Waching daily Nov 27 2018

Here's some testing that can be useful

for histamine intolerance.

Hi. I'm Dr. Chris Masterjohn of chrismasterjohnphd.com.

And this is Chris Masterjohn Lite,

where the name of the game is

"Details? Shmeetails. Just tell me what works!"

And today we're going to talk about some

testing for histamine intolerance.

I'll link to the last episode in

the description of this one, where I

describe histamine intolerance in a lot

more detail. But briefly, it means that

you suffer from diarrhea, or headaches, or

any symptoms that are reminiscent of

allergies in response to the histamine

content of the foods that you eat.

And one thing we might want to know in

something like this is, is it histamine

intolerance, or is it allergies, or

is it something else? And in particular,

could it be that you have an increased

burden of mast cells, which are one of

the main types of cells that release

histamine. Now, food allergies that cause

these types of symptoms are generally

classical food allergies, where the

standard testing that an allergist would

use would be the right approach to

identifying those food allergies.

In terms of histamine intolerance, the two

tests that are useful are serum diamine

oxidase activity. Diamine oxidase is an

enzyme that gets rid of histamine. It's

the main enzyme present in the gut, so

it's the most relevant enzyme when

talking about tolerating food histamine,

and blood levels of serum diamine

oxidase, or DAO, are low in histamine

intolerance. Histamine intolerance could

be mimicked by mast cell activation

syndrome or an overburden of the cells

that produce histamine. You could have a

chronic excess of histamine because of

the excess mast cell burden, and the food

coming in provides more histamine that

fills up your histamine bucket and

pushes you over, or you could have mast

cells accumulating, for example, in the

gut where something in the diet is then

activating them to release the histamine,

but that burden of mast cells is the

actual problem more so than your inability to clear

histamine in the gut normally.

You can look for the excess burden of

mast cells by looking at serum tryptase.

Now, serum tryptase is a very easy test

to get. LabCorp has it. Quest has it.

In general, most doctors would be able to

easily order it. Serum diamine oxidase

activity is not easily available. As far

as I can tell, the major laboratories

that I would think to use in the

United States do not carry it. There's

a laboratory Medichecks, based in England,

that does carry diamine oxidase activity.

If you happen to know of other labs that

offer diamine oxidase activity

commercially, please let me know in the

comments. So it might not be realistic to

measure diamine oxidase activity, but

measuring serum tryptase activity could

be very useful in the sense that if

serum tryptase is elevated, it indicates

an excess burden of mast cells, and that

indicates two things, number one, that

when you're thinking about nutritional

approaches, you want to focus on ones

that may reduce mast cell burden instead

of focusing on ones that may increase

diamine oxidase activity, increase

your ability to clear histamine. And then

number two, that might indicate something

very important about why those mast cells

are there that your doctor—that

you can then work with your doctor to

try to address because if you have an

inflammatory burden somewhere, that might

be an indication of another disease

process that needs investigation. Now, in

terms of the nutritional approaches to

stabilizing mast cells, or reducing mast

cell burden, or to supporting histamine

clearance, I'm going to deal with these—

I'm going to deal with these

successively in a series of episodes

that will culminate in an episode that

ties it all together and all the

different nutritional approaches that

you could use. So stay tuned for future

episodes for that. Now, if you look in the

literature, many people advocate using

more advanced testing that is even more

impractical. For example, many will argue

that the gold standard should be taking

samples of your intestines to look at

your intestinal expression of diamine

oxidase or even other enzymes that are

involved in histamine metabolism, and

some would advocate as the gold standard

giving you a blinded histamine challenge

where you are exposed to histamine doses

that have been added to tea, for example,

versus a placebo to see

whether your symptoms respond

specifically to the histamine.

These are deeply

impractical in most cases, but I think

the serum tryptase measurement,

and if you can find it, serum diamine

oxidase measurement, although they might

not give you the absolutely definitive

answer to what's going on,

they can certainly point you in the right

direction and be useful.

Yellow dust from China expected to blanket S. Korea for two days from Tuesday - Duration: 1:36.

A massive sandstorm has swept through parts of China... with dust carried by strong winds

blanketing many cities across the country.

It's expected to impact Korea as well, prompting local authorities to issue an ultra-fine dust

warning.

Seo Bo-bin explains further.

