So, if you don't know me already,
I am a blogger turned web designer,
turned coach, etc. etc. (laughs)
Um, so, I kind of wear all the hats.
I started out as a blogger and I created this blog called Bicultural Mom.
BiculturalMom.com
Um, which now is Bicultural Familia,
I've kind of switched it to being a family blog,
but at the time when I started Bicultural Mom,
I was in this place where we were separated from family.
We hade moved far from Michigan which is my, my home state,
to South Dakota which is a place I've never been, didn't know much about.
I just went there because on paper it looked good economics-wise.
So, my husband I moved there thinking that we
would be able to see there for like a year, maybe two
and then move either back
home to Michigan or possibly to Texas we
ended up staying there six years and it
was a very isolating experience for my
husband and I both and for our children
so I started this blog by cultural mom
is a place where I could talk about the
families but more like where I could
reach out to other people like us who
weren't visible in the community where
we lived and I built a big community
around it big enough that we started
another website so i started a website
called multicultural familia calm and we
all came together all of us bloggers
whether it be mixed race mixed religion
mixed culture we all came together and
talked about our families and our
identities we just shared with each
other so he said learn more about each
other and be closer after a little while
I started getting into web design
completely by accident so I was I was
working on my blog and editing my blog
and making it look pretty and I'm an
artist that hard I've always been a
creative type I remember in college I
also took this flash web design class
which was ridiculous because I don't
even know why they gained that class
because flash was already on its way out
when i was in college taking this course
but you know how outdated college course
dark so I started fixing up my blog
making it look really nice and really
cute and customizing it and then I
started having blogger friends who would
ask me hey can you help me update my
blog can you help me add a really nice
design can you help me create a logo can
you help me add this widget and I just
started helping my friends and at first
I started helping them for free and then
I started having people tell me you know
I don't want you helping me for free I
want you to get paid for your work I
want to ask you to do more and I don't
feel comfortable asking you to do more
if I'm not paying you and so from there
I turned it into a business I was
working with my friends only my friends
I wasn't taking on clients or anything
better trying to get clients I didn't
have a website up yet I just was working
with bloggers friends at that point and
then it got to the point where I started
having more and more people recommended
me to their friends and then their
friends would reach out to me and then
they say hey do you have a website G of
a portfolio so I just started building
those things so they built a website
actually was a page on my bicultural mom
website at first and I listed my
services there and my pricing and all
this kind of stuff how they could
contact me my friend unha dekhe who at
the time was running new latina now she
runs alleppey nothing big so she asked
me to come on and work with her a new
latina so I worked with her as a
managing editor for new latina for a
couple of years then I started helping
her develop latina something big and it
grew from there like at that point I
knew I knew I needed to create my own
website so i use my name and i created
chantilly pepino com so from that point
on once i created my own website it just
kept growing and growing because now
it's like I had this professional
presence it wasn't just a blogger who's
doing this on the side anymore but this
became my business so when hanukkah
hired me on for Latinas think big I
created all the programs i created the
banners like at her first New York event
I remember walking in the door and
seeing my banners that I designed the
programmes that I designed everything
down to the smallest detail that I had
put work into teen on Helicon like all
the work she had put in and we put in
together and it turned into the sea
there was all these amazing women there
it just it brought tears to my eyes and
then get emotional thinking about it
right now because still like latinas
thing big is one of the most powerful
things that have been involved in and i
love that brand and i'm so proud that i
worked on it and that the nothing
missing big logo is something that i
developed and it makes me very proud to
see it grow so i think i'm getting a
little off track here so now i've been
i've been thinking about this for the
past two years but i haven't felt like
i've had the opportunity to move in any
direction so it's not that I want to
stop doing web design or lower design or
copyright or any of those things but I
want the service part of my business to
be the side part of my business and I
want the main meat of my business to be
products and courses so this was my goal
two years ago I had a new baby and then
I couldn't put the time invest the time
into it that I wanted to invest and
moving my business in that direction so
I put it on hold and right now I feel
like I'm at the point where I'm ready to
start moving in that direction again so
my main goal right now is to focus on
building these products more of the
templates ebooks you guys I'm finding
that people really need help getting
started and it's not so much like I
don't have the finances but it's more so
like the fear like not knowing will I be
successful at this if I put myself out
there in this way are people going to
judge me they don't want to fail and
they're scared of failure they're scared
of the unknown so I know how that feels
I've been scared of the unknowns to and
google has been a huge resource for me i
searched google for everything I built
my entire business because of google i
learned how to build a website from
google i didn't know how to do web
design i taught myself web design with
google that I did I'm not somebody who
went to web design school I'm not
somebody who studied graphic arts
although I wanted to I love that idea
when I was younger I wanted to go to
college and my mom told me be careful
and she made me very cautious because I
wanted to go away to an art school I
wanted to learn film & 4
digital illustration and all these types
of things and my mom was like one how
will you afford it and there's just two
what are to get far away from us and
something goes wrong and we're not there
to help you you know three what if you
get your education and you're still not
successful what if you're starving
artists and you don't make any money and
I took a lot of that to heart like I
always thought I could do anything like
in my mind I'm a creative type person
right I was thought I could do anything
and that kind of flipped in some doubt
and I don't think that's what my mother
meant to do but I think she was afraid
to lose me I think she's afraid what if
I didn't make it so I think she probably
shouldn't have said those things but in
the long run it didn't matter anyway
because I came right back to what I was
meant to do because now what do i do i
do web design graphic design digital
illustration I to you know I do all
these things i build websites I help
people create businesses that's what I
do for a living and I do it all through
art I do it all through graphics I do it
all through technology so ultimately I
came back to doing the same thing that I
truly believe in my heart I was meant to
