Shoulder Line & Armhole Curving For Perfect Drafting Of Your Kurti | English Subtitles - Duration: 5:23.

hello friends, today m going to teach you how to do prefect drafting of shoulder line and armhole curving

for this, take fabric and fold kurti fabric in 4 layers equal to half of bottom width + 1.5 inch

and after that do simple drafting of kurti

first, at top take half of shoulder to shoulder length + 0.5 inch widthwise

at this point take shoulder line lengthwise, and at shoulder line take 1/4 of chest + 2 inch widthwise

after that, come on waist line and here take 1/4 of waist + 2 inch width wise

after that, come on hip line, here take 1/4 of hips + 1.5 inch width wise

and in bottom, we take half of bottom width + 1.5 inch width wise

like this I did the drafting of my kurti, now in this we take half of neck width at top

neck width is 5.5 inch, so its half will be 2.75 inch, and from here we make 0.5 inch slant shoulder line like this

this is because our front neck portion is a bit on higher side and shoulder line goes inclined as our shoulder are in inclined manner

so this is our half inch slant shoulder line and after this we come to armhole curving

for this, first we mark two straight lines lengthwise

one line is equal to half of shoulder to shoulder length + 0.5 inch, this will be a straight line

after this, on inner side means towards neck area, we take 0.5 to 1 inch less mark and join upper point to this in slant manner

this is because we normally put our arms on the front side and do all the work

we very less frequently take our arms to the back

after this we do curving on back line for back armhole curve

and on front line, we do front armhole curving

in drafting we did the same, on back line back armhole curving is done

and we took margin of 0.75 inch and did our front armhole curving

by doing this, the drafting and fitting of kurti comes perfect

while doing cutting, keep in mind that first cut slant shoulder line and then do back armhole curve cutting till bottom

after that, separate one layer of fabric which will be our back layer

and after that we do front armhole curve cutting

That isn't just any bus.

That's a Double Decker bus!

Look, there's a downstairs AND an upstairs!

I'm just waiting at a bus stop for the next bus to arrive.

All you have to do to catch a bus is put your hand out like this, and the bus will stop.

This is Brian, and he's the driver of this bus.

He sits in a place called the cab.

Here it comes now!

Brian presses this button, and the doors fold open!

This bus is special because it can move up and down to let people get on more easily.

Red mechanical, where've you been on this bus?

You've been playing in the junkyard?

Oh well, I hope you had fun!

Come on!

Let's get on board.

You can fit up to 75 people on this double decker bus.

I think I'm going to sit upstairs for the lovely views.

Here we are back at the Bus depot.

I'll just press the bell to ask the driver to stop.

Shall we have a closer look at the controls here in the cab?

The driver can press all sorts of buttons to make things happen.

This button controls the sign on the front of the bus, which tells people where the bus

is going to.

This is the ticket machine

And these screens are connected to cameras so the driver can see the passengers upstairs.

These buses travel all over the city, so they sometimes get very dirty.

Shall we put this double decker bus through the special bus wash to give it a clean?

It's time to use the water and brushes, To clean our double decker buses!

Through this truck wash our bus will crawl, Have you ever seen a bus so tall?!

Look at that, clean as a whistle!

Where do you think the engine is in this double decker bus?

Surprise!

It's here right at the back, and these buses are special because they run on electricity

and diesel.

When the bus is going slowly and picking up people from bus stops, the bus uses an electric

motor.

This makes it much quieter than other buses.

Just be careful not to fall asleep on your way home!

But even these buses need to be repaired sometimes.

Instead of bringing them to Gecko's Garage, they are brought here to the Arriva Maintenance

Garage where expert mechanics can repair them.

Look how many buses are being worked on at the same time.

This bus is having a wheel changed!

And Here's another bus driving into the garage.

It drives in and parks over a big hole in the floor called The pit.

If there's something wrong underneath the bus, a mechanic can go down into the pit,

and fix anything whilst standing underneath!

OR they can use a giant hydraulic lift to lift a double decker bus up and make it even

taller!

When everything is fixed on the bus, it's time to leave the garage and go back out on

the road to take more passengers where they need to go.

I've loved learning all about double decker buses today.

General Ventilator Set-up, by Nancy Craig.

Hello, I'm Nancy Craig.