Beijing and other regions in China are choking under severe smog.

As of Monday, a yellow alert for smog was issued in Beijing and other neighboring regions

in China.

South Korea has also been affected by the smog coming in from China and the concentration

of fine dust has gotten thicker.

According to the Korea Meteorological Administration, the smog was triggered by yellow dust originating

from inner Mongolia,... which had traveled through northeast China.

As of Tuesday afternoon, ultra-fine dust warnings have been issued all across the nation here

in Korea.

Ultra-fine dust levels have been rising throughout the day and the concentration levels are very

high.

Ultra-fine dust is more harmful to the respiratory system as the particle size is smaller than

ordinary fine dust.

"As the yellow dust adds to the fine dust already in our nation, concentration levels

will become even higher and will be extremely harmful to our body.

The fine dust levels are expected to increase and last until Wednesday."

As northwesterly winds come in, ultra-fine dust levels are expected to rise throughout

the day and yellow dust will remain in the forecast until Wednesday morning.

Children, the elderly, and people with respiratory disease have been advised to stay indoors.

U.S. considering reunion for N. Koreans, U.S. relatives: Report - Duration: 1:00.

A U.S. State Department official has reportedly suggested the possibility of holding the very

first family reunion for Korean-Americans and their relatives in North Korea.

On Tuesday, Radio Free Asia cited a representative of the National Coalition of Divided Families,

a civic group, as saying a senior official mentioned the possibility during a phone call

earlier this month with another group.

The State Department official reportedly told the group Divided Families USA... that Washington

has shifted its North Korea policy and made family reunions one of its top priorities.

Also, the official reportedly said, the reunion could happen before what was called the next

"nuclear talks," believed to be a reference to the second North Korea-U.S. summit expected

to place next January.

According to the civic group leader, the reunion will likely happen via video or phone, and

that an initial list of those who wish to meet their relatives in the North will be

delivered to the State Department.

More than 100-thousand Korean-Americans are believed to be separated from family in the

North.

In this video we wanted to show you our water filter.

That is this thing here. We think it is very handy, not only for van live but also

for hiking, anything where you need to get drinking water somehow.

Our problem was, that we arrived in India and

for some reason we couldn't get filtered water anywhere.

Also no big water bottles or containers with drinking water.

So, for the first days we had to by 1 L bottled water.

That was when we stated thinking about getting a water filter ourselves.

We wanted something simple, that we can use in the car.

We had heard of big filter systems in camper vans.

But we had never met anyone who used a small and handy water filter.

A filter that would also run without electricity.

Then Steffen found this one on the internet.

And we wanted to show you how it works and how to

test if the filter is still ok and how to maintain it.

And what we put on as an extra.

We used to have a small inline water filter

but that one didn't work good at all.

It was very difficult and time consuming to use,

so we ended up not using a water filter at all. Until we found this one.

This water filter pumps 10% of the water in a circle, so it is cleaning itself while pumping.

That is also why the filter hat two pipes. One pipe get the water to the filter,

the other pipe pumps the 10% for cleaning back.

And that's how the filter cleans itself.

The pores of the filter are so small, that they are supposed to filter bacteria and viruses.

Until now we didn't have any problems with the filter, we never got sick, so we believe that this is true.

We have been filtering India tap water, never had any problems.

In front of the fridge is our main water tank.

It can carry up to 22 liters. Steffen made two wholes into the water tank.

So we can use the filter easily.

It is very handy, because we can reach everywhere while cooking.

We can also reach the passenger seat, so we can filter water while driving.

Also here in the back, we can reach almost everywhere.

And here is the tube of the water filter inside the water tank.

I will close the tank for now.

It is very handy to have a water tank with a big opening.

It makes the cleaning very easy, especially when you filter a lot of water, you have to clean the tank every now and then.

Because the 10% of the cleaning water in the filter goes back into this tank.

That is why especially the last few liters are getting more and more dirty.

Therefore, the last liters in the water tank are not the best anymore.

That's why it is good to clean the water every now and then.

Before we had the water filter, we used

This tap, it has always worked very well. we never had any problems with it, no leaking, nothing.

then we also have 1 very cheap water tank,

it fits 10 l , we used to have 2 of these.

When it was so hot and we didn't have many water sources.

We used these one to refill our main water tank.