do and it didn't matter what anybody
said it didn't matter the words that
they had came right back to it and
without even trying I did not try to get
here I did not try to get into web
design all this kind of fell into my lap
and I ran with it I was like what do I
need to do to pay the bills what I need
to do to take care of my family I'm
going to do what I'm naturally good at
and what I'm naturally good at is
designed that's what I'm good at so I
went right back into it so this is kind
of the story of how my business evolved
now we have I have so many different
layers I could tell you there are so
many layers to me and to my business and
so many things i'm into there is to
start there is chantilly platino calm
which is my main business site then i
also have a family blog which is called
bicultural from you the other sat by
cultural Famicom I run that with my
family so my husband and I I do most of
the work on the blog I do the blog posts
I create video content ID
all the video editing my husband helps
us we'd like brainstorm ideas he helps
me get stuff set up so like if I'm doing
photography he he's like my he calls it
my wheel man so he sets up all my stuff
and he you know takes care of the kids
so I can get things done quicker and
that's something I didn't even mention
either my husband is a fail home dad so
I do the working and he does the taking
care of the kids in the house so yeah
there's my quarter for me which is a
blog and we have a youtube channel for
that but cultural media on youtube if
you look it up my daughter has a channel
called lil click TV li L click TV and I
help her run that too I haven't been
able to put videos up on there recently
because I've just been super busy but
she has a channel and I plan on going
all in on that for her soon because that
is her dream she wants to be a youtuber
so I'm there she just like gaining
reviews and she she's starting to do
like little vlogs and stuff like that
another thing i do is i have this group
called YouTube familia and you too
familiar as a group that i have on
facebook where I connected the whole
bunch of lucky next and multicultural
YouTube creators and video creators and
we just discussed everything about You
Tube so we talked about how to make
money on YouTube we talked about how to
get more views on your channel how to
get more subscribers and then just in
general like what our experiences are in
the platform and what we're up to what
we're doing in our lives like some a
couple of people are taking on like
daily vlogging challenges and things
like that which is been really cool to
watch so another thing i do is i have
multicultural bloggers and a lot of
people probably know about that but
multicultural bloggers calm is a website
where i have like a list like a
directory of bloggers if you're a
multicultural blogger you can go to
multicultural bloggers calm and you can
get listed right now I have this site
shut down temporarily because I am
working on a redesign it will be back up
soon but until then we have a Facebook
group and you can meet us in there the
facebook group operates
hand in hand with the website so the
website has a directory and the facebook
group is where we trying to promote each
other and connect she's an also joined
the facebook group and if you go to
multicultural bloggers calm and half the
link there i also have a youtube channel
for my business i'm really getting into
you too big right now so YouTube is a
big thing for me and I think everybody
should get into YouTube because video is
everything right now I mean if you could
do YouTube videos or do Facebook videos
it did on to be live videos but just
upload your videos to facebook both of
those platforms are excellent for video
even if you're not ready to be on video
maybe you don't feel comfortable being
on video like me I'm an introvert at
least do a podcast recording of yourself
like put a picture up in the video and
just you know put it make it as a video
on youtube when people don't play it and
listen to you you just take the audio
and you can go in windows movie maker or
iMovie add a picture to it and then just
publish that on your facebook or youtube
and that's a video so there's a lot of
people who like to listen to podcast you
can put it up on a podcasting platform
to but I recommend doing it like get
your voice out there because podcasts
and video are the two main ways that
people are absorbing content right now
podcasts are great because they don't
require you to be a hundred percent
present a video requires your presence
you can't watch a video without
physically looking at the video a
podcast is something that I can do
without having to be present for it
anyway to get back around to what I was
talking about my purpose through
everything I do and everything that I
did for my business through my blog
through everything is to help families
like mine to help women like myself I
feel like a lot of us women we work very
very hard and we get very small results
and I feel like a lot of women are very
underpaid very overworked and I want to
help other women to be able to ask for
more money to be able to make their
websites and their brands more
presentable and look more high-value so
they can get more money but the
impression that you give to somebody is
really important and it's very powerful
you want to make a great first
impression and that's what I try to do
for my clients is help them make that
great first impression and that's what I
try to do for myself
I started this journey and I call it
fearless 2017 and I have the hashtag
three loes 2017 well Jay share on
Instagram and on Facebook and every time
I post something on this topic I share
it and fearless 2017 is really about
going out of your comfort zone when a
brand approaches you you pitch them
higher than what you feel comfortable
pitching them charge them a higher rate
than what you feel comfortable when a
client comes up to you quote them a
higher price than what is comfortable
for you or you work with that client who
scares the shit out of you you work with
that big client who you thought you
weren't good enough to work with who's
come to you asking for your help and you
almost ran away but instead you take
that client and you work with that
client that's what fearless 2017 is
about it's about putting yourself out
there and this is something I've really
struggled with and it's held my business
back a lot and I notice myself tending
to go back into my comfort zone and do
what is comfortable and maybe not create
those products or those courses that I'm
terrified will fail but to just go back
to doing what I do now you know which is
building websites and stuff so the thing
is when I first started my business I
was all about getting out of my comfort
zone but see I was coming from a place
of necessity at the time my husband had
just lost his job and I didn't know how
I was going to make it and i had to pay
the bills so at that point I didn't give
a shit about how I felt about anything
at that point I didn't care if I looked
ridiculous I didn't care if I made
mistakes I had to put that money in the
bank so I didn't care at that point and
it's hard to get yourself to that point
when right now everything is okay like
things aren't great obviously I want
more of life than this but things are
okay we're getting by and it's easier to
slip into that place of comfort and not
push to do something more so that's
where I'm coming from right now is
coming from this place of wanting to get
out of this nice comfort zone I've been
in for the past couple of years and
really push to do something that I
haven't done before and something that
that I know can probably be successful
but I'm tariff
