Heartworm lifecycle and heartworm prevention for dogs - Duration: 2:13.

This is Charlie, a heartworm-positive dog.

There are adult heartworms in Charlie's heart reproducing and releasing baby heartworms

called microfilariae into Charlie's bloodstream.

If a mosquito bites Charlie, taking a blood meal from the infected dog, the mosquito ingests

the microfilariae too.

While in the mosquito's gut, the microfilariae grow and mature into another stage of their

lifecycle, infective heartworm larvae.

If this mosquito then bites your pet, these infective larvae are deposited on the skin

and enter through the puncture wound.

Your pet has been infected with young heartworms.

A heartworm test can only detect adult heartworms.

It takes six months on average from the mosquito bite for heartworm larvae to become mature

adults and be detectable on testing.

Because detecting juvenile heartworms is impossible, your veterinarian may recommend repeating

a heartworm test six months from missing a dose of heartworm medication or acquiring

a dog with unknown history.

A typical oral heartworm product kills the heartworm larvae that have infected your dog in the

last thirty days.

Imagine a cleaning service coming in and cleaning your house.

Once they leave, they can't prevent your house from getting messy again.

Heartworm medication is the same.

It doesn't create a force field around your pet, protecting them from being infected.

Your pet may continue to be bitten by mosquitoes and accumulate juvenile heartworms throughout

the month.

Therefore, another dose must be administered every thirty days to clean up again.

If heartworm medication is missed, juvenile larvae grow older and can grow past the age

they are killed by monthly medications.

We've missed our window of opportunity.

This can happen with a single missed dose and result eventually in a mature heartworm

infection.

The key to preventing heartworm disease in your pet is consistent administration of your

pet's heartworm product as directed and regular testing.

Ensure your pets are safe by talking to your veterinarian about a trusted heartworm product.

7 Behavior Changes - Center for Diabetes Education - Memorial Hospital Belleville, IL - Duration: 1:00.

Seven Areas of Behavior Change Memorial's Center for Diabetes Education

Angela Weeks, RD, LDN, CDE Certified Diabetes Educator

How do you help patients who have diabetes change their behavior?

We use a framework called AADE7 for behavior change.

AADE is our professional organization American Association of Diabetes Educators, and the

7 just represents the seven areas of behavior change.

So when especially when you're newly diagnosed with diabetes or really anytime, the treatment

plan can be somewhat overwhelming and having that framework helps people to kind of make

it less overwhelming.

Work on a specific area of behavior change and focus on that and build some success and

confidence and and then keep them going.

As we see them each visit, we assess their goals and help them achieve them and build

on those goals.

Change 1: Healthy Eating

Healthy eating is a popular one that people start with and they might say, "Well I want

to make better food choices, but that's really vague, and so it's kind of a hard goal to

work on if you don't have specifics, so we'll help them make that a little bit more specific.

For example, maybe they set a goal to make half of their plate at dinner vegetables,

you know.

That's something more specific, and they can really focus in on that which will then hopefully

lead them to better clinical goals like lower blood sugar or lower A1C.

Change 2: Being Active

Exercise is has a profound effect on blood sugars.

There are some studies that show that to even ten minutes of exercise can lower a person's

blood sugar as much as 20%.

So that's something I very often share with my patients because I think a barrier to exercise

is the thought in our head that thinks, "I have to go to the gym, and I have to be there

for an hour and a half, and I don't have that time" so we don't do anything.

So empowering them to realize that even small amounts of exercise can have a profound of

effect on their health.

Change 3: Active Monitoring

Most often we think of monitoring as in self monitoring their blood glucose, but it can

also be learning about another test that we do do called A1C that gives us an overall

idea of how their diabetes is managed.

A1C is actually able to tell us what a patient's estimated average blood glucose has been for

the last three months.

So like I said it gives us an overall idea of how the diabetes is being managed.

Change 4: Taking Medication

For a lot of people, that's a new thing.

Maybe they've never had to take a daily medication or maybe they take several medications and

it's maybe hard to remember or things like that so or they're maybe not taking them at

the proper time.

So we're educating them both about how and when it should be taken, and then helping

them make that behavior change to get that medication at the right time in the right

frequency.

Change 5: Healthy Coping

Healthy coping deals with how they're coping with their diagnosis.

Many times when I greet patients at the door at the office you can see they're very maybe

afraid or overwhelmed.

Uncertain about what this means to their life and and so coping has to has to do with that

getting them through that.

Denial is also a big, big factor, and so it's very rewarding for me as we sit through that

our first visit and they leave with a smile on their face realizing, "Okay, this is something

that I can live with and that I can manage."

Change 6: Reducing Risk

Reducing risk we're teaching patients about long term what can affect their health.

A big one, for example, might be stop smoking.

But other things could be checking their feet every day.

If a person is going to develop neuropathy, it often affects their feet first, and we

sometimes forget about our feet.

And we take it for granted and so simply looking at your feet every day and making sure that

nothing is wrong can prevent amputations.

Change 7: Problem Solving

It's a hard for patients to set goals in this area, but I think it's kind of more of a skill

that we teach throughout each of our class sessions.

So many areas of problem solving, for example, maybe a patient that's taking insulin, and

they go out to eat.

Okay, when do I take my insulin, you know?

Do I take it at home before I get to the restaurant?

Should I take it to the restaurant?

Things like that.

So teaching them about how to handle those situations and problem-solve through those

situations.

The full-name is actually Hemoglobin A1C, and so A1C is actually able to tell us what

a patient's estimated average blood glucose has been for the last three months.

So like I said it gives us an overall idea of how the diabetes is being managed.

It's able to do that.

