welcome back to dr. maclean explains my goal is to break down the US healthcare

system into understandable parts to clarify what is the problem with the

system and to talk about how to fix it in segment 4 we showed some

statistics on relentlessly rising healthcare spending and in segment 5

we looked at the perfect storm how rising costs are literally built

into the system as it is now in this segment we will ask the question so why

is it so bad for health spending to increase after all health care is good

and Noble by the way it also creates good-paying

jobs my answer is that health care spending is good of course but only up

to a point past a certain point it is not worth the cost

it sucks money from other important purposes and it fuels social and

political unrest let's look at each of these arguments first are we getting our

money's worth? here's the slide from the last lecture

showing the benefit from each additional dollar spent this is what economists

call marginal benefit if you've already spent a thousand dollars what more do we

get by spending ten or a hundred more dollars

the answer is we get less and less the more we spend on the left side is the

column of highly valuable health services like public sanitation and

immunizations for very few dollars spent we prevent disease and save lives

the next column moving right our routine services that give good value for the

money spent like kidney dialysis chemo for treatable cancers and heart bypass

up to age 70 the next column are lower value services like keeping

dying patients in uncomfortable ICU beds then we reach a cut point where we get

no added benefit from extra spending what economists call the flat of the

curve this is unnecessary testing treatments

or drugs which are wasteful here is a list of examples doctors and specialists

have recently got together to identify some worthless habits of care that can

be eliminated they call it the Choosing Wisely campaign and are trying to get

all doctors to break the habit of getting these here are some other

examples the last column of the graph shows that sometimes giving too many

treatments or drugs actually can be harmful as a geriatric specialist taking

care of the elderly I can tell you that over my years of practice I have

definitely cured more people by stopping unnecessary drugs than by over

prescribing the bottom line is that most experts agree that the u.s. is not

getting any more bang by spending more bucks and in some cases actually doing

more harm than good the next reason is that health spending is taking money

away from other equally important purposes this is what economists call

opportunity costs by spending an extra dollar on health care we are missing the

opportunity to buy better education business investment housing or

infrastructure the third reason is budget squeezes healthcare is driving up

Medicare and Medicaid for example which increases the federal budget deficit it

also squeezes state's share of Medicaid costs driving up taxes it squeezes

corporate expenses cutting into investments and employee pay this also

hurts us competitiveness abroad and drive some jobs overseas the last reason

I'll give is its effect on take-home pay I showed this graph in segment 3 the

Rand think-tank calculated that between 1999 and 2009 Americans were 30% more

productive and got 30% more wage compensation but all of that in

went into health premiums shown in red not into take-home pay shown in blue

health care costs are making us feel poorer I think people are finally

getting angry about so much health care spending and that's one big reason they

voted for Donald Trump hoping he could do something dramatic to fix it

I am worried that left unchecked soaring health care costs will cut into defense

spending drive up the national debt and ultimately weaken us as a nation and so

what is the conclusion the conclusion is that exorbitant spending is the problem

health spending eventually leads back into your wallet and my wallet and our

grandchildren's wallet in the future in the next episodes we will discuss how to

stop the excess of spending this will require us to fix the whole health care

system not just tinker with insurance Obama care markets and Medicaid we will

also look at some of the ethical issues and the political challenges involved

and I'm here to talk to you today about Arts and Economic Prosperity 5

our new economic impact study of nonprofit arts and culture organizations

and their audiences

Arts and Economic Prosperity 5 changes the conversation about the arts

from one of Arts as a charity to the idea of arts as an industry

one that promotes quality of life and provides economic benefits to the community

Arts & Economic Prosperity is the most comprehensive study of its kind

ever conducted we studied 341 regions across the

country in all 50 states plus DC the bottom line in communities large and

small the arts make an economic difference to their community

we focus only on nonprofit arts and culture organizations we didn't include Broadway

we didn't include the motion picture industry or individual artists all an

important part of the cultural ecology just not part of this study

Why only nonprofit?

well if you look at where most charitable giving goes where most

government investment in the arts goes it's going to those nonprofit cultural

organizations and it's appropriate to ask the question

In addition to improve quality of life and more culturally rich communities what's the ROI on that investment?

and that's what we've set out to find out in this study

What did we learn?