I'm one of the Respiratory Therapists here at Children's Hospital in Boston, and I'm

here to review some of the general setups of an ICU ventilator.

Ventilator Preparation.

This first chapter will demonstrate how to attach the components of a ventilator, prior

to patient use.

Most of the ventilators will have some sort of an expiratory filter for exhaled gases.

The humidifier that all ICU ventilators will have to warm up the gas, delivers humidity

to the patient.

The tubing is either disposable or non-disposable.

If it's disposable, it will be thrown away after each patient use.

If it's non-disposable, it will be sent to be cleaned.

The inspiratory end of the ventilator is connected to the humidifier.

The expiratory end is connected to that expiratory filter.

And there is a piece of tubing that goes from the inspiratory end of the ventilator to the

top of the humidifier.

The circuit then will require a temperature probe for use with the humidifier, which monitors

the temperature back at the ventilator, and then close to the patient's airway.

Now I'm going to show you how to turn the ventilator on and hook it up to the patient.

All ICU ventilators -- I'll turn it around here -- require a power source.

Most of them do have batteries, but the power supply is fairly limited.

And they all will require a high-pressure oxygen source.

This particular ventilator has an air compressor, so it doesn't necessarily need a high-pressure

air source.

If your ventilator does not have a compressor, for it to function you'll need a high-pressure

air hose as well.

The oxygen and the air get hooked up to a 50 PSI source.

The power knob is generally on the back of the machine.

High-pressure Leak Test.

When powering up a ventilator, it will typically ask you to perform a high-pressure leak test,

so that we make sure that the ventilator is good to go before placing on a patient.

I'm going to perform an EST, which is a high-pressure leak test, to make sure that the tubing has

no leaks.

The ventilator instructs you to make sure that you're not on a patient, that your wye

is capped, and it will prompt you to proceed with the high-pressure test.

Here, the extended system test is checking for leaks in the circuit; it's performing

a circuit compliance test to see what the distensibility of the tubing is; and it's

also performing a test of the oxygen sensor.

If there are any leaks in the circuit, then the ventilator will not pass its EST.

If it does not pass its extended system test, then we'll systematically go through and see

where the sources of leaks may be.

Those leaks may be in the humidifier, they may be in some of the connections to the ventilator,

or it might be that the leaks are in the circuit itself.

So there's lots of places where pieces are put together that could be prone to leaks.

Once your ventilator has passed the test and you are assured of its integrity, then we

can continue on.

This particular ventilator asks you to input a patient weight, so that it can figure out

some calculations related to tidal volume.

Initial Ventilator Settings.

After the patient is intubated, there's a period of time where the patient is manually

ventilated and pressure is monitored so that you can have a sense of how much pressure

it takes to visibly move the chest.

So here, it's taking about 25 cmH2O to move the chest, so that is where I will start my

ventilator initially.

I have chosen a pressure control mode for this patient that's just been intubated, and

I'm going to set some, just sort of, initial settings before placing the patient on the

ventilator.

So on this ventilator, typically we start -- depending on the disease state of the patient

-- at 5 cmH2O of PEEP.

To set my pressure, I'm going to -- as I said, it took about 25 cmH2O to move the chest.

On this ventilator I'm setting a change in pressure; so I'm going to start here at 25

cmH2O of PIP and 5 cmH2O of PEEP.

The breath rate is generally based on the size of your patient.

This is a smaller patient, so I'm going to start with a higher rate, something like 18

breaths/minute.

Inspiratory times, again, depending on the size of the patient, should be in the range

of about 0.5-0.8 seconds.

My patient is not breathing, but I'll add a little bit of pressure support ventilation

just in case there is a spontaneous effort.

And that amount of pressure support is generally initially based on the size of the endotracheal tube.

I will set a flow trigger, here, so that the ventilator and the patient are interacting

with each other, in terms of spontaneous breathing.

And after a patient is initially intubated, it's fairly typical to start the fraction

of inspired oxygen at 100%.

So now I'm ready to place the ventilator on the patient.

End-tidal CO2 Monitoring.

Ventilator is ready to go; I'm going to actually attach the end-tidal CO2 monitor next.

This is the cable to monitor end-tidal CO2, and this is the adapter that gets fit onto

the airway.

Depending on the patient's size, you'll have a different size end-tidal CO2 monitor.

The adapter is then positioned between the ventilator circuit and the airway of the patient.

