March for Our Lives & #NeverAgain: Celebrities Showing Support | Billboard News - Duration: 1:12.

With the nationwide March for Our Lives demonstration taking place this Saturday,

March 24, artists and celebrities are using their platforms to show their support.

"Hey, it's Justin here in Washington, D.C."

"And as mayor of Washington, D.C., I'm pleased to welcome the students

and families supporting the March for Our Lives this Saturday."

In addition to Justin Timberlake, Garth Brooks delivered a heartfelt message

to Parkland student Emma Gonzalez during his Facebook live

broadcast this week. Brooks told Gonzalez that the future is hers and

encouraged her to take it, shape it and mold it however she wants.

"Love. Do not let hate win. Love." Brooks concluded.

Other artists getting involved include John Legend, who along with his

wife Chrissy Tiegan pledged money. Lin-Manuel Miranda and Ben Platt

both donated proceeds from their "Hamildrop" single.

Ariana Grande, Demi Lovato, Miley Cyrus and Jennifer Hudson will all be marching,

while Fall Out Boy, Bebe Rexha, G-Eazy and Lizzo will all be performing prior to the March.

hi everyone thank you so so much for being supporters of Made Up Theatre and

guess what today's our big reveal day we're gonna tell you where we're moving

you know what guys let's do it improv style where we show and we don't tell

we're gonna show you where we're moving come on

it's official Made Up Theatre is moving to 4000 Bay Street in Fremont California

in the summer of 2018 located in Irvington at the Five Corners intersection

above the popular Bean Scene Cafe you might know this spot is home of Broadway

West after 20 plus years of delivering theatrical performances they have

decided to conclude their annual productions on June 9th they graciously

reached out to Made Up Theatre to take over their lease and we accepted Made Up

Theatre's journey to deliver comedy to the Bay Area began in 2010 and every

year has brought us new growth and opportunity but now we are ready for our

greatest opportunity to move into an actual theatre but with this relocation

project comes a lot of hard work and financial challenges with costs of the

new stage improved lighting and sound equipment updated licenses and permits

and overall renovations will need at least 26 thousand dollars to create the

theater we envision at this time we are fundraising to reach that goal

so if you'd like to lend a helping hand to Made Up Theatre we greatly appreciate

The Philosophy of Assassin's Creed Rogue Remastered: The Cure for Freedom? – Wisecrack Edition - Duration: 11:10.

Hey Wisecrack, Jared as always.

So Ubisoft recently reached out to us about doing something for the newly remastered version of

Assassin's Creed Rogue for PS4 and Xbox One.

Lucky for them, we've gotten a lot of AC requests over the years, and lucky for us,

Rogue has a unique perspective on eternal struggle between the Assassins and Templars,

and how it relates to the complicated nature of freedom.

Throughout the series the value of freedom is placed above all else.

"How else will we ensure freedom for the human race?"

The assassins, we're told, are fighting the good fight against those devious Templars

who are trying to rule the world with an iron fist.

While freedom is nearly universally celebrated in society and media, cue the legally safe

William Wallace knock-off, "FREEEEDOM!" through its protagonist Shay Cormac, Assassin's

Creed: Rogue asks an interesting and unique question: when does freedom go too far?

Welcome to this Wisecrack Edition on Assassin's Creed: Rogue.

And, as always, some minor spoilers ahead.

First a quick recap.

Rogue takes place around the numerically challenged Seven Years' War and somewhere between Black

Flag and Assassin's Creed 3.

We follow fledgling Assassin Shay Cormac, a trouble maker who begins to question the

Assassin Order's methods.

"Laurence Washington is dead" "You look disappointed" "The sickly way that man looked.

He'd have been dead in a month anyway."

He continues to struggle with defending his creed until halfway through the game, he is

given an assignment to find a Piece of Eden an ancient object said to bestow God like

power and wisdom.

"Be careful Shay, Pieces of Eden are powerful relics."

When Shay tries to retrieve it, he triggers the infamous Lisbon earthquake of 1755 that

killed thousands.

"What's the next city you want me to smite.

What happened in Haiti happened in Portugal a great earthquake!

Thousands dead!"

He confronts the Assassins who deny culpability for the earthquake, even though they were

aware that the same thing had caused the devastating earthquake in Port-au-Prince five years earlier.

