right now on Monday evening the second of July 2018 well we see a little

tropical swirl going on down here in the Gulf right off the panhandle of Florida

this is set for 9:00 p.m. tonight could this turn into tropical storm Beryl I

don't think so but let's just check it out it is

showing up from the National Hurricane Center this is the European model will

move up to around midnight tonight we see what it's looking like here just a

bunch of rain doesn't have any wind not very much at all 10 20 mile-per-hour

winds that's what it's looking like right now anyway and we'll put it back

and we'll move up to around Tuesday around 8 a.m. tomorrow looks like it's

got more rain in it lots of rain down around Mississippi Biloxi going up

through Louisiana of course I'm spilled as out here does it have any wind by

midnight tonight well Chuck are on out still doesn't look too bad at all as far

as the wind goes maybe 15 to 20 knots okay so just think about 25

mile-per-hour wind nothing nobody cannot handle we'll move it on up a little

further do you 3:00 p.m. tomorrow afternoon we see more confection has

started with the rain with the rain looking like Mississippi right down in

here is going to be getting some heavier showers from it checking the wind still

no big deal with the wind that is a good thing we'll move up a little further now

to around 7:00 p.m. tomorrow night let's zip on up to midnight the fourth of July

well there it is it's coming into Louisiana this is just 8:00 a disturbed

section of confection okay it's just a little confection going on with these

the humidity the thundershowers the way they're popping up the heat

everything else is too close too short to actually form into anything or that's

what it's looking like right now now as of 1:30 this afternoon from the National

Hurricane Center this system is showing up as you can see right here and what

does it say about a disturbance number one looking kind of puny guys as of 2:00

p.m. this afternoon a large area disorganized showers and thunderstorms

extending from the north central Gulf of Mexico across the western Florida

Panhandle and I've been telling you guys the Florida Panhandle is going to be

getting rain right well this is from the disturbance right here now it's

associated with a broad area of low pressure and associated surface trough

coming up from the Caribbean now close proximity the land should prevent

significant development of this system just like I told you I hadn't even read

this yet folks okay before it moves westward and inland over southeastern

Louisiana tomorrow at some time regardless of the development the system

is forecast to produce locally heavy rainfall and you saw all of the

confection building just a while ago now this rainfall should be along the

portions of the northern Gulf Coast during the next couple of days so

anybody in that area you're probably going to have a rainy fourth of July

okay and additional information you know if it comes about I'll put it out but

the chance of anything for me 10% two days five days whatever and in five days

it still got it hanging around this area right here but I think before five days

is over it will move on up into this area so you guys just be aware from this

Sippi to Alabama to Louisiana look out for some rain it doesn't look like it's

going to have much wind where that nothing that you cannot handle nothing

that you are not used to but you could get some very heavy downpours maybe some

flash flooding coming up some of the confection in those areas that we just

looked at look like some of it was pretty heavy I'll keep my eyes on and

keep you informed okay but that's what's happening with this could it become

tropical storm Burrell I don't think so but I'll keep my eyes

just under three weeks because basically what happens on a V ro is if you tag

Zack Snyder who will literally like everything and he's been putting some

images which are very cryptic which could imply that he's gonna be at San

Diego comic-con and maybe the snide cut he's gonna be released there so there's

been a lot of speculation about the possibility of Warner Brothers

confirming the release of Justice League this coming at the kind of comic-con

which this channel is basically being built around that Justice League movie

so I want to start bringing some Justice League content back if there was so as

we know Zack Snyder had to leave the project of Justice League for some

reason so this was said by a very reliable person Zack isn't the kind of

director who creates the film in the editing Bay it's all planned out

meticulously from the beginning to the end

that's how Man of Steel and Batman vs Superman he then continues saying what I

mean with 99% is that all the scenes that Zack envisioned was shot and exists

in one form or another of course there's still special effects that is unfinished

but I infer that it was 99% done rather than Zacks of vision or reshoot exempt

for reshoot plan so what this means is there is 100% a Zack Snyder cut that

pretty easily could be finished so at one point another what happen is Zack

Snyder would have presented this to some massive panel Warner Brothers and they

were said yeah cool or no we don't like this not this is probably what

reinforced the reshoot so it's not a case of does the Snyder cut exist

because I've seen some videos I had a little search for I made this video and

people saying the Zack Snyder cut does it exist I mean no offense but it

definitely exists because he was original director and it was his movie

so it's not question of does it exist the question is is it actually going to

be better than what we seeing because it's alright

saying release its Nautica release the sciatica but what happens if we release

the sciatica and then the final product is actually worse than the kind of joss

whedon edit reshoot edited version I mean I'm all for releasing the Schneider

cut but if that actually has a negative impact then I don't think they should

release sonica and what it's actually interesting in the last 24 hours

might put it in this video it depends if it gets a copyright strike but if you do