for it to fail so i posted on facebook
and i mentioned how several people have
told me that they wanted me to be more
visible about what i do and so i'm
trying to push myself to share more but
at the same time trying to cope with
that feeling of pushing my content on
other people but i think we as women we
need to move beyond that beyond the
feeling that whatever we're going to say
isn't valuable enough to people and plus
we all seem to have this need for
validation will seem to have this need
to be validated by other people like if
i post something on social media and
somebody doesn't immediately like it i
guess that about it why am i getting sad
why because somebody didn't like it
doesn't mean it wasn't valuable because
somebody doesn't write me a long comment
saying how much they love the blog post
I wrote it doesn't mean that that blog
post wasn't valuable we have a term for
it on the internet it's called lurkers
lurkers are people who are out there
they read your stuff they see your
social media they probably love it but
they don't say anything they don't click
like they don't comment they don't say D
and that's most of us some of us because
we're busy in their day-to-day life we
don't have time to leave a comment or
maybe we're in a rush to something else
or mind is somewhere else sometimes you
feel like okay well maybe this is what I
should be doing then and you get all
these doubts in your head and you start
thinking maybe maybe this blog post was
a complete mess maybe I shouldn't even
brought about this topic maybe you know
you're certain putting all these things
out there like maybe people hated it
maybe people just don't want to read my
blog anymore you get all this paranoia
about people not liking the content you
put out when what we should just do is
keep doing it and just keep pushing it
out we should just keep pushing out the
content and wait for the right people to
come along and sometimes it's not even
that the right people aren't there these
people were talking about these lurkers
lurkers come and they see and they may
take no action for a long time and then
one time something moves them to take
action like as an online entrepreneur
you feel you feel really isolated and
alone you start to question yourself
whether or not your work is worth it
if somebody doesn't immediately Wyatt is
my work worth it was it good enough does
it suck should I take it down right now
should I lower the price those the types
of things you start to think of as
online entrepreneur in any business
really and I think what we really have
to do is we have to have more confidence
in ourselves and trust ourselves and I'm
trying to teach myself to do that this
year trying to teach myself to have
confidence to not lower my rates when
something doesn't sell to figure out how
I can better market myself figure out
what I'm doing wrong or what I'm missing
and to not take it personal when
something doesn't do well if my blog
post doesn't do well or social media
post of them do well they have no
bearing on me and try not to take it
personal but the perfectionist in me
always pull my mind in that direction so
all this to kind of just say this is
what I'm up to these are things i'm
doing and i'm sure probably forgot some
to put them in so many things I just
kind of wanted to share with you and let
you know this is what I do this is who I
am this is how I got here and if you

MIPS Scoring Methodology – Part 6 - Duration: 8:59.

From here, I'm going to go ahead and pass it back
over to Adam.

So now we're going to talk a little bit about the MIPS
scoring methodology, and I'm going to hand
it over to Mary Wheatley, our principal health care
policy analyst with the MITRE Corporation, to walk
us through a few slides on scoring.
>> Mary Wheatley: Great.
Thank you, Adam.
So if we can move on to the next slide.
So, then, so we're going to go -- so, Molly just
gave you an overview of all the measures and
activities that are going to go into your MIPS
score, so I'm just going to briefly talk about how
we kind of create that score.
We're going to go category by category so you can see
how this all kind of translates into
a single score at the end.
So, we'll start with Quality,
the Quality Performance category.
And, again, for most people this is going
to be 60 percent of your final score.
And to recap, generally people have to submit
approximately 60 measures of the available measures.
When you submit a measure to us, whether it's a test
or "pick your pace", we're going to look at that
measure and we're going to try and score it.
We're going to assign that measure somewhere between
three to 10 points based on certain criteria.
And if we have enough data that we can reliably score
it against the benchmark -- well, at a minimum,
if you submit a quality measure we're going
to give you three out of 10 points just for giving
us the information.
If we're able to reliably score it against
a benchmark -- and we won't go into that in too much
detail -- but if we have enough sample size, if we
have a benchmark that we're able to do that,
then we can give up to 10 points based on how well
you do on those measures.
But the biggest thing here is that if you don't
submit a measure, you get zero points.
So, if you submit even a tester case you get a
minimum of three points, but you can get up to 10
points, especially -- which is easier to do if
you submit more cases so that we can actually measure you.
We will say that there are some bonus points
available for certain types of measures -- and,
again, more information is available, especially
through the technical assistance, and such --
but there are bonus points for electronic reporting,
there are bonus points for selecting, you know,
measures with -- you know, outcome measures or other
high priority measures.
So, if we can go to the next slide, please.
And -- it's not quite showing up here.
So, the next category is the Cost category, and for
this year we just want people to kind of get used
to the Cost measurement and what that means in MIPS.
So for Cost, no one is required to submit
measures -- it's something that CMS will calculate --
and we do it through claims data.
And, again -- similar to the Quality measure --
we're going to assign a certain number of points
for each measure and how it compares to a benchmark.
Those benchmarks are available on the QPP
website, if you're interested in it.
What's important to notice -- again, for this year --
the cost score is -- while we're calculating this
score for you, it's not going to contribute
to the final score that gets used for the payment adjustment.
So it's really for informational purposes
for the first year.
Next slide.
So, for Improvement Activities, again the idea
here is you choose the inventory of activities
that Molly mentioned before, and it's really,
like, how many and -- with the goal of you're getting
towards 40 points.
I think, particularly for this group, what's
important to know is that if you're in a small,
rural practice, or you served an underserved
practice, then the number of activities that you
need to report is less.
So, we basically have said that basically each
objective that you report is worth, you know, double
the amount, so you get -- so you only have to report
a maximum of two medium activities, or one
high-rated activity in order to get the maximum
number of points for the Improvement Activities.
But the way, again, the way the scoring works
is that you just report each activity, each activity
is worth a certain number of points.
And if you're in a patient-centered medical
home you automatically get -- you automatically get
full credit in the Improvement Activity.
Next slide.