Nationally nonprofit arts and culture industry is $166.3 billion dollar industry

that's billion with a B

That figure is composed of two numbers

spending by arts organizations

as well as the event related spending by arts audiences

Spending by the organization's themselves

$63.8 billion dollars

That's a mythbuster for a lot of people!

We all love the arts, right? they bring us joy they make us feel creative

"True Detective" season 3 with Mahershala Ali is 100% a go

After strong, strong indications that HBO would indeed be restarting its once-brilliant, then-troubled crime anthology franchise "True Detective," the network is now confirming that, yes, the show will be returning to the small screen and will, yes, star Oscar-winning actor Mahershala Ali.

In addition to Ali's casting there's word that the show will take place in the Ozarks, the mountainous, often economically challenged area that spans Arkansas, Missouri and Oklahoma.

Specifically, Ali — who took home an Academy Award for Best Supporting Actor for his performance in 2016's "Moonlight" — will play a police detective in Northwest Arkansas.

"True Detective" season 3 will mark the second major premium drama to choose the area as its setting. The Netflix series "Ozark" also takes place in the area, though much of the filming for that show was done elsewhere.

Entertainment Weekly reports that the logline for the third season reads, "The next installment of 'True Detective' tells the story of a macabre crime in the heart of the Ozarks, and a mystery that deepens over decades and plays out in three separate time periods.

HBO also confirmed that "True Detective" creator Nic Pizzolatto return to directing duties alongside "Green Room" and "Blue Ruin" helmer Jeremy Saulnier.

While many hang the creative and critical failure of the series' second season around the neck of Pizzolatto, who was without season 1 director Cary Fukunaga, the inclusion of Saulnier is welcome news.

Both "Green Room" and "Blue Ruin" are bitingly gripping films — frightening, taught stories with backwoods settings and strong performances. The alignment here seems dead on.

HBO also confirmed the involvement of "Deadwood" and "NYPD Blue Creator" David Milch who will pen at least one episode of the upcoming season.

Following the aforementioned troubles with the second season of "True Detective," the network seemed to put the show on a shelf for the last two years.

While chatter about its return was always present, the longer the series lingered, the lower its chances of survival seemed to be. There is no set premiere date as of yet.

This surge of violent crime — for example, we need to turn back the recent surge in

violent crime.

The increase in violent crime and murder rates that Attorney

General Jeff Sessions is referring to happened between 2014 and 2015. And while

it was the single largest increase since 1991, the crime rate still remained lower

than it was in 2012. Across the country, violent crime and murder rates are lower

than they have been in almost 45 years. Sessions clearly knows this. Look at how

he lauds Miami for reducing its crime rate.

Police regularly recorded upwards

of 500 murders a year and the city was in danger of crumbling, but the people of

Miami-Dade refused to tolerate this violence and last year the homicide

count was barely a third of what it was in the 1980s.

Yet, he doesn't use the same

reasoning for national violent crime. A one-year increase does not qualify as a

national trend. And so for all his dramatic assertions, Sessions is just

stoking Americans fears about crime. For this, he earns Four Pinocchios.

That's according to his Brazilian team-mate Neymar. The playmaker has been heavily linked with a move to Barcelona in recent months. Coutinho, 25, has handed in a transfer request at Anfield in hope of forcing through a switch to the Nou Camp.

The La Liga giants have had three bids rejected for Coutinho this summer. And Neymar, who left Barca for Paris St-Germain last month, says the Liverpool star is unhappy at Anfield.

Asked about Coutinho following Brazil's victory against Ecuador last night, Neymar said: Unfortunately, instead of being a happy time for him [Coutinho], for his family, its a moment of anguish, disappointment and sadness.