The end-tidal CO2 monitor is placed in line with the ventilator circuit to not only non-invasively

monitor end-tidal CO2, but also to monitor the integrity of the airway.

When I'm monitoring end-tidal CO2, I'm looking at not only the value itself, but also the

shape of the end-tidal capnogram.

So I should see a nice exhaled wave that flattens out at the top, to know that my patient is

fully exhaling.

If there is a sudden loss of end-tidal CO2, that will alert me to a problem with the integrity

of the endotracheal tube.

The first thing I would do is to make sure that the ventilator components are hooked

up to the airway.

If they're hooked up to the airway and I still do not have an end-tidal CO2 tracing, then

the next thing I would assume is that I've lost the endotracheal tube from the patient.

Initial Patient Assessment.

When I initially place the ventilator circuit on the patient's endotracheal tube, I'm looking

immediately for chest rise.

Here I'm seeing the patient's chest is visibly rising, and then I come back to the ventilator

to look at some of my parameters.

I would like to set a goal for mechanical breaths of about 8-10 cc/kg.

So that is where I'm monitoring here.

So I'm just a little bit below my target, but looks like the patient is doing okay right now.

Ventilator Alarms.

Once I have established the patient on the ventilator and I'm happy with the way the

patient looks, then I'm going to set my alarms on the ventilator.

The alarm screen is accessed here and these alarms are fairly typical of all ICU ventilators.

I'm going to want to set a high respiratory rate so that I am alerted to a change in the

patient's condition, in terms of how fast they're breathing.

Typically -- the patient is not breathing now -- you would set that alarm at about twice

what the respiratory rate is.

So here I'm going to set it just around twice of the rate of 18. The next very important

alarm limits are the low and high minute ventilation alarms.

I'm going to take you back to this screen.

And remember that minute ventilation is a calculation of respiratory rate times tidal

volume.

So on this ventilator, I will set a low and high minute ventilation to alert me to some

change in the patient's condition, in terms of monitored tidal volumes.

Then I'm going to set it roughly half for the lower limit and twice for the upper limit.

Then it's very important to set a low peak pressure in case there is a leak in the circuit,

and the pressure does not reach what is set, then the alarm will alert you to a low pressure

situation.

And this I set somewhere between the PEEP and the peak inspiratory pressure.

And then the next alarm is the high peak pressure.

This is a very important alarm as well because this will alert the clinician to a change

in peak pressure that's high -- maybe an obstructed airway or a lot of secretions in the tube.

If there's any reason that the peak pressure is higher than what you have set, you would

want to troubleshoot that condition.

That alarm is typically set about 5-10 cmH2O above your monitored peak pressure.

And then I'm going to set an apnea interval.

The apnea interval is going to be based on the size of your patient.

So if your patient is not on a rate, and breathing all on their own, then I would want to be

alerted to a time when there is a pause in spontaneous respirations.

On a bigger patient, I might set it around 20 seconds.

On a smaller patient, I might set it closer to 10 seconds.

So if my apnea interval alarms, then I know that there's a period of time that the patient

has not made a spontaneous effort.

On this ventilator, if the apnea interval alarms it will switch to back up ventilation.

I have a low PEEP alarm, which alerts me to a potential leak in the circuit, and I'm not

maintaining a baseline pressure.

So I will typically set that about 2 cmH2O below my set PEEP.

The other two alarms I set are low end-tidal CO2 and high end-tidal CO2.

A low end-tidal CO2 may alert me to a situation where the patient's status is improving, or

they're breathing faster and their end-tidal CO2 is a little bit lower.

Or if they're not getting an adequate amount of ventilation and their end-tidal CO2 rises,

I would want to know that as well.

Assessment and Monitoring.

There should be an assessment of the monitor parameters and alarm settings at least every

3 to 4 hours.

So on this patient, I'm in a pressure control mode, so I'm going to very closely monitor

the exhaled tidal volume parameters.

And I'm going to set my alarms to alert me if my tidal volume parameters are lower.

In a volume-targeted mode, then I'm going to closely monitor my peak inspiratory pressures

because that value will change, and I will closely monitor and alarm for a high peak

inspiratory pressure.

Depending on the severity of the patient and depending on the frequency by which you're

making changes in the parameters on the ventilator, will determine if you would be doing an assessment

more frequently.

