The first time can be overwhelming — it's a big deal, emotionally and physically, and so it may be a good idea to avoid, at least to begin with, getting too complicated or fancy.

hi guys this is dr. April McNeill with the Real MED Life sorry has been such a

long time since I've gotten back to you guys with another video but guess what

residency is no joke but I count it a blessing to be able to still be here

today and provide you with this video so guys I survived Inter Year. I am

officially a senior as of July 1st 2017 so I thought that it would be befitting

that I did a video talking about the essentials to surviving Intern Yeaar

so let's get started first things first number one be confident guys I'm telling

you being confident is one of the most valuable attributes of being a resident

physician Attendings like concise firm and confident plans where they find

that a intern is kind of fluttering and it's like well maybe we should do this

they just give you the plan and there's very little educational value in that so

even if your plan is wrong be confident and what you came up in a plan The Attending

can then say well this is why we wouldn't do it this way or yes your plan

was great and this is why we would do this and it allows you to actually learn

and to grow because ultimately you're in residency so that you can be prepared

that if you were Attending somewhere where there is not an academic

institution and you are the final decision maker you want to be able to be

confident in those decisions that you make

number two organization now you are talking to someone

who is extremely organized in my life I should give you a house tour one day but

when I started intern year I felt like I just had a breakdown I

was not doing well I had a call my mentor she helped me get through a plan

of how I can organize my patients and also organize my plan for

them. I think I was just being overwhelmed and I couldn't get my

thoughts together and that's not something that I was used to so I would

say make sure you develop a routine that is good for you most of the time is

pretty standard of the components of that routine you just need to find out

what's best for you to work it so usually you review your patients.... what

are the outstanding to do's ......what's your plan for your patient .......after you guys

round you need to call consults and you need to make sure all your orders are in

.... then you need to complete your notes

and once you get a hang of that you'll be more efficient with your time and

will have more time to do my next point number three you need to study you are

working these 80 hour weeks some of us you know do not have many days off in

between and it's my no longer the world you are going to study for those eight to Ten

hour days of studying.. you might have 15 to 30 minutes a day but you need

to utilize that 15 to 30 minutes well and this is how I would say that you

would do that make sure you actually review the conditions of your patients

first and foremost so that it can help you formulate your plan for them ...I'm

a pediatric so they put out a publication monthly called the pediatric

review they're quick concise tells you what the

current guidelines for these different patient conditions are and you can

review them very quickly...... also your specialty might give you daily prep

questions that you can sign up for and that way it comes to your email or to

your phone you answer a multiple-choice question and then you get to review the

answer very quick in a hurry five minutes or less and then one thing my

program director said that I felt was very pertinent even though it seems hard

to do is that you still need to keep those habits

even on hard months so if you're in that ICU or you're in the emergency

department any of those things that are a little bit more pressing and demanding

on your mind you still need to keep up those

practices and this lends well to when you're about to take boards at the end

of the residency you've practiced this whole time and then also it's just like

again it helps you make better plans for your patients

number four remain teachable when you first come into residency as an

intern you're a bright-eyed and bushy-tailed but you hit the floor and

you are deer-in-headlights because you're like what am I supposed to do my

orders actually matter ......now what if I hurt somebody ....so you get a lot of

guidance at the beginning but then as entire year goes on you start getting

your footing you start getting more confident and then I think are in a

type-A sometimes creeps in and we think that we know everything.... remember remain

humble and teachable there is always something for you to learn ....listen to

your nurses your nurse practitioners your technicians your pharmacists

everyone that's a part of that learning and educational team can help you grow

As a physician by self care I think this is a very important one because we're so

wrapped in actually sleeping and surviving that we forget to take care of

ourselves make sure you actually go get your yearly physical make sure you go to

the dentist because you have to ultimately take care of you so that you

can be in the best position to take care of your patients some of the other

things that you can think of is your mental health a lot of programs have

programs to help us if we need counseling if we just need advice and

tips how to navigate this world also you

should invest in things to kind of pamper yourself or to make life a little

bit easier so I get a massage monthly that helps reduce my stress I prioritize my budget to include that you can also pay for a house

cleaning service pay for a chef or order those home delivery meals so you don't

have to think about what you're going to cook and eat that week ....finally not the

last essential for intern year but one that I would present today

Don't let how you perform gain priority over the purpose of what you do and who

you are.... when your focus is on the health and wellness of your patients you will

perform well....if your focus is always on performing will like it was kind of like

when going through medical school having that gunner mentality when you don't

meet up to your own standards it can cause you to have a major setback in

your mental stability and health because you're always trying to chase performing

well as opposed to chasing growing and developing as a future physician ....guys I

you take care of your equipment not

just during hunting season.

- Just like any device, whether it's your automobile,

whether it's your house or whatever it may be,

your crossbow needs some pre-season maintenance.

It needs to be checked out.

Not only is that smart from a safety standpoint,

but it's also smart from an accuracy and dependability,

you know, standpoint when you get ready

to head to the field.

Then once you get all of that looked at,

and you've cleaned everything up,

and you're comfortable in it,

let's take it out to the range.

Take it out and shoot it a little bit.

Make sure it's still dialed in,

and also, finally, check every one of your bolts

for any type of cracks; any type of tears in there, because

not only can that cause serious damage

to your crossbow, it can cause serious damage

to you by not paying attention to those.

And when you get all that done and dialed in

and you head to the field, hopefully you'll

be able to keep those nerves calm,

because one of those big bucks or a big hog

or something is gonna step out in front of you,

and if you can keep yourself calm,

I guarantee you these TenPoints are gonna

that bolt exactly where you're aiming.