see I'm running over top of this it didn't get a copyright strike so

basically what is cyborg seeing he is essentially going to be seeing the

storyboard that Zack Snyder posted online the dark side dream you know this

is a massive hint of the dark side dream of the you know of the mother box and

everything are overlaid over top if I can but it's interesting that the closer

we get to San Diego comic-con they're more hints the more information that

keeps leaking out now the other issue is is it gonna be any good and will we ever

see it because although I don't work in Hollywood sadly because he shot it

principal photography would pretty much be his property but I don't know legally

if Zack Snyder could actually publish it without Warner Bros consent now

ahmud Walter I believe that's how you say his name the head guy at DC now I've

been hearing and read some things that rudely he's seen a snide cut but then

you have the issue of the new kind of vision that the Tron envisioned with the

Aquaman movie with the Shazam and obviously with one one - does that fit

in with the release and the Snyder cut because releasing the Snyder cut means

they have to do dioxide at some point now yes Zack Snyder did work at one one

one one one - yes he is credited for Aquaman not Shazam but eventually if

they do build the universe up like they say they're going to just making good

movies then we could actually get a proper Justice League one day

and you never know we might actually get the Man of Steel to release that

actually going to make it as Henry Cavill has been out quite a lot recently

talking about he's just needing to mix the deal and

kind of make everything official you never know Zack Snyder might be heading

to comic-con because he's gonna go yeah I'm directing - still - and that would

be absolutely insane because regardless - infinity war regardless - all these

other movies Man of Steel is still my favorite comic book movie but there are

some pretty cool scenes from man Batman vs. Superman I mean the Batman fight he

has in the warehouse is absolutely insane regardless to what you think of

that movie that fight scene is absolutely stunning so guys let me know

are we gonna see the Snyder cut very soon and do you think the sonic cuts

actually going to be better than the original cut and do you agree with me

that the cyborg vision is actually of the Darkseid storyboard that's someone

released I don't know if it was Zack Snyder but there was a scene there I

Any patient with symptomatic arrhythmias

that cannot be treated with medications

is a good candidate for cardiac ablation.

Arrhythmias like atrial fibrillation,

supraventricular tachycardias, extra beats,

and ventricular tachycardias are arrhythmias

that can be treated with cardiac ablation procedure.

And any patient who cannot tolerate medications,

does not want to take medications

or is symptomatic despite taking medications is a candidate

for cardiac ablation procedure.

A Holter monitor is a small monitor

that's placed on the chest either through wires,

or with just a tape on the chest.

So if we don't have good documentation

of what the arrhythmia is,

or if the patient has symptoms that are not long lasting,

enough that they can come to the ER

to get an EKG when they have the symptoms,

we put this monitors on.

They can go home with this monitors, wear them at home.

They can use a symptom activator to record their symptoms.

And so we can correlate their symptoms

with the actual arrhythmias,

or the rhythm on the monitor itself.

So we can tell the patient this is what

your symptoms are from.

WATCHMAN Device is something

that we use in patients with atrial fibrillation

who have a high risk of bleeding.

So a patient with atrial fibrillation

has high risk for stroke.

Now, the risk of stroke depends

on different clinical characteristics,

but a lot of these patients also have bleeding issues.

Now, somebody may have a GI bleed,

they may have blood in the urine,

they're not stable on their feet and have risk of falling.

Now, obviously, you don't want

these patients to be on a blood thinner.

So, WATCHMAN is something that's indicated

in that subgroup of patients

where we can decrease their risk of stroke

and, at the same time, we can also

decrease their risk of bleeding.

A majority of these patients,

after six weeks to a year of being on a blood thinner

don't have to take blood thinner

for the rest of their lives.

At the same time, not having their risk of stroke increase.

The human heart has four different valves.

Valves are like doors between the rooms in your house.

Mitral valve is the valve between the left top chamber

of the heart called the left atrium

and the left bottom chamber,

which is called the left ventricle.

So when the left bottom chamber of the heart squeezes

blood out of the heart,

all the blood should go through the aortic valve,

but in patients who have mitral regurgitation,

when the left bottom chamber squeezes blood,

some of the blood leaks back into the left top chamber

through the mitral valve.

Now, that leak could be mild, moderate or severe depending

on how bad it is,

but patients who have severe mitral regurgitations,

they can have symptoms because of the leak.

Dr Nannapaneni on What is a Cardioversion - Duration: 1:02.

A cardioversion is a procedure where we shock the heart

back into normal rhythm.

