How PRP Alone is Proven to Stimulate Hair Growth, but Remains Unpredictable and can be Improved - Duration: 11:23.

Thank you for your question.

You submitted a question without a photo and you're asking this question, is your PRP

working. And you describe in the text of your question that you had suddenly noticed significant

hair thinning. And that you went ahead with the PRP treatment and then you underwent a

second one. And then at the time you submitted this question, you had undergone a third one.

You feel that your progress photos do not show improvement. In fact, it appears that

you were getting worse. So you're also asking should this be working out this way, will

this PRP stimulate hair growth, should it already have an effect on your hair growth.

Well, I can certainly help you understand how I counsel patients who have hair loss

in my practice and give you a sense of where this modality has a role and in what kind

of strategy is employed. A little bit of background, I'm a Board-certified cosmetic surgeon and

Fellowship-trained oculofacial plastic and reconstructive surgeon. I have been in practice

in Manhattan and Long Island for over 20 years. Hair transplant and managing hair loss has

been a very big part of my practice. In fact, I'm the founder of TrichoStem™ Hair Regeneration

Centers, a system that we developed starting about 7 years ago using PRP and Acellular

matrix to help men and women with hair loss, typically genetic pattern hair loss non-surgically

and leveraging this concept of stem cell based restoration of hair growth going into details

further.

To begin with, the current state of knowledge and slow but steady acceptance of PRP as a

modality for hair loss, I feel like there are still a lot of questions that do remain

unresolved and in medicine, that is often the reality. There are many things that clinically

have benefit where at the basic science level, it is still difficult to fully explain the

cause and effect and predictability. So the art of clinical medicine of course is to treat,

observe and extrapolate based on information and experience.

Well, to go back to what we did with our procedure Hair Regeneration, approximately 7 years ago

when we were doing a lot of transplants, we were using a combination of platelet-rich

plasma and Acellular matrix which is a wound healing material to help improve the graft

survival as well as improving the healing of the donor area, both of which we were always

challenges of hair transplant. Well, a beneficial side-effect was that thinning hair appears

to grow thicker. And that thinning hair that grew thicker occurred long after the procedure

was completed and the material was placed.

So, we took it upon ourselves to explore the opportunity to see if we can have a process

where we can treat hair thinning with a non-surgical approach rather than allow the hair to go

away and then keep doing transplants. Well, to make the long story short, we were very

successful with this. We developed Hair Regeneration and the company called TrichoStem™ Hair

Regeneration. We treat patients from all over the world and what that has met is that we

have so much volume of patients with a wide range of ages and conditions such that I've

been able to really define different sub groups of people on how they respond. Globally speaking,

that the overwhelming majority of our patients have benefit from this treatment sustainable

for 5 years and that is for both men and women. Now within these groups however, there are

going to be certain groups of patients who have more aggressive hair loss patterns. Generally

speaking, these tend to be younger people who have had an early onset so maybe teens

and 20s and who have very rapid progression.

Now when we think about that scenario, we also have to look at what are the known associated

causes or interventions that are available to medically treat hair loss. Well, there

is finasteride, an oral drug that is a DHT blocker of dihydrotestosterone. And then there's

minoxidil. When we think about hair loss, it's very important to understand that hair

loss is still a progressive condition and regardless of what modalities are being used,

there is no current cure for genetic pattern hair loss. There are strategies to slow it

down and prolong the lifespan of the hair and therefore provide longer coverage as well

as the surgical strategy which is to place grafts of hair in places that need in that

area to provide coverage. So it gets down to basically providing coverage.

Now, one of the things that I find that a lot of people who are relatively new to this

world of stem cell based hair loss treatment is that when you don't really do hair loss

management as a long-term strategy, a lot of times people are taking a shotgun approach.

And I'm not saying this is the situation for you but a lot of people who are relatively

new to this are getting excited because they go to meetings, they see good results with

PRP and they decide that they're going to add that to their practice. And then when

people are not doing well, they shrug their shoulders.

The challenge with hair loss management is understanding really what the likely prognosis

is for individuals with a clinical picture. Now you describe a scenario where you had

suddenly had significant thinning. Well, that is unusual unless it's more of typically

the awareness of significant thinning. There's a difference. There's the awareness of significant

thinning as often occurs when people think they have more hair than they do and then

suddenly they realize, wow, they barely have any scalp coverage or they get a view of the

top of their head from a mirror or a photo or something and then feel like, "Oh, suddenly

I lost my hair." A lot of times it has been progressing along but because their view when

they look at the mirror, they can only see a frontal hairline and they look pretty good,

they don't realize how much hair they've lost.