And so, the last report category is Advancing Care
Information, and as Melissa discussed, it got
restructured so that the base score is worth 50 percent
of your score.
So, if you leave that base score you will
automatically get 50 percent credit, and as you
report additional measures against -- the measures
change based on your certification --
you can increase your score up to a maximum of 100 percent.
And so, in general, when you consider all the ways
you can report -- you have the base score, you have
ways you can get additional performance
in certain measures, and you can get bonus points for
reporting to additional registries, or for using
electronic health records for your Improvement
Activities -- you have the ability to get up to 155
points, which means there's multiple ways
to get to that maximum credit because the maximum credit
for any score is 100 percent.
So, there's lots of ways to get to 100 percent
in the Advancing Care Information.
And so, if you take that -- so, what we do is we take
these algorithms, we apply it to all these
activities that are submitted for these
different categories.
And as you go to the next slide, what we do to calculate
your final score is -- you know, similar
to when you were in school -- we say, we take your score
for each category which is somewhere between zero and
100 percent for each of these categories and then
we multiply it by the weight, and then we end up
with a final score that is somewhere between zero
and 100 points.
And that final score is important because that
final score is what we use to determine your payments
for the -- payments for 2019.
And so, if you go to slide 50, I want to spend
a little time talking about these points.
So, what we have done in this first year for "pick
your pace" we have set the -- we have said that if
you get three out of 100 points for your total
score, you can avoid a negative payment adjustment.
And that means if you report a Quality measure,
if you report an Improvement Activity,
if you report the base score for Advancing Care
Information -- you will get at least three points,
and you'll get a neutral point.
So, the only way -- it's only if you don't
participate you get zero points -- is where you get
a negative four percent payment adjustment.
And this is a policy that we certainly implemented
for the first year because we want people to really
get, you know, used to the MIPS program, and used to
the scoring, and learn how these -- how all the pieces sort of
fit together.
We also created a point, a target, and that's 70 points.
And so, in MIPS, we have this algorithm that we
have to calculate to determine what your
payment adjustment will be if you exceed three points.
And the short answer -- and what we have to do is
we have to get everyone's scores in, and we have
to see how much money's available to distribute,
and how many people qualify for additional
bonuses, and then we have to calculate what the amount is.
But the reason why I'm bringing up the 70-point
mark is that at 70 points there's a $500 million set
of funds from the first few years and if you get
70 points you get access to that additional funds.
So, what we can say is at 70 points we get a minimum
additional half percent adjustment along with
whatever adjustments we calculate for MIPS.
So, what does that mean?
It means that if you score at least three points, you
avoid a negative adjustment.
The higher your score, the higher your adjustment
will be but we can't tell you exactly what that is
-- we have to calculate all the scores first and
see how much, how many people we have to
distribute -- how much money there is to
redistribute and how many people we have to
redistribute to.
But if you get to 70 points, you know that
you'll get a little bit of a bump up because you can
access those additional funds for exceptional performance.

Budgeting - How to Create and Live on a Budget - Duration: 9:15.
Alright so today were talking how to set up a budget.
Now one or two people have responded they want to talk about budgets so we're going
to do that today.
I want to thank primarily my cousin Mary Ann, who gave me the idea to talk about this so,
um, Minnie, this is for you, okay?
So by the way I'm Mike Palios.
I give my clients confidence to make great financial decisions.
You're watching this, so many of you probably know that.
So, there are a few ways you can do budgets.
Now I am going to talk about two.
But there are lots of ways; you've got the envelope system.
I don't know what the envelope system is, but a lot of people do.
We're going talk about how to track it in Excel the obsessive-compulsive way, that's first.
The second way is we're going to do the simple way. We're going to do OCD first.
Alright, so, when people set up budgets, they put different numbers and allocations to dollars
for particular things.
So, "we're going to spend $200 on groceries", "we're going to spend $300 on gas",
"we're going to spend x on this and x on that".
There are fixed expenses, and then you have discretionary expenses.
So, let's say as an example one of your fixed expenses is that you're going to
spend $200 on gasoline and for through no fault of your own gasoline goes up $0.20 a
gallon and you get to like the 20th of the month and you have allocated $200
a month for gas and you're at $200 month for gas and you're at $200 bucks a month and
you've got an empty tank.
What are you going to do?
Are you going to take the bus?
Are you going to walk?
No, you're going to go fill the car up.
So, we're talking about specifically with budgeting about discretionary spending.
Gasoline, groceries, electricity - they are not discretionary; you have to pay those.
What is discretionary is the following: entertainment, like going out to movies, playing golf, um,
Dining out.
Buying clothes, triathlon stuff….
TRIATHLON STUFF!
Hey, I've got to…I'm like part of this too with you.
So were going to focus just on dining out.
So, Christie, zoom in on this just a little bit so just so everybody can see.
Alright.
So, dining out.
And let's just say that we have budgeted…our number our budget is $400.
Okay, you can back up now, thank you.
Christie's laughing at me.
Christie has not seen this before, so she's kind of going be; she did not know this were
going to kind of roll play with Christie.
Okay, so, let's say that you have decided that $400 is going to be your number for dining out, alright?
It does not matter what the number is, we're using this as an example.
So, what we do with this Excel spreadsheet, this assumes you know how to use Excel.
If you don't know how to use Excel…problem.
Oh by the way, I forgot to tell you, this is not going to be a two-minute video.
This is probably going to be closer to like 10, because I cannot have this conversation
in 2 minutes.
It is impossible.
So, $400, assuming you know how to use Excel, every single time that you go out to eat you
keep the receipt, and then you enter it in the square here.
So, we went out to eat and it cost us $65.22.
Whatever, I don't know, okay?
And then three days later you go out to eat again and it is $32…oops….21 oops ok so
I know that it is very, very difficult to see.
Christie, come in here just a second again.
So, you have budgeted $400 and after two times going out, you're at $97.43.