Shay argues that, "Who are you to decide what city falls next," but the Assassins

say, "We have the responsibility!"

So Shay flees the Brotherhood.

After being injured in his escape he is nursed back to health by an elderly couple who turn

out to be Templars.

Shay is then recruited by the Templars, and sent to kill his former Assassin bretheren.

In most Assassin's Creed games we're told the Templars are evil because they oppose

freedom, and the Assassins are good because they fight for freedom, which is best embodied

in their mantra: "Nothing is True, Everything is permitted."

But AC Rogue, by showing the inner workings of both sides, explores the paradoxical nature

of freedom, and the shortcomings of the Assassin's belief system.

Sure, the Assassins seem noble; after all, you don their mantle as you fight against

the corruption of the church and history's other bad-guys.

But just how benign is the saying "Nothing is true.

Everything is permitted."

When you consider the events of Rogue, the Assassins' disposition toward absolute freedom

starts to look a bit questionable.

We can best understand why through the concept of positive freedom.

Positive freedom is the freedom to do something, and to self-determine what that something

is.

It's the freedom to choose to blast weird German techno whenever you want That's different

than negative freedom - the freedom from certain external forces, like the freedom from your

roommate Brad telling you to stop blasting weird German techno at 2 AM.

Or Brad commandeering your stereo.

"The problem with unlimited freedom is that it often comes in conflict with the rights

and freedoms of others.

While you may be the master of your own fate, what if your actions limit the freedom of

others?

What if Brad can't be the master of his own destiny because he hasn't gotten any

sleep after the unceasing onslaught of Autobahn every night until 2AM?

And, if you reject any limitations on the freedom from outside interference, or negative

freedom, who's going to step in to help Brad get some sleep?"

If "Everything is permitted" then people might not agree with when and how I want to

listen to music.

The Templars address this problem of freedom by negating it altogether.

They, in essence, don't believe in the value of positive freedom, that people can make

their own decisions, or negative freedom, that they should be free from despotic control.

Their philosophy is, essentially, that the freedom of one person will ultimately end

up rubbing against someone else's, and so such freedom must be restricted.

Philosopher Isiah Berlin highlights one way in which the self-determination of positive

freedom can have horrific results.

Freedom, for Berlin, can often be defined collectively with others.

Groups, tribes, or even nations can set out to define their own destiny.

And that's where things get a little tricky, since in the name of a collective freedom

many have decided to repress minority groups - the German people collectively yearned for

the freedom to determine their national destiny, but, well, we all know how that turned out

for certain groups in Europe.

Obviously the assassins and the nazis are about as similar as Motzart and Fred Durst,

but the assassins are willing to sacrifice the freedom of some for the sake of freedom

for everybody.

If we are all free to do whatever the hell we want, and the Assassins don't mind earthquakes

killing thousands, then is absolute Freedom really the ultimate good?

The Templars seem to understand this problem with positive freedom, with people being their

own masters, so they actively try to suppress it in the name of a better world.

"What marvelous destruction captain, I saw the smoke all the way from the Morrigan" "That

should discourage those miscreants.

New York is safe for now," And, as Rogue shows, they're not as misguided as one may think.

Paradoxically, certain rights and freedoms can be enhanced with a heavy hand from say,

the government or the Templars.

So for instance, by limiting someone's freedom to enslave another person, law can instate

a better version of freedom not predicated on owning other humans.

We see this conundrum in other AC games, such as Black Flag where the British templars crack

down on the Pirates you play as in the name of safer seas, and liberty for would-be pirate

victims.

The Templars believe they know best and that order can be achieved through guidance and

rule, but this isn't exactly a perfect ideology either.

The total control of individuals for the sake of the greater good leads to your typical

Big-Brother dystopian nightmare.

So while causing earthquakes, stealing fortunes and slitting throats is okay if "Nothing is

True, Everything is permitted," the polar opposite is that we should all be thanking

our masters for living under a jackboot.

But hey, at least they're open about it.

What makes Rogue unique is that Shay distances himself from the dogmatic belief structures

that characterize the protagonists of previous games and embraces a rationally informed moral

choice.

But does Shay make the right choice?

To figure that out, we'll need help from one of the big dawgs of the European Enlightenment-

Immanuel Kant.

