Person Centered Planning is a set of approaches designed to assist someone to plan their life
and supports.
It is used most often as a life planning model to enable individuals with disabilities or
otherwise requiring support, to increase their personal self determination and to improve
their own independence.
It is a unique process that focuses on a persons gifts, talents and dreams.
This can be around a particular area of their life such as ones future independent living
or ones whole life and looking ahead.
Using a process called Mapping, friends, family and others, including the individual can share
information to build their Person Centered Plan.
Person Centered Planning includes path to action plans, focusing on key questions and
themes, questions such as "who am I?", my history, gifts, talents, what works?, what
needs and fears do I have, are just a few examples.
Inviting people who are a part of the persons life such as friends, family members, neighbors,
school teachers, can all be invited to help with the process.
Finally, graphic recording can show us pictures or words so that all abilities can understand
the plan and that way we honor the whole person.