Okay, you can back up a little again.
Okay, so, you got to do this every two or three days.
I did this for like five years where I entered all my receipts every three or four days and
I had everything here for five year's worth of data.
So, I know what I spent on everything all the time.
So, let's say that your number again is $400 and you get to the 10th of the month
and for whatever reason this number here in dining out - or any discretionary item you
want - is at $375, okay?
So, we're going to do $375.
Guess what you're NOT doing for the next 21 days of the month?
You're probably not going to be going out to eat because you hit the number on the discretionary
side, okay?
Now, flip side you get to the 10th, I'm sorry, you get to the 25th of the month and
your number is...I don't know…you've got a $400 number here and you have actually
spent $100, okay?
Well, you can now you can make a few choices you can go out to eat.
If you are in Tampa, you go to Bern's; do the number.
Number 2, roll it over to the next month.
Number 3, save that money.
Now, what this assumes is a few things.
For you to be able to do that, it assumes a few things.
Number1, it assumes everything that we do, everything that we do, assumes that you are
perfectly and fully protected.
If you get sued, if you get sick, and if you pass away unexpectedly…assuming you are
fully protected and assuming you are saving 20%.
If you are doing those things, go ahead and hit the number and
don't feel bad about it, okay?
Don't feel bad about it…you are doing everything in the right order.
All of you work hard to earn your money, and you need to enjoy the fruits of all your hard
work.
Now you've got to keep up with this and…
Now, here is where the challenge comes into play.
When you're married, you've got a husband and wife.
The wife is going to be keeping track, or the husband is keeping track.
You've got to keep all your receipts, and you give your receipts to the spouse or to
your partner, okay?
So, this is where problems can come in.
The bookkeeper in the family is going to look at these receipts, and they are going to say;
I'll use me as an example.
They're going to say, "Hey, Mike, what's this triathlon receipt for?"
I have to be willing and open-minded to know that Bessie, all she wants to do is put it
in the right category.
She's not challenging me on if I spent too much, she's not questioning me on what I do.
She just wants to categorize it right.
So, I've got to be patient when I talk with her, and I've got to say, "You know, that
was a triathlon thing, um, enter it right there."
I cannot say, "Well, you go out to dinner all the time, or whatever…"
You have to communicate, alright?
"You buy shoes all the time…you go to Target all the time."
No, no, you can't do that.
It's a lesson and an exercise in communication between couples on making sure that this is
effective.
So, open communication, and you….many times, you have to have a third party in the room
to make sure everything is, like, cool.
I've done that probably 20 times over, at least, 20 times over the last like 8 months,
with this discussion.
So, that's the OCD way.
There is another way.
This way was created by one of my mentors and another advisor up in St. Louis.
His name is Joe DeLisi.
The Joe DeLisi way…Can you see this okay?
- Yep (Christie).
So, the Joe DeLisi way is the following….you make 100% of you income, okay?
Take out Tax.
Oops, I'm sorry, wrong way.
25%...for some of you, that is higher.
Some of you, that is lower, okay?
Depends on what your tax bracket is.
Hey, I just thought of another idea.
Christie, we're going to do tax brackets, and how they work!
Okay….Left with 75%.
Okay.
Some of you tithe, some of you don't.
I don't care if you do or not, but we're going to put it on here, and now I'm left
with 65%.
Minus 20%...You're now left with 45%.
Assuming, you are perfectly protected and you are saving…oh, jeez, wrong pen!
And you are saving 20%...that was supposed to be a highlighter, but it was permanent, so
it scratched it out.
You're saving 20%...the rest of this 45%, do whatever you want with it.
Mortgage, dinner, golf, it does not matter.
You're doing everything in the right order, and you're saving 20%, and you're perfectly
protected.
So, you can be the obsessive-compulsive way, or you can be the simple Joe DeLisi way.
I have moved from the obsessive-compulsive way for the first 5 years I was here, to the
last 18 months or so, I do it this way.
Because it's simpler.
I don't have to keep track of every little thing.
And, I have all the knowledge of keeping track over the last 60 months.
So, I know what everything costs, okay?
So, um, we've got a lot more videos that we're going to be doing.