During the Enlightenment, Kant formulated a way to ground morality in reason instead

of religion or custom known as the 'Categorical Imperative'.

So let's see how Shay's decision stacks up.

"The first formulation: "Act only according to that maxim whereby you can... will that

it should become a universal law," sort of boils down to 'Do Unto Others'.

So, if you want to know if killing people and then searching their bodies for loot is

moral, you should consider what will happen if everyone follows this rule.

The second is to "Act in such a way that you treat humanity... never merely as a means

to an end, but always at the same time as an end."

This essentially means, life is the highest value in and of itself, so never harm others

or exploit them as a mean to achieve some "bigger" goal.

The final rule says that "the will of every rational being" should be thought of "as

a universally legislating will."

Which ties the first two together and suggests that there is a universal reason or logic

that we can all access through our own rationality.

It also offers a framework that includes a sense of freedom - since every person is capable

of participating in this kind of moral reasoning - without some of the dangers of the Templars

or Assassins, where people become freedomless pawns to their grand plans, even if, paradoxically,

freedom is part of that grand plan.

According to Kant, no action or morals are ethical if they contradict these three rules,

so let's see how Shay scores against the Assassins.

Shay defects because he believes that by causing an Earthquake and justifying it as part of

their 'responsibility', the Assassins have done a grave injustice.

This act violates the first formulation because it implies killing thousands for your own

ends, which could never be a universal ethical act.

One point for Shay. zero for the Assassins.

The Assassins are also saying those lives are acceptable sacrifices for their greater

goal, a violation of the 2nd principle, because using people as a means to an end is a big

no-no.

Another point for Shay.

And finally, through their belief in total Freedom, the Assassins are trampling on people's

legislative will by ignoring their own agency and literally destroying their homes and autonomous

lives, which contradicts Kant's third formulation.

Shay wants no part of it.

So 3 points for Shay.

Zero for the Assassins.

So what can we learn from Shay's decision?

That fighting for freedom is not so simple.

By pursuing their own freedom, the assassins have destroyed the freedoms of others.

But by rejecting the indoctrinated philosophy of "nothing is true.

everything is permitted," Shay could be the few ethical heroes of the Assassin's Creed

franchise.

Unfortunately for Shay, the Templars aren't exactly model citizens either.

In the name of regulating freedom for the greater good, the Templars do plenty of deplorable

things.

While we see them helping local people in various places, "Now let's use some of

that money for the good of the city," and stopping the Assassins from committing what

amounts to a war crime.

"There is an abandoned factory nearby holding a dangerous poison."

We also see them lie, maim and kill in the name of their own creed.

In the end, the same sense of moral dubiousness follows the Templars as much as the Assassins,

"When I'm issued new recruits burdened with regerets, I tell them the surest way to lose

them is with gunpowder," with them both contradicting Kant's Imperative by using people as a means

to their own ends and killing those who stand in their way.

AC Rogue's more nuanced exploration of freedom, good, and evil is a welcome addition to the

series.

Shay's decision to betray the Assassins brings the philosophies of both factions under

scrutiny and makes the player view each in an entirely new light.

Was this moral ambivalence about freedom always there?

Have we been playing as the 'Bad Guys' all along?

Here's to hoping this complex question gets explored in future installments.

President Buhari to visit Borno for Wal-Wanne rice pyramid - Duration: 1:43.

President Buhari is expected to visit Borno state on March 27, to unveil the Wal-Wanne rice pyramid at Ramat Muhammad Square, Maiduguri.

As a result of this development, Borno ois said to have joined about five other states that have made giant strides in the Buhari administration efforts to make Nigeria self-sufficient in rice production, Leadership reports.

It joined other states like Kebbi, Ebonyi, Ogun and Jigawa, to key into the Central Bank (CBN) funded Anchor Borrowers Scheme.

Alhaji Kabir Abiso, the director, Walwanne and Sons Limited, one of the Anchor Borrower clients, disclosed that work on the pyramid had reached advance stage and it would be unveiled soon in Maiduguri.

He further disclosed that three pyramids, each consisting of 11,000 bags of rice paddies, were harvested in Maiduguri.

we had reported that President Muhammadu Buhari on Monday, March 19, announced he would soon inaugurate and personally chair the Food Security Council (FSC) that would include the federal government, six governors and a host of others.