So we were able to carry 40 l drinking water around with us.

But now we gave one of the small water tanks away.

Because we have many water sources now and we didn't need both anymore.

We use the 10 l water tank to refill our main tank,

because we cannot move our main water tank so easily.

Here we have a holder for the water tank.

This one. Fits ok.

Steffen has also made a third whole into the water tank, it a whole for the air.

So the air can get in the filter. We didn't want the water to spill out all the time, that is why we added this black tube to it.

The filter is supposed to filter 10 000 l of water medium quality.

We always try to get water as clean as possible.

This doesn't always work, but usually it does.

There are two things we don't like about the filter:

We can conned the filter directly to the bottle.

We don't mind this - this we actually like a lot

The problem is, if you turn the water filter too tight,

It is very easy to tighten the filter too much.

If the filter is too tightened.

Then the filter is leaking.

Then you have to loosen the filter a little bit again, it can be a bit tricky to find the right spot.

If the filter is too loose, it will also leak.

When this happened for the first time, we thought that the filter was broken.

And we just had used the filter for 2 weeks only at that time.

Then Steffen twisted around a little bit, at one point it stopped leaking again.

So we found out, that we can just loosen the filter a bit ourselves.

You are not allowed to tightened the filter too much.

It needs to be a little loose.

Maybe you can show that we can twist it now in both directions.

Yes, it needs to be a little loose. If this happens to you, it probably doesn't mean that

the filter is broken, simply loosen it a little bit

and then it should be completely fine again.

The 2nd thing that we didn't like to much about this filter was the taste of the water.

We filtered a lot of tap water, so the taste of the water wasn't so great.

The filter is a mechanic filter, not a chemical filter. That is why the taste of the water is not getting improved.

That is why we got an activated-carbon-filter in addition.

We build this one ourselves.

So it fits on the bottom of the MSR-Filter.

We can simply screw the activated-carbon-filter on the MSR filter.

We can also still use the lid, so no water can come out.

This works fine, but we had to build this ourselves.

It would have been cool if we could have bought a custom made version of this.

But there is nothing available, is there?

No, I couldn't find anything.

So this version fits, and works.

But it would have been way easier if we could have simply bought an

activated-carbon-filter that fits on the MSR filter.

How we build it: we bought an universal activated-carbon-filter

but that didn't fit so well, and also we couldn't use the thread for the bottles anymore.

So we ended up building some stuff around the activated-carbon-filter.

Now we are quite happy with it.

It very simple, in the middle is the universal activated-carbon-filter (black) and around that we put 2 Pilid Lids (green).

So it used to be only this black thing, and we put the green things around it.

We used hot glue to connect the Pilid Lids with the activated-carbon-filter.

It works very well. No leaking, easy to use.

We have now two filters on our filter. One filter is the MSR filter, it lasts up to 10 000 liters.

And the hot activated-carbon-filter, which can only filter up to 200 liters,

But changing the carbon-filter is very easy. You just need to open it,

change the granulate, pump one liter of water through, and the second liter you can already drink.

What we always do, so that this one doesn't get loose and we cannot tighten the filter too much.

We just put some duct tape around it.

Now the carbon-filter cannot get lose.

We use normal duct-tape, so nothing special.

So we managed to solve the 2 small things we didn't like about the filter.

We have been using the filter now for 9 months.

And we filtered 3 000 liters so far, so about 10 liters a day.

This includes cooking, drinking, sometimes we use the filtered water to clean the dishes as well.

I think the 10 000 liters last for at least 2-3 years. Even if you use the filter daily.

We have been using the filter also when we went hiking.

We really liked that we can drink the water immediately after filtering.

We have met people that have been using silver tablets,

but with silver tablets you have to wait some time before you can drink the water.

Same with an UV-filter.

And we were able to filter the water, from whatever source, and we could drink it right afterwards.

When we were camping, we just tried to find a place close to a water source.

In the mountains this was pretty easy.

Also, when there were enough water sources, we didn't have to carry so much water around with us.

When we were hiking, we only took these two water bottles with us.

And usually we only had filled the small bottle with water.

The filter is quite heavy, 500 g. So by only filling the water bottles as much as we needed to fill them.

So the weight of the water filter didn't really matter because it gave us the opportuity to carry less water around with us.