going for when you're wearing the lashes
are you doing something more subtle or
something more bold and smoky where you
want a little bit of a heavier thicker
fuller lash to complement that look if
you're going for a very subtle natural
look I would go for the 110 ardell

close they're super just light and airy
but still have a good length on them and
they look very pretty with just a very
subtle natural I now if you're going a
little bit more bold and smoky I would
go for these 105 lashes they're the
glamour lashes after you decide what

makeup look now it's time to get all
your supplies so what I do before
applying my lashes is I always apply one
to two coats of mascara i use the
covergirl LashBlast mascara how all
we're trying to do is a few light coats
just to darken our lashes we don't want
them to be clumpy or anything we just
want them to blend in well with the
falsies so after you apply your mascara
make sure that you have some glue this
is the house of lashes glue there's also
a duo lash glue from the drugstore which
is around the same price probably and
they have like a squeezee tube or they
have a wand that you
purchase and I actually do like the
wands it makes it so much simpler than
the tweezy tubes to apply to the lashes
then I have my tweezers sometimes in
these packs like this they'll have these
little like plastic applicators that
work just as well there's just a little
bit harder to maneuver when you're a
beginner honestly even though tweezers
might sound a little bit scary I would
recommend using tweezers and but please
be careful be careful when you use
tweezers because they're getting very
close to your eyeballs and I always keep
a pair of scissors or nail clippers with
me just to trim the lashes if they're a
little bit too long widthwise if that
makes sense my eye look isn't super bowl
today but for the purpose of the video
Ayman ease these Boulder lashes so first
I'm going to do my left eye and all I'm
going to do is take them off the little
applicator like that you want to do is
get the lash bendable and movable so
you're just going to kind of wiggle it
back and forth and I even like to stick
it on my finger and kind of wrap it like
so next you want to measure the width of
your lashes so I'm just going to take my
fingertips and apply the lashes on my
lash line and see if I need to cut any
of the width off but those look pretty
good sometimes you'll get lashes that
are super long because everybody has
different shaped eyes but these ones
work pretty well for me so I'm just
going to peel them back off we're going
to see this side that's a little bit
shorter goes on the inner corner in this
side which is a little bit longer it
goes on the outer corner now i'm taking
my house of lashes blue and i'm just
going to apply that on to the band where
you have enough glue that you know it
will stick but not an overboard amount
where it's just going to slide around
and get goofy and gross and wait about
30 seconds with this glue to apply it
now if you're using a squeezy tube like
the duo lash glue I would recommend
waiting 60 seconds for those to make
sure it's tacky that is honestly one of
the most important tips make sure your
lash glue is tacky because if it's not
then it will just slide around on your
eye so I like to have a little mirror in
front of me and I'm going to look down
into the mirror and I'm going to get as
close
to the lash line as possible and I like
to put the center of the lash on first
and then take my tweezers and secure the
edges make sure you don't have any glue
on your tweezers as well because it will
kind of just make the lashes stick to
the tweezers and then they'll pull off
I've had that happen several times so i
just press my lashes and the false
lashes together just so they're blended
and you can't really tell the difference
and this glue is blue right now but it
will dry to be clear and sometimes I'll
even take a black liner afterwards just
kind of hide that lash line with the
black liner I'm going to do my right eye
I have that lash wrapped around my
pointer finger again I'm going to just
keep bending it the more you bend it the
easier it is to bend to your eye shape
so I apply a little bit more glue to the
edges of the lashes because of your eyes
water at all sometimes they will tend to
pop up which you do not want I'm just
going to do the very same thing look
down into a mirror like a little
handheld mirror like i have and get the
lashes as close to the lash line as
possible i always start in the middle
and then secure the edges afterwards
this just works best for me
look how much bolder and Chris burger I
makeup looks after applying lashes they
are one of my favorite makeup products
if you can call that a makeup product
ever i love lashes so so much you want
to hide your lash line even more just
take a liquid liner and go over the lash
line a bit so i hope you guys enjoyed

Graduating Medical Student Headed for Duty with U.S. Navy - Duration: 1:17.
- So, I've always wanted to be in the military in some way,
and then once I figured out
that I also wanted to be a doctor,
the HPSP program, which is
the Health Professions Scholarship Program,
seemed like a good way to sorta merge the two.
Between your first and second year of residency,
you have the opportunity to do what's called a GMO tour.
These are operational tours
in which you act as a primary care physician
for the forward Navy.
- Wake Forest School of Medicine definitely
approaches physician education from a holistic standpoint,
developing great thinkers, great leaders,
and people that are gonna be representative of our program.
- No one in my immediate family have served in the military.
Both my grandparents, however, did.
One grandfather was in the Army and served in World War II.
The other grandfather was in the Navy
and served in the Korean War.
I had the opportunity to restart the tradition of serving
in our military and following after my two grandparents.
I think the armed forces is at the forefront
of medical technology and medical research.
- Tommy was instrumental in several of our projects,
doing research, looking at outcomes of patients,
and he definitely has a bright future ahead of him.

Quick Hits for Principals: Automated Detentions - Duration: 1:51.
Hi, I'm Scott.
If you were to describe the highlights of your day,
"giving detentions" wouldn't make the list.
But what if that process could be automated?
Today we're going to look at how you can automate detentions for students with repeated tardies.
Once you've gone through the process of setting up attendance letters for students with multiple tardies,
it's easy to automatically link them to detentions.
To get started, go to Office → Attendance → Scheduled Letters.
Since your letter is already set up, you can click on Letter Parameters on the left side of the screen.
Click Edit, then check the box that says Create Discipline Records for Students Receiving Letters.
Then click the Discipline Offense button.
In the popup box, enter an offense, such as excessive tardiness.
Be sure to change the offense date to Variable, so it will always be current.
And click Save.
On the next screen, set up the action to be taken,
in this case detention, when an attendance letter is distributed.
Again, set the date to Variable.
When you're finished click Save.
The system will now automatically issue detentions whenever an attendance letter is sent.
Use that letter to inform students that they have a certain amount of days to serve the detention
and put some accountability back in their hands.

We're going to enter into Part 2 of the webinar today.
And this is really the getting started, the "how
do I do it" portion of the webinar.
So, we're going to start, really, with the checklist
for preparing for and participating in MIPS.
And it's my pleasure to introduce another
colleague, Dr. Asbhy Wolfe, Chief Medical
Officer for Region 9 at CMS and a family
physician, to run us through this list and give
us some pointers on how to get started.
>> Ashby Wolfe: Thank you so much, Adam, and good
afternoon -- or good morning -- everyone,
depending on where you're accessing the call from.
It's my pleasure to be here, and I am going to be
walking you through the final segment where we'll
discuss the practical aspects of the program --
the how-to of the participating in this MIPS
pathway of the quality payment program.
And thank you, Tomali [phonetic sp] and Mary
for that detailed review of MIPS.
I'm going to try to break down some of what Molly
and Mary discussed and described on the next several slides.
If we move to slide 52, you'll see a checklist.
Really, we hope this will be useful to you
as clinicians, as practice managers, as part of
the care support team in preparing for participation
in this transition year, 2017.
And you'll see on this slide there are a number
of points to take a look at.
I'm going to be walking you through each of these
checks in the next several slides, including looking
at how you determine your eligibility, how you might
choose to submit your data, what submission
method to look at, and how to choose your measures, et cetera.
This checklist is really meant to serve as a
general guide to every clinician who would be
participating in MIPS, regardless of your level
of experience with prior reporting programs like
PQRS or Medicare Meaningful Use.