According to Buhari, the FSC will also work closely with the private sector to ensure the alignment of all related policies for the improvement of farming in the country.

Infective Endocarditis: Diagnostic Testing for Identification of Microbilogical Etiology [Hot Topic] - Duration: 18:38.

Hi, I'm Matt Binnicker, the Director of Clinical Virology and Vice Chair of Practice

in the Department of Laboratory Medicine and Pathology at Mayo Clinic.

Infective endocarditis, which involves infection of the inner lining of the heart chambers

or valves, is associated with a high mortality rate, with more than one-third of patients

affected dying within a year following diagnosis.

So, it's important that health care providers have a good understanding of how this infection

is diagnosed so that therapy can be optimized.

In this month's "Hot Topic," my colleague Dr. Robin Patel will review the laboratory

methods used to diagnose infective endocarditis.

Specifically, she'll discuss the role of blood cultures, nucleic acid amplification

tests, histopathology, and recently, broad-range bacterial sequencing, and how these methods

can assist in the diagnosis of this disease.

testing of endocarditis:

Which tests should you use?

When should these tests be used?

The mortality of infective endocarditis is high, with more than one-third of patients

affected dying within a year following diagnosis.

Identification of the specific microbial etiology is essential for optimal patient management.

Delays in diagnosis contribute to late initiation of effective antimicrobial therapy, influencing

morbidity and mortality.

Staphylococci and streptococci combined cause about 80% of cases.

Staphylococcus aureus remains the prevailing pathogen, associated with 25% to 30% of cases,

while coagulase negative staphylococci cause approximately 11% of cases.

Streptococci, primarily viridans streptococci, account for about 30% of cases, with Streptococcus

gallolyticus being involved in 20% to 50% of streptococcal cases.

Enterococci account for approximately 10% of cases.

Gram-negative bacilli account for about 5% of cases, and include the HACEK group organisms

Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella species and, less

commonly, non-HACEK Gram-negative bacilli, such as the Enterobacteriaceae and non-fermenting

Gram-negative bacilli.

A number of "uncultivable" or challenging to cultivate organisms may cause endocarditis,

the most common of which are Coxiella burnetii, Bartonella species, and Tropheryma whipplei.

The modified Duke criteria provide a basic framework for diagnosis and definition of

endocarditis.

The finding of two or more blood cultures positive for a typical microorganism consistent

with infective endocarditis is a major criterion for diagnosis, as is positive Q fever serology.

Echocardiographic findings are also considered by the Duke criteria, although they are beyond

the scope of this "Hot Topic."

Endocarditis is an endovascular infection associated with the persistent presence of

infecting microorganisms in blood.

Accordingly, blood cultures are the standard test to determine the microbial etiology.

Routine blood cultures incubated on modern automated, continuous-monitoring blood culture

systems allow recovery of almost all easily cultivable agents of endocarditis without

a need for prolonged incubation or terminal subculture.

Blood cultures should be drawn prior to administration of antibiotics.

Conventionally, three sets of blood cultures, with each set including one aerobic and one

anaerobic bottle, are collected.

Alternatively, two sets may be collected, with two aerobic and one anaerobic bottle

per set.

Yield of blood cultures is directly related to the volume of blood cultured, with properly

filled blood-culture bottles being essential.

Most, if not all, blood cultures from patients with endocarditis caused by microorganisms

able to grow in blood-culture systems should be positive, provided that they are appropriately

collected and drawn prior to the administration of antimicrobial therapy.

Separation of blood-culture draws over time is unnecessary.

Standard blood-culture incubation times of five days are adequate for recovery of almost

all cultivable causes of endocarditis, including Candida species.

The HACEK organisms were classically considered challenging to detect in blood cultures; however,

with current blood-culture systems, neither extended incubation nor terminal blind subculture

is unnecessary for their recovery.

Current blood-culture systems also contain sufficient supplements to support growth of

Abiotrophia and Granulicatella species, formerly known as the nutritionally variant streptococci.

Brucella species are infrequent causes of endocarditis in the United States; however,

their detection in routine blood cultures is typically achieved within the standard

five-day incubation period.

Cutibacterium acnes deserves special consideration, however, as some strains of this species may

require prolonged blood-culture incubation.

Fungal endocarditis is rare and is most commonly caused by the Candida species, which should

grow in routine blood cultures.

