shankar Siva said the initial results are "very promising".
"It's an attractive option to potentially delay hormone therapy and its side effects in a select group of patients," Associate Professor Siva said. "We still need to work out exactly who the best patients are who will benefit from this treatment.
Dr Siva said planning for a large-scale international randomised clinical trial is underway to examine whether the treatment could extend the life of patients with prostate cancer. The results from the trial have been published today in the medical journal European Urology.

A group of South Koreans who visited North Korea to prepare for reunions of families
separated by the Korean War... returned from a three-day trip Friday afternoon.
The team had 20 people -- including workers from the South Korean Red Cross and the private
sector firm Hyundai Asan, and technicians.
They were led by Kim Byung-dae, the director general of the humanitarian cooperation bureau
of Seoul's Unification Ministry.
They inspected the Mount Kumkang resort -- the venue for the family reunions -- checking
the meeting rooms, hotels, rest areas, the fire station and the electricity supplies.
When they arrived back in the South, the team told reporters...
South and North Korean technicians inspected the facilities together... and things went
smoothly thanks to close cooperation from the North.
They added that the resort needs a great deal of renovation as it's not been used in a long
time, since October 2015.
The team stressed that the two Koreas will get down to renovation soon, after a review
by South Korean experts and inter-Korean discussions.
Seoul and Pyongyang agreed in their Red Cross talks last Friday to hold reunions of separated
families at North Korea's Mount Kumkang for a week starting August 20th.
100 participants each from South and North Korea will have a chance to meet their relatives
on the other side of the border.
The South Korean Red Cross this week chose 500 candidates for the reunion through computerized
random selection.
The organization will eventually narrow that number down to 100 after checking on the candidates'
health, their willingness to participate,... and whether their family members in the North
are still alive.