So that being said, when you think about this type of steady downward progression, I would

say that it's important to at least consider, if you're male, to consider a DHT blocker.

Again, without more details about your clinical picture and a clinical picture that I rely

on is age of onset, current age, degree of hair loss, history of other interventions

and the effectiveness of other interventions. The reality is of course there is only a finite

small number of interventions.

So to answer the question about whether or not the PRP is really working requires a more

global detail of your clinical situation. I think that the one thing that I can add

to this that might be helpful to you to ask your physician is whether or not it is advisable

to start adding DHT blocker like finasteride to try to reduce the effect of DHT on your

hair. If you have male pattern hair loss and it is 95% of hair loss, it's genetic pattern

loss then you might be in a situation where a DHT blocker can help improve the longevity

of the hair follicles and maybe give the PRP an opportunity to actually have an effective

stimulatory effect. Again, this comes from a lot of experience and many patients are

reviewing our photos very carefully again, almost 7 years of doing this and treating

patients all over the world. So I think it's worthwhile to discuss this with your doctor

and learn a little about their experience with medical and surgical treatment of hair

loss and take it from there.

There's three main challenges in logistics in infrastructure in the UK at

the moment for retail. The first is the heavily congested road and rail systems

the second is the land constraints and the lack of land which is available for

industrial use and the third is a skills shortage both haulage workers and

construction workers which is going to be further exacerbated by Brexit.

The government really needs to get to grips with all these issues. They need to

invest heavily in the road and rail systems otherwise the UK is going to get

left behind from the rest of the world - the likes of Hong Kong and Singapore - and

they also need to address the skills shortage both in haulage and construction workers.

Ecommerce in retailing is creating a massive strain on infrastructure

it's both the supply of goods to consumers online and

it's dealing with returns. For example after Black Friday there was five million parcels

that came back to retailers at a time when they were desperate to get them

back into the shops again. Personalisation in deliveries is going to be the big thing

this year in retail. Retailers want to move from a blunt, same delivery option

for all consumers to reward those consumers that shop frequently with them.

That's going to require massive cost and analysis of all that data to understand

how they should alter their supply chain logistics to deliver that personalisation in deliveries.

Dr Emily Landau, a senior research fellow at the Institute for National Security Studies (INSS), said no matter what you think about Mr Trumps leadership, it was not over the top and should have been predictable, given the threat.

The president was widely criticised for riling North Koreas despot leader Kim Jong-un in early August after news the hermit kingdom had successfully miniaturised a nuclear warhead to fit into an intercontinental ballistic missile which they claimed could reach the US mainland.

Mr Trump, tweeted: North Korea best not make any more threats to the United States. They will be met with fire and fury like the world has never seen, he has been very threatening beyond a normal state.

They will be met with fire, fury and frankly power the likes of which this world has never seen before. Dr Landau said his reaction was entirely understandable given the US mainland was being threatened.

On Monday, Mr Trump toned down his rhetoric when North Korea fired three missiles over North Japan, instead of towards Guam as Kim had threatened.

Dr Landau, an expert in negotiations with Iran and North Korea, told express.co.uk: Leaving aside anything else anyone would want to say about the Trump presidency, in many different respects, this particular dynamic is driven by North Koreas nuclear advances.

This is because of what the North Koreans have demonstrated, as far as their ability to strike the US mainland - and thats what started this new chapter.

Its not about Trump saying fire and fury because I can say any US president sitting in the White House in the wake of a North Korean missile test that demonstrates the ability strike the US mainland, you would see a deterrent threat issued by the United States.

I would go so far as to say that it would be bizarre if there was not a deterrent threat issued because its a new stage in terms of the threat that North Korea is posing.

Thats something which the United States needs to respond to. I think that needs to be made clear..

Dr Landau said much is made about Trumps often inflammatory remarks, however, during Barack Obamas presidency North Korea made major nuclear advances and fired four missiles, including two in the last few months before Mr Trump took over.

She added: One of the main things Obama told Trump when he handed the presidency over was North Korea is going to be your most severe security threat. Dr Landau admitted Mr Trump makes it difficult because he elicits such hysteria, but urged people to look at Defence Secretary James Mattis statement which he released a day after the presidents fire and fury message.

She said: Mattis was even clearer in terms of what will happen to North Korea if it thinks about striking the US mainland.

So, yes, fire and fury, but really if you look at the statement and the tone he used I think its a little bit more than Trump.

Trumps was a deterrent threat, we can criticise the specific words but they werent outrageous by any means. Mattis is regarded as a more measured person, he didnt get that kind of attention the way Trump did.