Smoked him.

Smoked him, I'm telling you right now.

Smoked that sucker.

When it comes to the TenPoints, right out of the box

you're dealing with a product that is boresighted.

You know, and boresighted's gonna help you get close.

It's gonna basically, at 20 yards, it's gonna get you

in the ballpark, but as you begin to dial it in

and make those fine-tuned adjustments, you have to

realize that everybody's gonna interact with the

crossbow, interact differently with the scope

on how they position themselves.

So as you expand from 20 to 30 and 30 to 40 and 40

and beyond, each person has to spend time pulling

that trigger and seeing what's going on.

Watching those bolts and studying those groups,

and then make those fine-tuned adjustments

so that they can dial their crossbow in.

One of the newest products

- So I have here the Thompson Center Compass Rifle.

This is the bolt-action rifle by Thompson Center.

Comes in 11 different calibers from 204 Ruger

all the way up to 300 Win Mag.

Some great features about this gun:

each one of these guns comes with a threaded muzzle.

Also comes with thread protector on here,

so if you choose not to put anything on here,

you just keep this thread protector on.

But it has standard threads to allow for muzzle breaks,

and also suppressors if you choose to.

5R rifling is great because it decreases the

amount of fouling, and also bullet deformation,

So this comes with a three-position safety.

So you have the fire position.

We all know what that is.

So, when the gun is actually loaded

and you're ready to shoot, you actually just

make sure this is in fire, and you can pull the trigger.

So you have the center position, which is

actually your safe position.

This allows you to be able to manipulate the bolt,

but you actually can't pull the trigger.

In addition to that, we have the bolt lock position.

Bolt lock position on this gun is a great feature.

I really like it, because when you're in

thick conditions and you're hunting, you actually

make sure that you don't manipulate that bolt.

So you can see the bolt is locked, so if you've got oak

brush, or if you're got some thick trees around you,

you will always make sure that

that bolt is in the downward position.

Some additional features that are sometimes overlooked

on the Thompson Center Compass, that it

comes standard with sling swivels.

It has not just a front sling swivel,

but it also has a rear, and this is great

if you're gonna sling this gun up before you

go to the range or before you go to the field.

In addition, it has a free-floated barrel,

and it's pillar-bedded, and it has a cheek piece.

All three of these things really help with accuracy.

shooting our new Thompson Center Compass,

or any other firearm for that matter,

- I look back over time and I think of all the gunshots

that I've made, and all the noise that's been,

- There's all sorts of little products out there

that really can help you around any camp,

any fishing trip, any hunting trip, or, you know,

picnic, tailgating, whatever, and one of them

that we like to use is Arctic Ice.

You know, they come in several different

basically sizes as well as abilities to chill.

What we do with them, literally out here

at the camp a lot of times, especially in turkey season,

even in early deer season, we're gonna have temperatures

into the 80s and 90s, and it's just really difficult

to keep ice for long periods of time.

Even with the great super-performance

coolers like that we use.

I mean, it's just gonna melt.

Everybody's getting water constantly,

and that's gonna cause a lot of melting of your ice.

So what we do is take all this Arctic Ice

and pack our cooler.

We'll put a couple of them on the bottom

and a couple of them on the side,

and then maybe about eight or 10 pounds

of ice in there, and that's gonna allow us to

have cold water and beverages for about five to seven

days, even under these incredibly hot situations

and conditions that we're often in.

And you can see one of the key things

that I like about it...

Well, actually, it might be tough to see.

As the ice melts that we put in here,

it actually refreezes onto the Arctic Ice,

and it makes a block in here after it's been

in a while, and that's a great key to

keeping all your beverages cold.

Because instead of all of it turning to water

and running out, and we've all seen that

when we open up the bottom of a cooler,

we're basically making our ice last longer.

When we start talking about seasoning a cooler,

people will probably roll their eyes.

"What does that mean?"

You know, you start thinking about seasoning meat.

Well, literally we're seasoning a cooler right here.

We've got a thin line of ice on the bottom,

and we've taken four of the Arctic Ice bricks,

and we've laid them across there.

And, you know, Arctic Ice comes in several different...

You've got the Alaskan series, the Tundra series,

the Chill and Brew.

We've put one of each in here right now,

just so you could see the differences of each one of them.

Now that we've got everything set up in here,

we know we're gonna basically double or triple

the performance of this cooler already

by really maximizing the cold chilling factors

that we can put in here.

So now we'll lay a few sodas in here,

and then we'll put just a little bit more ice

on here, and as that ice melts, because we all

know ice is gonna melt, I mean, it's just inevitable.

That ice will go down and interact with

that Arctic Ice, and it will actually refreeze,

and just it ensures that you're gonna have

the coldest cooler you possibly can for

any beverages that you're looking at,

whether it's an adult beverage, whether it's Kool-Aid,

whether it's a soda, or you're keeping food cold

at a highly remote location for a long period of time.

Now, in some of the smaller situations,

some of our smaller coolers, some of the backpacks

that we use, even when we're shipping meat home,

we'll take this Arctic Ice along with frozen meat

or any other beverages.

Could be lunches, could be snacks or whatever,

and maximize the coolness of just using the product

like this with no ice.

So whether you're kind of doing what we're doing

in a big cooler here, or you've got a little small

cooler where you're trying to keep a picnic lunch,

or a lunch going to work with you cold,