Non-candidal fungal endocarditis is very rare, requires specialized testing, and should only

be considered in patients with specific risks after more common etiologies have been excluded.

Blood cultures are negative in 2% to 40% of cases of endocarditis, with some studies reporting

higher blood culture-negative rates.

The causes of so-called "culture-negative endocarditis" fall into two categories:

negative blood cultures due to concomitant or antecedent antibacterial therapy, or the

presence of an organism that does not grow in routine blood cultures, with the former

being more common.

In patients who have not received antibiotics, the most common etiologies of culture-negative

endocarditis are Coxiella burnetii and Bartonella species, with the former accounting for 28%

to 37% and the latter for 12% to 28% of cases.

Tropheryma whipplei causes up to 6% of culture-negative endocarditis cases.

Cutibacterium acnes, a rare cause of endocarditis, may cause culture-negative endocarditis due

to the requirement for prolonged blood-culture incubation for growth of some strains.

In addition, some strains may not grow in blood cultures.

Mycoplasmal endocarditis, while rare, is primarily caused by Mycoplasma hominis and is usually

diagnosed using a nucleic acid amplification test.

For some organisms that do not grow or do not easily grow in routine blood cultures,

serologic evaluation can be helpful.

Organisms for which serologic tests have been shown to aid in the diagnosis of endocarditis

include Coxiella burnetii and Bartonella species and, in areas where Brucella endocarditis

occurs, Brucella species.

Serology for Coxiella burnetii is the best-established serologic test for the diagnosis of endocarditis

and is included as a major criterion in the modified Duke criteria.

In chronic Q fever endocarditis, anti-phase I IgG Coxiella burnetii titers greater than

or equal to 1:800 are diagnostic.

Nucleic acid amplification tests performed on blood can be helpful for diagnosis of Coxiella

burnetii, Bartonella species, and Tropheryma whipplei endocarditis, but their sensitivity

is imperfect.

Although testing of cardiac valve tissue is more sensitive than testing blood, a PCR result

on blood is helpful when positive.

Surgery is performed in some, but not all, cases of endocarditis.

If a microbial diagnosis has not been established at the time of surgery, excised valvular tissue

provides a valuable diagnostic specimen.

It should be submitted for histopathological and microbiological evaluation.

If a microbial diagnosis has already been established, additional microbiology testing

is typically unnecessary, but histopathologic evaluation is often still performed.

On gross examination, vegetations may be soft, friable, or firm and vary in size based on

the infecting organism.

Discrete vegetations are not, however, always present.

Representative sections of the valvular material should be processed for histopathology.

On histologic examination of excised valve tissue, patterns and degrees of inflammation

will vary depending on the infecting organism.

Endocarditis caused by highly virulent organisms, such as Staphylococcus aureus, is often associated

with acute inflammation characterized by extensive neutrophilic infiltration as well as large

colonies of microorganism-associated areas of tissue destruction.

In cases of subacute endocarditis caused by less virulent organisms, such as viridans

streptococci, in addition to focal colonies and neutrophilic infiltration, evidence of

healing, including fibrin deposition and mononuclear inflammatory cells, may be present.

In endocarditis caused by Bartonella species, Coxiella burnetii, or Tropheryma whipplei,

valves primarily show chronic inflammation.

Mononuclear, rather than neutrophilic, infiltration predominates.

Abundant foamy macrophages are the primary finding in Tropheryma whipplei endocarditis.

Macrophages may also be found in Coxiella burnetii endocarditis, although the infiltration

is typically less pronounced than with Tropheryma whipplei infection, and the inflammatory response

may be mistaken for degenerative changes.

Histopathologically, Bartonella endocarditis typically shows marked fibrosis and with minimal

vegetation formation, in addition to macrophage and lymphocyte infiltration.

Bacteria may be apparent as basophilic or eosinophilic colonies on hematoxylin and eosin

(H&E) stained tissue.

Not all bacteria are readily detectable in H&E stained tissue, however; and it is common

practice to evaluate a panel of stains when endocarditis is suspected, including Gram

and Grocott-Gomori methenamine silver stains.

A tissue Gram stain is commonly used but may fail to highlight some organisms, particularly

in the setting of prior antibiotic administration.