If there was not a deterrent threat issued that would have been the odd thing, that would have been extremely odd. If North Korea starts threatening to strike US mainland - when its the mainland its a different thing.

Take 9/11, the minute there was 9/11 you knew there was going to be a strong reaction by the US, it wasnt only about the horrific way it happened and the number of people which were killed, it was a strike on the US mainland.

You strike the US mainland youre going to get it, and its the same here..

Why Ali's Wedding is an important movie

YOU can thank Claudia Karvan for Ali's Wedding. Without her, Australia's "first Muslim rom-com" may never have happened. Comedian, writer and actor Osamah Sami was on the set of 2009 TV movie Savedwhen he started telling Karvan his life story.

The tale he told seemed so outlandish, Karvan flagged down director and producer Tony Ayres to hear it — a faked medical degree, deported from America for starring in a Saddam Hussein musical, and ran away from his wedding.

Ayres' immediate response was "That's a movie". And now it is. Ali's Wedding, co-written by Sami and veteran scribe Andrew Knight, is out this week and it's a sweet, charming take on all the outrageous events of Sami's young life.

Sami plays Ali, a barely fictionalised version of himself, a young Iraqi-Australian whose father Mehdi (Don Hany) serves as the cleric of their local mosque.

Feeling under pressure from his parents and community, who always expected him to succeed academically, he tells everyone he scored highly on his medical degree entrance exams when he didn't.

He fakes going to uni for a whole year, in part to be closer to Diane, a whip-smart student from his mosque who wants to be a doctor.

Ali and Diane are enamoured with each other but his parents have already arranged a marriage for him with someone else and her father disapproves of her interacting with men.

"Our parents came here under strenuous circumstances, went through poverty and war, to bring us here," he told news.com.au. "We have it so good here in Australia, it's a wonderful country.

So you're expected to study and do well. But it's not that easy. I say that with us, you've got to either be a doctor or a taxi driver.".

Sami's real-life father, who moved his family here when Sami was 13 years old, died unexpectedly from a heart attack four years ago.

Sami wrote about the harrowing experience in his book, Good Muslim Boy, and this film is a tribute to his dad.

Hany's portrayal of Sami's father is one of compassion, empathy and respect. For Sami, watching his dad immortalised on screen was a bittersweet experience, especially when the wounds of losing him are still fresh.

He, almost imperceptively, chokes up when talking about it. "Don Hany does such a splendid, wonderful job that when we were filming, I felt like I was talking to my dad," he said.

"He really absorbed all those beautiful qualities and his performance is imbued with that beautiful, gentle soul dad had.".

Sami said his family is supportive of his creative endeavours — now that he is neither a doctor or a taxi driver — and that he couldn't have made this film without them.

Although mum asked to read the screenplay and she was like, 'I'm not that angry, why do you have me shouting in the film?' I'm like, 'Mum, you're shouting right now'. She's like, 'But you deserve it now'.

And when I started writing it, dad was still alive and he consulted on the script and which aspect of the story I should tell. He said, 'Be true. Be true to the events and be true to yourself.'".

Osamah Sami stars in the film Alis Wedding. Source:Supplied. There are comparisons to be drawn between Ali's Wedding and Pakistani-American comedian Kumail Nanjiani's indie rom-com The Big Sick, which was released a month ago.

Both films are about a personal love story that happened to their writers and male leads.

Both films are also about the experience of a Muslim immigrant in their adopted Western homes and the challenges they face in straddling two cultures and the expectations of their conservative families and traditions.

Like The Big Sick did for American audiences, Ali's Wedding represents a chapter in the our national narrative that isn't just another whitebread, been-there-seen-that story. Especially at a time when the political discourse around Muslim-Australians can veer towards the toxic.

"We're always asked to assimilate in some form," Sami said. "How can we assimilate if we don't see ourselves reflected in the wider Australian narrative?.

"It would be arrogant of me to say, 'this is a crucial film'. That's up to the audience to decide.

"What I do know is that we have permanently etched a slice of the Australian narrative on the big screen and it's going to be there forever. That's important because we are part of the Australian story.".

Ali's Wedding premiered earlier this year at the Sydney Film Festival where it won the Audience Award.

Hopefully, a mainstream audience will also flock to Ali's Weddingto see that the Muslim-Australian community is just as multidimensional as any other Australian community — it's not just one thing or the other.

"It's not just a story about Muslim-Australians, it's a story about Australians," Sami said. "Hopefully everyone see a part of themselves in the film, regardless of their religion or background or colour, because feelings don't discriminate.".
