119.This is the main method I use when passing SDJ. - Duration: 2:33.

Yesterday, I had a very difficult colonoscopy case.

I even tried for more than 80 minutes but eventually failed at the first trial.

He was thin and tall.

His stomach was very big.

The colon seemed also very very long.

I tried again in 2 hours after failing.

Another endoscope was used.

And I succeeded.

The scope was even shorter than the previous one. However it was easy to insert.

What I have learned is that if all the other methods do not work, it is a good way to start again later....

...with another endoscope.

I found it later that the first endoscope need to be repaired.

(it could not be fully deflexed.)

This is the main method I use when passing SDJ.

There will be a loop somewhere.

After entering the D-colon as much as possible, reduce the loop.

Typically about 50 ~ 60 cm can reach SF.

This is the middle T-colon

After passing through the middle T-colon, it can be advanced by pushing in or pulling it with a torque applied COUNTERCLOCKWISE.

(The only point where counterclockwise torque is used)

The excess air still has to be aspirated.

When passing through the HF area, I generally use upward deflexion with clockwise torque.