So, we're going to walk through each of these
items -- just remember, this checklist is really
a means to help you get started.
You have the flexibility to determine how many
elements are really applicable to you in your
practice, and don't feel that you need to complete
this checklist in order, or all at once.
We're going to talk through each step and you
can really personalize it as you see fit,
and as it would be helpful.
So, moving through slide 53 to slide 54, let's talk
about that first checkpoint -- determining your eligibility.
So, how do you this for your practice?
Well, to go back to the eligibility piece that
Molly mentioned earlier -- really, it's a question
of determining your annual patient count and billing
amount for Medicare Part B -- and getting back
to that specific question: did you bill more than
$30,000 in Part B allowed charges and provide care
for more than 100 Medicare patients in a given year?
Again, this applies to Medicare Part B fee for
service -- so really taking a look at your
claims for service provided between September
1, 2015 and August 31, 2016 will be key in order
to determine your eligibility.
Now, CMS will be providing additional information
to you this spring to help you in this effort.
The key thing I want you to take away from this is
that there is this two-part piece to the eligibility.
If you are a physician, a nurse practitioner, PA,
Certified Registered Nurse Anesthetist, or Nurse Specialist
you have to be able to answer that question: "Did you bill
more than $30,000 in Part B allowed charges, and provide
care for more than 100 Medicare patients in that year?"
Now, as I mentioned, CMS will be providing
additional information to help you determine this
eligibility, and we've been really deliberate
to make sure that all clinician information
is up to date and that our eligibility data is accurate.
So, let's move on to the next data point --
the next decision point here -- on slide 55.
How do you choose to submit data?
Do you participate as an individual or as part
of a group?
So, on slide 56 you'll see this particular checkpoint --
an eligible clinician has two options for participating
in the MIPS pathway, as was mentioned previously.
You can participate as an individual, meaning that
you, as a clinician, would report information under
your unique National Provider Identifier,
or NPI, combined with your Tax Identification Number, or TIN.
So, using this combination we can identify you as an
individual participant in MIPS.
Alternatively, if you and at least one other
clinician in the group would like to participate
as a group we would take a look at your data under
the single Tax Identification Number
to which you and your fellow clinicians have
re-assigned your billing rights.
So, it's important to note that if you,
as a clinician, decide you want to participate as a group,
your group is assessed as a group across all of the
MIPS performance categories.
Now, we encourage clinicians to begin
discussing how they want to participate
in this performance year.
It's not just about knowing that this choice
exists, it's about talking to your clinician
colleagues, if you have them, and really
identifying the best option for your practice
and the population of patients that you're serving.
Now let's talk about the next decision point --
choosing a submission method.
How do you do this?
How do you get your data to CMS?
So, on slide 58 you'll see a few suggestions.
The good news is that if you have already been
submitting data to CMS through either PQRS or
Meaningful Use, for example, chances are you
are already familiar with some of the submission
methods that we have as options to report
in the MIPS program.
As was mentioned earlier, depending on whether
you're participating as an individual or as a group,
our submission methods for MIPS include claims,
attestations, qualified registries, qualified
clinical data registries, electronic health records,
and, for groups of 25 or more, the CMS web
interface in the CAPS for MIPS survey.
So, in making this determination it's very
reasonable to speak with your specialty society
or to get additional help through one of our
technical assistance contractors.
You don't have to make these decisions on your own.
We have a list of technical assistants --
it's currently available on our website.
As was mentioned earlier you can visit qpp.cms.gov
for information on your submission options,
as well as information about using our technical assistance
program for decision support.
So, let's move to the next decision point.
Now that you've made some initial determinations
about eligibility and about how you're going
to get your data to CMS, you can prepare to participate in 2017.
So, on slide 60, you'll see a couple of additional
suggestions about how you do this additional preparation.
So, thinking through your practice readiness --
if you've previously participated in a quality
reporting program, like PQRS or Medicare
Meaningful Use, that's an opportunity where you can
build off what you've learned already.
Additionally, think through if you're prepared
to begin reporting data for the 2017 transition year.
You can review the options for participation,
including whether you simply want to test
the system, as Molly was mentioning earlier, or if
you'd like to participate more as a partial-year
participant, or for the full year.
So, there's a couple of different pieces here
on this slide that are worth thinking about as you
begin to plan your method of participation.
Now, let's talk about the data itself.
What information are you sending to CMS, and how
are you going to make that decision?
So, moving through slide 61 to slide 62, this slide
really details some of the information that would be
helpful as you choose your measures and activities --
what data you will be sending into CMS.
Step-by-step you can review the measures
or the improvement activities that make the most sense
to you based on your specialty and your scope of practice.
So, we highly recommend going to qpp.cms.gov
and clicking on the "Explore Measures" tab right
at the top of the page.
That will take you to an opportunity to review each
of the performance categories that
are available in MIPS, and you can select a performance
category of interest.
You'll have several different ways to sort
this information -- looking critically
at the Quality measures, the Advancing Care Information
measures, or the Improvement Activities.
And depending on which performance category you
choose, you can actually search by keyword, by
specialty -- you can even search by method of submission.
So, for example, if you are a clinician who wants
to report a quality measure, or wants
to take a look at what quality measures match up with
your specialty -- and let's say you take care
of a significant number of patients with arthritis --
you can actually go to the Quality Measures tab, type
in the keyword, "Arthritis," and the tool will actually sort
for you which measures are applicable there.
You can also search by specialty, so if you're
a dermatologist and you want to see what quality
measures apply to you, simply use the dropdown
menu and you can identify which quality measures apply there.
By searching these different options you can
actually see all of the information that's
pertinent to that measure, or to the improvement
activity that you're interested in reporting.
Now moving on to slide 63, a couple of tips
to think about here.
So, definitely consider your patient population
and the clinical conditions that you treat.
