The Advance Beneficiary Notice is a standardized notice that the provider must issue to a Medicare beneficiary

prior to providing certain Medicare Part B items or services.

The ABN allows a beneficiary to make an informed decision about whether to allow a service

be performed and accept financial responsibility for those services Medicare does not pay.

The ABN lets the patient know ahead of time that the service you are

providing may not be considered medically necessary by Medicare.

The ABN will then also serve as proof that the beneficiary knew prior to receiving the

service that Medicare might not pay.

The Advance Beneficiary Notice is to be used for Fee for Service Medicare patients only.

It is not used for items or services provided under the Medicare Advantage (MA) Program

or for prescription drugs provided under the Medicare Prescription Drug Program (Part D).

You should check with those plans to see what would be appropriate for use.

Please take note of the release date and only use the current version.

Older versions will not be accepted

This most current version is located on the CMS website at www.cms.gov

To locate and download the ABN form; go to NGSMedicare.com and click on provider resources, click on forms then select coverage.

When you click on the form for Advance Beneficiary Notice of Non Coverage a pop up will appear

and ask if you would like to proceed to an external site.

click ok

You will then be redirected to the CMS website; where you will scroll down to the downloads

section and select ABN forms English and Spanish to download.

ABN forms instructions are also available to download for helpful tips.

Gareth Bales agent Jonathan Barnett has called transfer speculation surrounding the players departure ridiculous and stupid. The Real Madrid forward has been linked with a move to Manchester United, after Jose Mourinho hugged and joked about signing the winger last Sunday.

The Real Madrid winger is the winner of three Champions League medals in four seasons. Before Uniteds 2-1 win on penalties over Madrid, Mourinho said to Bale: I cant buy you because you dont talk to me.

The rumours are intensifying, as Madrid boss Zinedine Zidane refused to rule out the Welsh winger leaving the club . The Frenchman said: I hope everyone remains. I want everyone who is here now to stay.

But anything can happen right up until August 31. However, Bales agent remains insistent that the speculation is nothing but talk.

Reals reported £161m bid for Kylian Mbappe sparked further rumours that one of the BBC, Benzema, Bale or Cristiano Ronaldo could be on their way out of the Spanish capital.

The movements of Kylian Mbappe could leave the door open for Bale to leave Madrid.

Los Blancos boss Zinedine Zidane hasnt ruled out a move away from Spain for Bale.

United had a chance of signing Bale in 2007 when the youngster was plying his trade at full-back for Southampton. Instead the Welsh winger moved to Tottenham, where he grew into the forward role.

The Red Devils had another opportunity to recruit the 28-year-old, but were outbid with a then world-record fee of £85m by Real Madrid. The Welshman was deployed as a full-back in his youth, before making the switch to the wing at Tottenham.

Mourinho remains a big admirer of the Welsh forward. Despite spending the majority of last season on the physio table, Bale has still managed to win an impressive three Champions League medals in his four years with the Galacticos.

Regardless of the comments of his agent, Mourinho – who is a great admirer of the forward – will continue to keep both eyes open on the Welshmans movements.

  Spurs have made no signings because Pochettino is happy with his squad – Kamara TOTTENHAM have been quiet this transfer window because Mauricio Pochettino is happy with his squad, according to Chris Kamara.

Spurs are the only Premier League side not to have signed a player so far this summer. And they have an extra £50m to splash after right-back Kyle Walker's move to Manchester City.

But Sky Sports pundit Kamara – known as Kammy – believes there is a very simple reason for the lack of spending. Speaking to Daily Star Online, he said: "Pochettino has been quiet because he's probably happy with his squad.

"They finished close two seasons back, they finished even closer last season.   Chris Kamara claims Spurs havent made signings because their squad is already good.

"So he'll be thinking 'keep this squad together, let's improve'. "Obviously they've lost [Kyle] Walker but I presume that was the manager's decision to allow him to go.

The veteran commentator claimed Jurgen Klopp's lack of spending was down to the sky-rocketing transfer fees. Kammy was speaking to Daily Star Online as part of his Hairy Tales tour of London.