The Grocott-Gomori methenamine silver stain, while classically used for the identification

of fungi, may offer increased sensitivity for identification of bacteria in valve tissue.

Additional stains that are useful in some settings include Warthin-Starry, Ziehl-Nielson,

and periodic acid-Schiff stains.

Like Grocott-Gomori methenamine silver, periodic acid-Schiff will highlight most bacteria and

may offer increased sensitivity over tissue Gram stain.

Periodic acid-Schiff with diastase is also the stain of choice for visualizing Tropheryma

whipplei within foamy macrophages in cases of Whipple's endocarditis.

Although Warthin-Starry stains may be used to identify weakly staining bacteria, such

as Bartonella species, staining is non-specific, and heavy background precipitate often renders

this stain difficult to interpret.

A Ziehl-Nielson stain may be used for the detection of acid-fast bacteria, such as mycobacteria,

but these are rare causes of endocarditis.

Caution should be exercised when interpreting staining properties of organisms in valves

removed from patients on antimicrobial therapy, as bacterial morphologies and stain properties

may be altered.

Additionally, the presence of organisms in tissue does not necessarily indicate active

endocarditis, as clearance of organisms is delayed compared to sterilization of the vegetation.

Consultation with a pathologist with specialized expertise in infectious diseases pathology

is recommended.

Several studies have shown that culture of valve tissue suffers from low sensitivity

and specificity, with positive cultures in only 6% to 26% of endocarditis cases.

A microorganism different from that identified by blood culture or valve PCR is detected

in approximately one-third of cases, suggesting a high rate of false positivity.

Cultures of valves from patients without evidence of endocarditis are falsely positive in one-quarter

to one-third of cases, due to contamination during processing.

Because of the low specificity of valve cultures, routine culture of valvular tissue removed

for reasons other than possible endocarditis is not recommended.

In cases of blood culture-positive endocarditis, results of valve cultures may cause unnecessary

confusion if valve cultures generate discrepant results.

In cases of blood culture-negative endocarditis, valve tissue culture still suffers from low

sensitivity and specificity, although growth of an organism does allow for antimicrobial

susceptibility testing.

When available tissue is insufficient for all tests of interest, culture should not

be prioritized over more sensitive assays, such as molecular testing.

Molecular methods used in endocarditis diagnosis include organism-specific PCR and broad range

bacterial PCR followed by sequencing.

Broad range bacterial PCR, with amplification primers targeting the bacterial 16S ribosomal

RNA gene, is a molecular method for detecting bacteria in general.

Following amplification, bacterial identification is determined by sequencing amplified DNA

followed by comparison of the sequence to established databases.

Broad range bacterial PCR and sequencing performed on valve tissue has a reported sensitivity

of 33% to 90% and has a high specificity.

Therefore, for patients with infective endocarditis who undergo surgical excision of affected

valves, and for whom no etiologic agent has yet been identified, testing valvular tissue

by broad range bacterial PCR and sequencing is recommended when histopathologic examination

of excised tissue shows acute inflammation.

Broad range fungal PCR is technically possible but has low yield for endocarditis diagnosis,

due to the rarity of fungi as causes of endocarditis.

It is not routinely recommended.

Notably, organism-specific PCR assays demonstrate superior sensitivity compared to broad range

bacterial PCR and sequencing.

Therefore, for diagnosis of culture-negative endocarditis, broad range PCR should not be

performed in lieu of organism-specific PCR.

Organism-specific PCR assays targeting Bartonella species, Coxiella burnetii, and Tropheryma

whipplei, and possibly Mycoplasma hominis should be considered.

Caution should be exercised in the interpretation of nucleic acid amplification tests from removed

valves after completion of antibiotic therapy.

Long-term persistence of bacterial DNA may occur in patients who have completed a full

course of antibiotic therapy, even cases several years after diagnosis of endocarditis.

Conversely, results can be falsely negative due to the presence of PCR inhibitors, microbial

nucleic acid below the limit of detection of the assay being used, or sampling error,

since microorganisms are often not homogenously distributed in resected valves.

A multimodal testing strategy for the diagnosis of endocarditis, employing culture, serology,

histopathology, and molecular analyses, is essential for optimal sensitivity and specificity

in identifying an infectious etiology.

A proposed testing strategy is shown.

