Know the Cholesterol Level That's Right for You — And Stick to It

HIGHLIGHTS Get your cholesterol tested at least once every five years. If you have high cholesterol, that puts you at risk for heart disease, stroke, and peripheral arterial disease.

Your personal cholesterol goal depends on other risk factors you may have, like smoking, diabetes, and high blood pressure. .

The National Lipid Associations recommendations for how clinicians can help patients control their cholesterol levels focus on setting a personal goal based on your overall risk for cardiovascular disease.

Heart disease is the number one killer in the United States, and lipids — fats that circulate in your blood — are the primary cause.

Recommendations published in the September 2014 issue of the Journal of Clinical Lipidology outline patient-specific ways to lower specific risks for potentially fatal cardiovascular events.

We spoke with NYU Langone Medical Center's James A. Underberg, MD, director of the Bellevue Hospital Lipid Clinic in New York City and president of the National Lipid Association (NLA), about what the recommendations mean for patients and their families.

Everyday Health: According to the latest from the NLA, how often should you have your blood levels of cholesterol checked?. Underberg: For adults 20 or older, a good rule of thumb is every five years.

Everyday Health: When the blood test results come back, if your doctor tells you that you have dyslipidemia, what does that mean for your future?.

Underberg: The way to understand it is as an abnormality in the lipid profile that puts you at risk for vascular disease or stroke. It can be abnormal triglycerides, not just cholesterol.

The first step is to find out that there's a problem. The second is to find the cause. Everyday Health: What are the cutoff numbers that are considered normal for lipids?.

Underberg: We describe these as desirable, rather than normal. Desirable levels of LDL cholesterol are under 100 milligrams per deciliter (mg/dL), and non-HDL cholesterol of under 130 mg/dL. These are the numbers to aim for.

In addition, HDL should be above 40 mg/dL for men, and above 50 mg/dL for women. And triglycerides should be below 150 mg/dL. Everyday Health: Does a particular level mean you might have inherited high cholesterol, also known as familial hypercholesterolemia (FH)?.

Underberg: The number that the NLA uses is an LDL cholesterol level above 190 mg/dL. This is in agreement with the American Heart Association and the American College of Cardiology. If your LDL is over 190, you need to be concerned about FH.

Everyday Health: Does this mean the other members of your family should get checked, too?. Underberg: Indeed, family members should get checked. Obviously your kids should get checked. Just as important, your siblings and your parents should, too. Call them up and get them checked.

Ask them to get tested by someone who knows about FH. I say that screening should be north and south, east and west — across the family tree and across the United States.

Everyday Health: Why might your blood cholesterol target number, or goal, be a different number than another person's?. Underberg: We decide based on two things. One, the inherent level of risk; and two, what the blood cholesterol level is.

If cholesterol is low, but you have many risk factors — like smoking, diabetes, high blood pressure, or other heart disease — then your cholesterol goal is lower.

On the other hand, if you have high cholesterol but fewer risk factors, your goal may be higher. It is not just putting the information into a calculator, but making it personal.

Everyday Health: If cholesterol levels don't reach the goal, how do you know when it's the right time to give medication a try — and what do you start with?.

Underberg: We always like to give lifestyle interventions a really good try. Not for a few weeks, but for several months. If after three to four months the goal isn't met, it may take reinforcement. See what's working and make some changes.

But if after one or two attempts you're still not at goal and cholesterol is significantly above where it needs to be, then we recommend medication. Statins are the first line of therapy. But not everyone can take a statin.

A woman thinking of getting pregnant would not want to be on a statin. And a person who had side effects from statins in the past also wouldn't.

But in some cases, you may require more than a statin, and may need another medication added to control cholesterol. RELATED: 9 Hidden Heart Toxins and How to Avoid Them.

Everyday Health: What are the risk factors within our control that, together with fat in the blood, increase the chances of having a heart attack or stroke?.

Underberg: Some things we can't control, like our age and our family medical history of heart disease. But the ones we can control include inactivity, smoking, obesity, and poor diet.

These are in our own control. Sometimes treating other conditions may be helpful, too — for example if you have diabetes or high blood pressure. We need to think about the entire patient.

Everyday Health: What are the lifestyle changes NLA recommends to keep cholesterol levels down?.

Underberg: We really focus on lifestyle, and have a second document coming out on diet. In the current recommendations, we focus on those interventions that have been shown to lower cholesterol.

These are a diet restricted in saturated fat (found in butter, lard, meat, full-fat dairy, and coconut oil), weight loss, and referral to a registered dietician. If you also have diabetes, you'll do better with a low-carb diet than a low-fat one.

Remember that clinical treatment guidelines are based on large groups of patients in clinical trials. But I still see patients one at a time who may not be the same as the people in those groups. That's why personalization is so important.

Korea to expand playground for all children - Duration: 4:31.

A place where time fly by as children interact and make new friends while having fun.

Unbeknownst to many of us, the common recreational features are unreachable to some.

On our news feature tonight Lee Unshin introduces to us a new wave of playgrounds that are more

inclusive, catering to the needs of kids of all abilities.

From the 1st grade to the 12th,... students at Myunghyun school are thrilled.

An outdoor playground with a sandbox, a swing, and spring rides... has arrived at last.

(Korean) "For outside activities, we take the students

hiking on the nearby mountain so they can build up their strength, or go to museums

or art centers.