It's worth thinking about what areas of your
practice might be the most amenable to practice
improvement goals.
For myself, I'm an actively practicing family physician.
I continue to see patients as part of my job at CMS
and, for an example, if I wanted to really look
critically at my patients for whom I'm treating for
diabetes, I could really make that a theme in terms
of my practice improvement goals -- looking at, focusing
on increasing diabetic control among my patient populations,
choosing quality measures, and perhaps an improvement activity
that might support those goals.
And being able to sort through, on the Measures
tool, the valuable information that would
then be provided to me in the CMS feedback report.
So, those are things worth thinking about as you're
selecting what measures and improvement
activities, or Advancing Care Information measures
are the most applicable.
Now, for those of you who have not participated
in PQRS or Medicare Meaningful Use, you may
not be as familiar with the existing feedback
reports that clinicians receive under those programs.
The Quality Resource Use Report, or QRUR,
is currently the way we're providing information
to clinicians who are participating
in our legacy programs.
It's a report card of sorts, provided to
clinicians by CMS, and it provides an overview of
the quality of care that clinicians are providing
to their patients, as well as a review of the cost
of that care.
That quality information is actually pulled
from PQRS data and the cost information comes directly
from claims.
So, if you're a clinician who has been participating
in PQRS or Medicare Meaningful Use we would
highly recommend that you take a look at your
existing QRUR to potentially identify
measures that could match up with your practice
improvement goals, as well.
Now, let's move on to slide 64 and talk about
how to verify the information that you need to report.
Slide 65 provides an extensive list of this
information, and I do want to emphasize a few key
specifications that may be helpful to you in
reviewing the information that you need to report.
Taking a look at the measure number,
the measure title, of course, is important to know which
populations are applicable there, and looking at the
submission method options -- not every quality
measure or improvement activity can be submitted
by the same submission method.
So, looking at what submission method makes
the most sense to you is very important.
Additionally, the measure description
and instructions on reporting, as well as the criteria
for the measure, are key things to take a look at.
And remember, many of the measures for MIPS are
measures that have been developed by specialty
societies, they've been rigorously tested
and validated, and some of them have been in use
previously in PQRS and Medicare Meaningful Use.
So, some of the measures will probably look very
familiar to you if you have been participating
in those programs.
The new element of MIPS is the increased flexibility,
so making sure that you, as a clinician, can pick
measures that match up with your specialty
and your scope of practice.
So, definitely taking a look at these details will
be key ways to assist you in making a decision.
Now let's move to slide 66, and then 67.
And, finally, talking about the submission of
data -- making sure that you have an eye on the
timeframe for when you need to submit your data.
We at CMS highly recommend that you send in your data
to CMS before the submission deadline,
which is March 31, 2018.
So, depending on how you're participating in
MIPS, you're capturing data about your performance
in 2017, and you'll be tracking your progress
throughout the year.
You'll begin submitting this data on January 1, 2018
with the deadline for getting that information
to us by March 31, 2018.
So, again, highly recommended that you submit your data early
so as not to miss the deadline, and make sure that we have
timely receipt and accurate data.
So, remember -- these steps we've just been
discussing will help make participating in MIPS
possible for all eligible clinicians, really
regardless of their previous experience with
any of our legacy programs.
CMS would really encourage you to discuss MIPS with
your colleagues, with your specialty societies,
and with CMS, as we are working together to learn
from your experience and to continually improve
the program to the benefit of not only you,
our clinician partners, but also your patients.

Kinesiology Taping for Shin Splints - Duration: 2:37.
Hey guys, Dr. Michael here from Velocity Sports Rehab.
In this video we are going to go over a RockTape application for shin splints.
What I have here are three pieces of tape that I cut.
There are two small ones and then one longer one.
Then what I did was measure how long my shin is for the longer strip.
And then about 3 to 4 inches for the small strips.
Then I'm going to feel for my tibia which is my shin bone.
And then I'm going to feel to the outside where the muscle is, which is the anterior
tibialis muscle.
Then I'm going to rip the back side of the tape.
Wherever you are feeling the pain is where you are going to want to tape.
So let's say that I have pain in the middle portion and maybe a little bit lower.
So my first strip that I'm going to do is on the tibialis anterior muscle, just to the
outside of the tibia.
Rip off the back of the tape and place it on the skin.
Slowly roll the tape off, putting no stretch on the tape.
Then just follow the muscle along the tibia.
Give the tape a little bit of a rub so that it sticks to the skin.
Then I'm going to take my smaller strip and rip them again on the back.
Find the area that I have pain.
So I'm going to anchor my strip on the side.
Then push the muscle closer to the bone and then tape over, again with no tension.
And then the last one, same thing just a little bit higher.
Rip the back of the tape and I will place it on the side.
Using my hand and not the tape, I'm going to push the muscle towards the bone and then
just lay the tape on there, with no tension.
Give it a little bit of a rub to make sure that it all sticks to the skin.
And that's how you tape for a shin splint.

Information is everywhere!
So, it's pretty easy to research almost any topic.
What else is pretty easy?
Accidentally plagiarizing.
Plagiarism by definition is: "The act of taking another's idea and presenting it as your own."
You see that?
That right there is a citation.
That tells you that this statement or idea is not my own, and this is where I found it.
And if you just copy something without saying where you got it, that's plagiarism.
Even if you don't use the same exact words in the same exact order, it's still plagiarism.
Change one or two words out?
Still plagiarism.
And even if you copy and paste from your own previous original work,
yep, that's plagiarism too.
Now, how to avoid plagiarism: well, when you're writing a report, you should gather information
from a variety of different resources.
But you need to cite those resources when you include statements and ideas that are
not solely yours.
Now, if you form your own conclusions and ideas based on the information you learned
from your research?
That conclusion doesn't need a citation!
You've made that statement all on your own.
And you don't need to cite yourself!