Notably, this algorithm should be applied in the context of clinical evaluation of the

patient and other findings, such as echocardiography, supportive of a diagnosis of infective endocarditis.

Positive blood cultures are the standard means of microbial diagnosis; blood cultures should

be collected prior to initiation of antibiotic therapy.

In cases of culture-positive endocarditis, with two or more positive blood cultures,

no additional microbiology testing is necessarily needed, although histopathologic evaluation

of valvular tissue, if excised, is typically performed to confirm the diagnosis.

In cases of culture-negative endocarditis, Coxiella burnetii and Bartonella serology

should be performed, and consideration should be given to performing Tropheryma whipplei

PCR on blood.

If the patient undergoes valvular resection, histopathology and staining of the resected

valve is recommended, with evaluation by a pathologist with specific expertise in infectious

diseases pathology.

If a microbiological diagnosis has not been established, molecular testing of fresh excised

valve tissue (or formalin-fixed, paraffin-embedded valve tissue, if fresh tissue is unavailable)

should be guided by histopathologic evaluation and includes broad range bacterial PCR and

sequencing, especially in the presence of acute inflammation, as well as specific PCR

assays for Tropheryma whipplei, Coxiella burnetii, and Bartonella species.

If there is chronic inflammation with macrophage predominance, Tropheryma whipplei PCR is especially

recommended.

Ideally, a representative sample of valvular tissue should be specifically collected in

the operating room for molecular testing.

Why We Don't Like to Ask For Help - Duration: 4:21.

We don't let people help us, we don't let people support us in the referrals we're

looking for. We don't do that for two reasons. Number one we don't want to be a

burden to other people and number two we don't want to look weak and vulnerable

and we can deal with these quite quickly. Number one, let's look at this idea that

you shouldn't ask because you're a burden to other people

Marcus can we get the house lights up for me please just wanna see you all for

this. So can you do me a favour, can you put your hands in the air

if you have helped someone in your network in the last six months or so

personally or professionally, just put your hand up

keep them up for me. If you haven't got your hand up that's where you start

after today. Everything's been about getting

referrals, you should be giving them as well and looking to. Okay so keep your

hands up if you know that it turned into something positive for that person okay

I'm just going to work with the stools because I can't see everyone at the top

so please forgive me, Can I just ask you how did it make you feel? You felt good?

How did it make you you feel? You felt great. Who else had their hand up?

How does it make you feel? Thumbs up; thumbs up; awesome. How did it make you feel? Thumbs up

Did anyone feel that that person was a burden to them? No, we don't, do we?

because we don't want to be a burden and yet we get pleasure from helping other

people. If you don't ask for

help from your network you're denying them pleasure. I'm not suggesting you

walk out of here straightaway go to the first stranger on the street and say

"I need your help would, you refer me?!" I'm talking about where you have trusted

relationships people who would get the same pleasure from helping you as you

would get pleasure from helping them. So can we dispense with that reason not to ask?

The second reason we don't ask is because we don't want to appear

vulnerable or weak well let's try this out

- two different options you tell me option number one: "My business is really

struggling at the moment. I've lost five clients recently because I just couldn't

deliver the service they wanted. I think we're gonna go bankrupt within a month,

so if there's anyone you can refer me to I'd really appreciate the help" <LAUGHTER>...... or....

"We've just finished a project six-month project with coca-cola

we got phenomenal results with them they're delighted. We've just transformed

all of their business processes. Now I know Pepsi Cola have a similar problem

and I know you know someone who works there. I think we could help them I'm

more than happy to have a chat with them if you think it will be a value. What do

you think?" Does that seem weak and vulnerable? You see the difference

between the two? You can ask for a referral and seem strong and seem

confident if you believe in what you do, if you believe that what you do makes a

difference to your clients and customers then you're doing them a favor by asking

to be referred to them. And if you believe that it will come across in the

way you ask. And actually did you notice in the way I pitched that last

one, I didn't say would you refer me to them so I can sell to them, I said do you

think I could help them. I want my champion to look good by introducing me

so it's changing the dynamic and that dynamic is key because I want my

prospects, when I'm referred to them, I want them to want that conversation with

me because they know that I can help them with a problem going back to the

definition of a referral I shared earlier. Now that changes the whole dynamic of

that conversation because I'm no longer selling they are buying and we all know

that difference of being in a room with someone who already wants what we offer