Because we could use every source of water and we could drink the water immediately.

Also, we didn't treat the filter too well.

When we were hiking, the filter was always on the backpack of Steffen's backpack.

Sometimes it fell on the ground. And we also were camping once at 4200 m attitude.

The next morning everything was frozen and we had accidentally left the filter outside our tent.

We thought, that the filter might be broken now, because it froze.

MSR says, that the filter is shockproof and freezeproof.

And - luckily the filter is still working! It didn't break through freezing.

We tested the filter to see if there are any damages inside,

and we want to show you now, who to test the filter for damages.

First we need to get rid of the carbon-filter.

This is the actual filter. The plastic needs to be taken off? -yes.

Then we need a water bottle.

The bottle needs to contain water filled up to 1/2 or 1/3.

We took off the carbon-filter and screw our water bottle on the filter.

We also got rid of the water connections.

Now we need to pump, until there is no water coming out anymore.

Takes some time.

Now you can see how the water is leaving the filter.

Ok, great. Now there is only air left inside the filter.

Now we have to block the upper whole, so we can pump the filter full with air.

If there is a leak somewhere inside the filter the air will go through the filter and escape in the bottle, so we we will see bubbles.

We keep the upper whole closed, and pump 10 times.

Now we have to wait 30 seconds.

30 seconds are over already?

If there was a leak, we would see bubbles here in the bottle. But our filter is still OK.

This is the filter.

Wow, it is quite dirty.

Through these little pipes the water gets filters.

It only outside. So it's no problem if this part is dirty.

Inside the pipes is not allowed to be any damages or dirt.

So it's no problem that it is so dirty from the outside?

No.

Because the filter is still clean from the inside?

It's because we know, that the filter doesn't have any leaks, we just tested it, so the dirt cannot go through.

When the filter is blocked or broken, it's easy to exchange the filter.

You can just get it out and buy a new filter cartridge.

Simply put the new one into the MSR Case.

If you notice, that your filter is leaking at some edges around here.

Or if it gets hard to pump water through the filter.

Then we take the filter apart and put silicone on everything.

We have done this 2 times in 9 months so far.

So we have the water filter and 2 bottles of water, they are totally enough.

We use one 1,5 l bottle and one 1 l bottle.

We try to use only one water bottle at a time.

Because when we drink, bacteria are getting into the bottle.

The water can get off quite fast.

So we try to limit that to one bottle, so water is running though faster.

Also, we have looked at how the Indians are drinking. They are very clever.

They drink like this:

Its very good to do it like this, especially if you use your water bottle very often in a day - and with 2 people.

It works very fine. Oh - I forgot to press record.

Still, we have to clean our water bottles often.

We do that with a toothbrush, sometimes a bit of soap,

the bottles can handle heat so sometimes we use how water.

After a Wildfire, Rebuilding Life Can Be Hardest for the Oldest

CHICO, Calif. — The deadliest and most destructive wildfire in California history, the Camp Fire, was finally brought under control by firefighters on Sunday after raging for 17 days. But for many people left homeless by the fire, the most frightening part of the disaster is just beginning.

The hardest-hit community, Paradise, Calif., was a popular place to retire, with more than one-quarter of its residents 65 or older, according to census figures. Many of them have now lost everything late in life and must start over from zero, often with little support and with major health challenges.

After a battle with throat cancer, Daniel Cayer wears a tracheostomy tube, breathes through a hole in his neck and speaks through a device pressed to his cheek. He had been living alone in a trailer park in Paradise, and had planned to celebrate his 72nd birthday with a barbecue on Nov. 8, the day the fire started. But a plume of black smoke that morning scotched those plans, and soon the flames were in the trees near his home.

He had to go — but where?

Cramped emergency evacuation shelters can be dangerous for older people with health conditions like Mr. Cayer. Infections can spread quickly, like the norovirus outbreak that sickened nearly 150 fire evacuees last week.

But Mr. Cayer said he had no family to turn to instead; he hasn't spoken with his daughter in 15 years, he said, nor with his sons in 30 years. So he's been sleeping in a camper outside a local church shelter in Chico, the first place where many fleeing Paradise have taken refuge — and where, because of the fire, the air quality has ranked among the worst on the planet.

"Most people would put on a mask," Mr. Cayer said through his speech device. He pointed to the hole in his throat. "I can't."