But I'm so happy there's now an outdoor play area for them right here."

The school provides special education for kids with learning difficulties.

Most of some 120 students here have neuro-developmental difficulties... but there are a few who carry

physical disabilities... and for them, enjoying the rides won't be the same as it is for their

friends.

(Korean) "It breaks my heart that the students who

are in wheelchairs or walkers can't quite use the standard swings or slides like the

others."

For many, a playground is something that they grew up with, an adult-free zone to meet new

friends and play outdoor games,... and it's often taken for granted.

(Korean) "For any kid, to play is to live.

A playground is a necessary place for their social, emotional, and cognitive growth, along

with physical and coordination development through the equipment."

For that reason, Children's Grand Park in Seoul has opened a playground that's designed

for kids of all abilities to enjoy.

(standup) At a glance, this playground isn't that different

from any other one.

But when you look closely you'll notice small details, such as this wide open pathway instead

of stairs, leading up to the slide, so any child can either walk or climb up in a wheelchair.

The slide is wide enough for an adult and a child to ride down together.

There's a merry-go-round with a corner to secure a wheelchair,... basket swings that

are spacious enough to get on lying down... and a sandbox with a place for kids in a wheelchair

to come in and play.

(Korean) "This playground was built through extensive

research and lots of trial and error.

As a result its features are very safe and kids really seem to enjoy themselves here.

This is what a playground should look like, companies came here to benchmark the model.

But coming to the park can be quite a hassle, especially for those who have physical disabilities

who come here by car."

The lack of THIS kind of kid-friendly space across the nation is linked to legal matters.

Korea's anti-discrimination act for persons with disabilities and children's safety regulations,

don't specify anything about playground features for kids with disabilities.

Legal guidelines to deal with accidents or insurance problems are needed for such equipment

to be made.

(Korean) "This is long overdue by lawmakers.

So I proposed a modification act to take care of any incomplete legal or regulatory matters,

so that any child, handicapped or not, gets to enjoy the playground.

"

Kim's proposal includes providing support for the production of recreational equipment

for kids with special needs,... with the required budget backed by the government.

Something that various institutes, including Myunghyun school, have long waited for.

(Korean) "To purchase such equipment, schools, especially

private ones like ours are often met with budget issues.

If the government can appoint companies to manufacture them it will bring down prices."

So that no child is left out of the joy of playing outside.

The move is expected to lead to more inclusive recreational grounds like the one at Children's

Grand Park,... an area for all kids to just enjoy being a kid.

(Korean) "When working on the design of the playground,

I made sure the features are feasible for them, but at the same time just as enjoyable

for others.

And I wanted to highlight how adjusting small things from the conventional playground can

accommodate all children, abled or disabled."

Hi! I am Milana.

Hi! I am Arty.

We are MilArt and this is our IndieGoGo campaign

for "Snow Queen Waltz"!

So, what is MilArt? MilArt is a duo!

A duo of Milana and Arty, a wife and a husband,

pianist, composer, singer-songwriter

and her producer.

What is "Snow Queen Waltz" you might ask?

Let us tell you this story!

Milana has been playing piano for over 30 years.

I've been an audiophile for two decades.

Five years ago we started our shared musical journey.

Together!

For these five years

Arty kept asking me to write a Tango song.

So last December I accidentally did it!

When I heard it for the first time

I was literally bewitched!

Don't get me wrong!

Milana wrote incredible songs in the past!

But this one, this one was something special!

Something from a different league!

It was inspired by Rachmaninoff and Piazzolla.

So, the song turned to be a fusion of

a passionate Tango and Jazz Waltz.

I immediately heard it in my head:

a full orchestral arrangement for this song.

And I visualized a theatrical story, a fantasy:

a reality turning into a dream,

a dream turning into a reality.

We spent Winter and early Spring

writing all the additional orchestral lines.

It was a great challenge to take the original piano

and almost completely rewrite it.

We were lucky to have friends

with orchestrating experience

who helped us all the way.

The vocal part was also a test for me:

I used to sing jazz but this song required

a fusion of jazz and opera.

I had to take several vocal lessons from a great coach

to master the drama and the power of operatic singing.

The next big thing was to write the script for the video.

It was also something new to us: to take the story

and turn it into the list of specific scenes:

second by second.

Some compromises had to be taken

between what we wanted

and what we could afford.

Scouting for filming locations was also a big part of it.

We literally drove hundreds of miles

checking different halls and churches,

and banquet venues.

And we ended up renting a theater,

which may seem like an obvious choice

for producing a theatrical song.

We filmed the first part of story already

and it was such an overwhelming experience.

With Arty's attention to details

we had to shoot some scenes over and over again.

So, it took us the entire day

to film one and a half minutes of the story.

But the big day of filming the main part of the story

is yet to come!

28th of October will be the day

when we will film Milana

with six dancers

and one additional character

playing another role on stage.

and there will be like a dozen or even more

people to help us to make it happen:

a video director, a camera assistant,

a light assistant, stage manager,

a couple of stage assistants, decoration manager,

a choreographer, makeup artist.

Many-many people to participate in this project.

And we'll have three different stage designs

to film three minutes of the story.

And we'll have only one day to film it all.

This day will be the most challenging part

of the entire project:

both scheduling and budget wise.

This is where we need your support!

What I like the most about this project

is that this is a truly collaborative one.