It's very mysterious to patients how do you know what my blood sugar was for three months

ago.

But hemoglobin cells live for 90 days and so the sugar in our blood or the glucose in

our blood will actually stick to those cells, so the higher... the more often the blood

glucose has been high, the higher that number will be.

What happens in group sessions?

In those group sessions, we use a tool called a conversation map.

So the diabetes educator is really a facilitator in these sessions, and the dynamic is really

quite interesting.

So in some ways, the other participants are teaching their other participants.

Session 1: On the Road to Better Managing Your Diabetes

So the first session is called, "On the Road to Better Managing Your Diabetes," and we

just kind of give people an idea of what it's going to be like to manage their diabetes

in that session.

And we talk about their feelings and how they're coping.

Session 2: Healthy Eating and Diabetes

And then the next session is called, "Healthy Eating and Diabetes" and so as the name implies

we spend the whole time talking about what that means to eat healthy, and how they can

go about that, and some different options for meal planning, and label reading, and

and how to handle special situations like dining out or sick days.

Session 3: Monitoring Your Diabetes

In the next session, it's called, "Monitoring Your Diabetes" so we help them understand

like, for example, what does A1C mean, and when should I have that test completed.

But we also talk about the day-to-day things as far as monitoring, and how to how to make

sense of those monitoring results when they test their blood sugars, how to prevent their

sugar from getting high or or low.

In order to do that you have to understand the causes.

And then in case that does happen, we prepare them how to treat those situations.

Session 4: Continuing Your Journey with Diabetes

And then the fourth session is "Continuing Your Journey with Diabetes," and so we talk

there about all the different medication options.

If you watch any TV these days you know there's quite a few options for medications for diabetes

and so we're able to expose patients to how those medicines work, and what might be an

option for them, pros and cons, and then finally some of the reducing risk kind of things like

tests they should be having, such as A1C, or a foot test, or how often they should get

their eyes checked.

Meet Angela Weeks, a Certified Diabetes Educator at Memorial's Center for Diabetes Education.

I'm a registered dietitian and a certified diabetes educator.

So I am a staff instructor, and so I see patients both individually and teach some group classes

about diabetes.

What is your goal for patients at the Center?

The goal would be to help them manage it as well as possible, keeping A1Cs at or below

seven percent.

Usually a weight-loss goal is something like about trying to help them lose five to even

seven percent of their weight.

Exercise, trying to be active at least 150 minutes weekly, so those are some of the things

that we're shooting for.

Do you have any patient success stories?

Yes I have several.

One lady came in and overweight, newly diagnosed with diabetes.

Went through our whole program and just gradually lost weight.

Started to exercise.

Got her A1C level into the normal range in fact and was just very happy with the results.

Learned things that they didn't know.

They were like, "Oh well, we didn't realize that it was too much carbohydrate to eat corn

and mashed potatoes and stuffing at the same meal."

So once they knew, she changed and was very happy.

Another man recently went through our program.

He continues to come to our support group.

He's lost in the range of about 50 pounds.

Again, diabetes is under control.

Still has it...doesn't go away, but happy customer.

How did you get into this field?

I have a sister that was diagnosed with Type 1 diabetes when I was a child, and I was immediately

drawn to being the dietitian and/or diabetes educator.

I was fascinated by the fact that what she ate was part of her treatment plan.

Never thought about a chronic disease at that point and realizing there was no cure but

again that food had such a powerful effect on her health and her treatment plan.

What training did you need to become a Certified Diabetes Educator?

So first to become a dietitian, I went to the University of Missouri-Columbia and completed

what they call a coordinated program.

So my clinical hours were included in my degree.

And so when I graduated, I was ready to sit for the registration exam to be an RD, and

then as I started my job here.

I quickly knew I wanted to be a diabetes educator, so I focused my attention that way and gained

the education hours here as a clinical dietitian, and then took that exam as a Certified Diabetes

Educator in 1999.

How many patients has Memorial's Center for Diabetes Education served?

We opened in 2006, I believe.

You have to have a recognized program so we set everything up to meet the requirements,

and we're a recognized program through the American Diabetes Association.

And I would estimate we see about 200 outpatients a year each year.

What do you hope patients gain from working with you?

Confidence and I guess the word would be relief of that the stress of feeling overwhelmed

and not knowing.

May sound cliche but knowledge is power in this situation and again when when patients

go through those visits they're saying, "Wow this was so helpful" and "Why didn't I come

sooner?"

And it feels good to have them feel empowered and refreshed.

It's supposed to be able to get rid of heartburn and stomaches too.

I guess this was something thought up by the food stalls. By cutting the ingredients into small pieces, the flavor of the dashi will get absorbed. And the only place you're going to find this anywhere is here.

When doing the tapes it helps to use a mirror

see what you are doing

I use a mirror most of the time

I use Nexcare Sensitive Tape

You don't need much

You will rip the tape in two pieces

One on your cheek, and one under your eyelid

You only need about this much

[rip it longways down the middle]

[put one piece by the corner of your mouth, sloping up to the center of your ear]

[Put the other piece under your lower eyelid]

(This is where a mirror is most handy)

[Make sure you "lift" the eyelid so it doesn't leak!]

(the tape may wrinkle up if you are going fast like me, but if you go slower, it looks smoother)

(On days that I don't care what it looks like, I don't take as long to apply it)

You want the tape to go UNDER the lash line

Don't tape your lashes!

You might have skin wrinkles like this

Maybe not

You can adjust the tapes as needed

You can play around with them a bit until you are satisfied

See, you can move the tape

You want the tape to stay sticky

Don't play around with them too much

The tapes help stop my eye from "crying"