Many of the thousands of structures in Paradise and surrounding parts of Butte County that were lost in the fire were nursing homes, assisted living facilities, other geriatric care centers or mobile home parks catering to retirees. Roughly 2,300 residents of the fire zone had relied on in-home health aides, according to Shelby Boston, the county director of employment and social services.

"There were a lot of people who were barely able to make it at home, and those are the people that tend in situations like this to make their way to the shelters," said Dr. Andy Miller, the public health officer for Butte County. Some patients with memory issues and the early stages of dementia have had their conditions made worse by the chaos of evacuation and by losing their daily routines, he said.

Coping with the aftermath of such a disaster can be daunting enough at any age, but for those in their 70s and 80s, it can seem almost impossible. "Half of them don't have it in them to start all over again," said Mari Stewart, the nursing supervisor for the clinic at the East Avenue Church in Chico, which has been turned into a shelter.

Disasters can go on taking a heavy toll long after the initial crisis has passed, experts say. "Being cut off from your medicines, being cut off from your medical care, being cut off from your social supports, the added stress of having lost things and living in an evacuation center and having to relocate — studies clearly show that there are additional hospitalizations that occur, even deaths that occur, in the months afterward," said Dr. David Eisenman, the director of the UCLA Center for Public Health and Disasters.

When older people are displaced, he said, "for them to not be allowed to rebuild and live in their community again might produce a kind of harm that's much greater than if you or I or younger people had to rebuild and move away."

By the time the Camp Fire was declared contained on Sunday, it had burned more than 153,000 acres and destroyed more than 18,000 structures, almost 14,000 of them residences, with hundreds more damaged. The death toll for the fire stood at 85.

Most of the 500 evacuees who have been in and out of the East Avenue Church shelter since the fire began have been older people, said Ron Zimmer, the pastor of the church. The challenge, he said, is not just caring for them, but finding long-term answers for people who have lost everything and have little wherewithal to rebuild.

Younger families "tend to have larger social circles," Mr. Zimmer said, but "our seniors tend to have closing social circles, so they don't have anywhere else to go."

After reaching safety, the first problem for many older evacuees was being without medical supplies and medications they depend on. The valve on Mr. Cayer's trach tube must be cleaned and replaced multiple times daily, for example, but he had to leave most of his essentials behind in Paradise; now he worries about his access to oxygen and about getting sick.

Doctors, nurses and volunteers at the emergency shelters worked to locate and contact the evacuees' primary care physicians — many of whom had to flee the fire themselves — and get hold of pharmacy records or other information that would help them replace the missing medications quickly.

"I had to beg like a dog to get my medication," said Dorothy Melton, 69, who is staying at the church shelter. She said she was taking prescription drugs for anxiety and high blood pressure, "but I don't know what it is."

Joy Reich, a Paradise resident at the Neighborhood Church shelter nearby, needed immediate attention for respiratory problems when she arrived. "I have asthma," she said, "and that smoke and asthma did not mix well. It's been awful."

For Ms. Reich, who said she was "82 going on 150," rebuilding in Paradise is not an option. She is moving to Seattle to be closer to her son, she said. But nothing can bring back the family birth and death certificates she had, dating back three generations, or the book she had spent 25 years writing, inspired by her mother and children. She had made a point of keeping the pile of loose-leaf manuscript pages — almost ripe for publication, she said proudly — by the front door, in case of an evacuation. "But I just freaked that day," Ms. Reich said, and forgot it on her way out.

Without a relative to turn to, Mr. Cayer said he plans to stay at the shelter in Chico as long as he can, and then return to the trailer park in Paradise to see what might be left of his life. But it is not yet clear when that will be.

Mr. Cayer would like to purchase a new trailer and set it up again in the same lot, he said, but he had no insurance on the old one, which cost him $32,000. And moving back into a world of ash and rubble would entail substantial health risks, especially for someone with breathing issues, as Mr. Cayer well knows.

All that he has left, he said, is his turquoise 1929 Ford Roadster hot rod, which he managed to tow through the flames as he escaped.

"I have nowhere to go, I have no family," Mr. Cayer said outside the church shelter, where he has spent his days in the parking lot polishing the car and tuning its 400-horsepower engine.

"That's why I had to save this," he said. "It's my baby."