In addition to the parameters that I'm monitoring on the ventilator, when I do an assessment

of the patient, I'm going to be monitoring breath sounds, the integrity of the airway

in terms of its patency, and where it is fixed on the patient in terms of airway position.

And the patient will be continuously on a monitor monitoring heart rate, blood pressure,

and I'll be looking at the oxygen saturations as a guide for my FiO2.

Disassembling the Ventilator.

When the patient is extubated, the ventilator is powered down and disassembled for cleaning.

Any non-disposable parts, like the temperature sensor, are sent down for cleaning.

The disposable parts are disassembled and thrown away, and the ventilator itself is

wiped down in its entirety with a cleaning solution before it is assembled again, ready

for another patient use.

This concludes our session today on the general assembly and set-up of a mechanical ventilator.

how to hold your breath for longer hypoxic swimming trainning - Duration: 2:52.

The main reason why you feel the urge to breath is because there is an excess of carbon dioxide

in your lungs and blood.

This is why you feel weak and sometime dizzy after you hold your breath for a long time.

The other reason is the lack of oxygen.

On this video we will see what you can do to help increase your lung capacity.

Before you attempt to do any breath control, I'm going to remind you shallow water blackouts

are more common than they should be, so please be careful.

If you have a breathing condition such as asthma be extra careful and consult your doctor

before doing any hypoxic training.

Increasing your lung capacity happens naturally when you exercise on a regular basis.

If you follow a workout program such as our Swordfish program you will increase the strength

and function of your muscles.

The more efficient they become the less oxygen the will require to swim and the less carbon

dioxide the will produce.

The heart is also a muscle and if you train it it will become stronger and you will be

able to hold your breath for longer because of better circulation.

It is also important to breath correctly when you exercise.

Let's do a little experiment right now.

Prepare a stop watch.

Take a breath and hold it for as long as you can and pause this video.

Welcome back! write down your time.

Now we will take a breath correctly before holding it again and beating your time.

Diaphragm breathing is the way to go.

When you push your diaphragm down you give your lungs more space to expand and thus you

can inhale more oxygen.

Try that right now and see how good it feels.

Prepare the stop watch again.

Stand up or sit up straight and inhale pushing your belly outwards first and then expanding

your chest.

Inhale as much air as you can, hold it and start the stop watch.

Write down your times in the comment section below.

Ok now that you know how to breath properly and you/ll exercise regularly what's next.

excess carbon dioxide and lack of oxygen in your blood.

Coaching for Performance - Meet my Books - Saba Imru Mathieu - Duration: 3:22.

Hi! I'm Saba Imru from Leaders Today. In this series of short videos

I introduce you to the books on my bookshelf, about coaching and about

leadership and other interesting topics. I hope you'll be inspired to read them too.

Today I've chosen a book which is an introduction to coaching, it's a bit of a

classic. Let me show you.

It's called "Coaching for Performance" and it was written by Sir John Whitmore in 1990.

John Whitmore was a precursor of coaching. He brought it from the United

States to the UK and introduced it into companies into organizations and created

really the Coaching Culture or the Coaching mentality, the coaching approach

in Europe. When he first published his book, there were very few books about

coaching and there was even less research on coaching. So it's written

from a practitioner's perspective. On the one hand he actually goes into what is

coaching and how do you coach, so he's got a chapter on goal setting, one on

effective questions, one where he presents the GROW model of coaching

which maybe some of you have heard of, which was initially developed by Graham

Alexander a decade before, but it's John Whitmore that made it famous. And he talks

about what being a Coaching Manager means and the positive impact that this

new approach could bring to the workplace. He was very concerned by the

fact that people were unhappy at work, that there were blame cultures, that

there was an excess of stress and where people weren't fulfilled doing their

jobs, so he really promoted and advocated for coaching as a means to have a new

type of more democratic leadership and a replacement for the command and control mode.

So you'll find that his book is interesting, it's easy to read, it's fun

and it's also inspiring. So let me read you one of my favorite

quotes from this book. And if you buy the book you'll find it on page 168, at least

this edition that I have. He says that "I suggest that the evolutionary journey of

our species has reached the stage at which the hierarchies of the past are

due to be replaced by a new form of devolved leadership and collective

responsibility". And he considers the coaching approach as a fundamental

aspect of this new type of leadership. So, well, I hope you enjoy reading it and see