So when the heart is in an abnormal rhythm

like a-defibrillation, and the patient's symptomatic

from it, and they're feeling their palpitations,

or they're just feeling light headed,

they're feeling maybe chest pain or fatigue,

we make them come in to get this cardioversion procedure

so that we can shock their heart back into normal rhythm.

Sometimes this is done in the ER as well,

so when patient walks into the ER

and they're in an abnormal rhythm,

and if their blood pressure is too low,

and the ER physician thinks that it's that they may not

be able to tolerate this rhythm for a long time,

he will shock them and bring them back into normal rhythm.

The process of doing the cardioversion itself

takes about 10 to 15 minutes.

But the preparation before and after can take up to an hour.

The heart primarily has four valves,

and the aortic valve is one of the valve.

I call it the front door of your house.

So the blood has to cross the valve,

and then it gets outside the heart.

Aortic stenosis is narrowing of that valve

where it does not open fully,

so now the blood that comes out of the heart,

it comes at a higher pressure,

which means the heart has to work harder

to squeeze the blood out through a smaller opening.

This puts stress on the heart

and can cause a lot of symptoms in the patient,

and it can also have implications

on survival for the patients.

When you have a patient come in

with congestive heart failure,

you can get the early indications

that it may be mitral regurgitation

by doing a physical exam.

You can hear a heart murmur,

and depending on the nature of murmur,

you can tell if it is mitral regurgitation or not.

But, you know, a definitive diagnostic test

is an echocardiogram, which is the ultrasound of the heart.

And it can clearly show us if there is mitral regurgitation,

how bad it is, and also the mechanism

of mitral regurgitation.

A defibrillator is similar to pacemaker

in some ways, but it also has a shocking function.

So if the patient has a very fast, abnormal heart rhythm

that can make them have a cardiac arrest,

the defibrillator will shock them

and put them back into normal rhythm.

It looks like this.

It's about that big, and it goes in the chest

in the same position as a pacemaker,

just below the clavicle underneath the skin and the fat.

It also has two or three wires

that will go through the heart,

and one through the top right chamber,

on through the bottom right chamber,

and some patients may need a third wire

that will go through the bottom left chamber.

An ablation procedure is a procedure

where we cauterize a small area of the heart

to get rid of an arrhythmia.

So what we do is we typically bring the patients in

on the day of the procedure.

We put them to sleep for the procedure.

It could be light sleep or general anesthesia,

depending on how complicated the procedure is.

We put up catheters through the groin.

There are big blood vessels that go

from the groin to the heart.

Through those big blood vessels,

we put in catheters up to the heart.

Sometimes even go to the left side of the heart

if we have to do the procedure

and cauterize a piece of tissue

to get rid of the short circuit or the trigger

that's causing the arrhythmia.

Cardiac arrhythmias are abnormal heart rhythms.

They can be from irregular activity in the top

chambers of the heart called atrial fibrillation.

It can be from short circuits in the heart

like atrial flutter or supraventricular tachycardias.

It can just be from focal fighting in parts of the heart

and those are usually extra beats

and it can also be from scar in the heart

like a ventricular tachycardia.

The symptoms that people commonly experience

with these are palpitations,

shortness of breath,

chest pain, they can feel lightheaded,

they can feel like they're about to pass out.

They may actually pass out from these arrhythmias.

It may require ER visit.

A pacemaker is a small device

that helps control the heart rate.

If the heart rate is too slow,

it will help pick the heart rate up

so that the patient can have a good, underlying heart rate.

It looks like this and it goes right below the clavicle,

usually on the left side.

Underneath the skin and the fat,

we make a small pocket for the device itself.

It will have two wires that go to the heart.

One goes to the top right chamber

and one goes to the bottom right chamber.

The procedure takes about an hour to do.

We send patients home with a remote monitor

that can communicate with the device

and can transmit information to us.

Dr Yaganti on What is a MitraClip Procedure - Duration: 1:09.

MitraClip procedure is indicated for patients who

have severe symptomatic mitral regurgitation.

It is only considered in patients who are

considered to be high risk for open heart surgery.

Currently it is approved in patients who have an STS score

above 6 per cent for mitral repair,

or above 8 per cent for mitral replacement.

STS is the score model that surgeons use often to

determine what is the risk of someone

dying during the open heart surgery or

having complications during open heart surgery.

There are other reasons why you could also consider

patients who do not meet that criteria

if they are very frail, if they have bad lungs,

if their heart function is poor and there may be issues

coming off an heart lung machine,

those patients can be considered for MitraClip procedure.

So a typical evaluation would include a patient seeing me,

and then seeing the surgeon, and if the surgeon

is of the opinion that the patient has high risk

of undergoing open heart surgery,

then we evaluate them for MitraClip procedure.
