An Obamacare Provision Is Saving Money And Lives - Duration: 8:33.

So, Sheree, embedded in the Affordable Care Act ... I think people know the broad strokes,

and I would say I think America is far more conscious of the broad strokes of what the

Affordable Care Act … we should say the patient protection of the Affordable Care

Act because I think people are becoming more and more aware of the patient protections

involved in what we now call Obamacare.

Within the major pillars, the expansion of Medicaid, the regulations on what constitutes

actual health insurance, the baseline for these things, and then the subsidies involved

and the exchanges, there are a lot of different pilot programs, and that's the one that you

write about that is taking place in Missouri.

Tell us about this story in Missouri.

The story in Missouri is actually a reflection of many things that started happening around

the country that were funded by the Affordable Care Act several years ago, actually.

2009, 2010, 2011.

This program was an integrated care experiment that was funded for Medicaid patients.

Patients that are often called the 5-50.

The five percent of patients who use 50% of medical care, and can cost in some communities

as much as $10,000 a year in ER costs and hospital costs.

In this case, people who also were struggling with various mental illness.

These types of innovations, these types of programs were a key part of the ACA.

This one, I believe, was done through the Social Security Administration.

There are others that were largely done through the Centers for Medicaid and Medicare Innovation

Center, which is still alive, but there is question about how long it may be.

There were efforts to reform payment models and give people better care.

All right.

Let's start with this 5-50 designation.

I've heard of that dynamic a lot more as of late.

For many years, we heard that basically 80% of health care costs are born by 20% of the

population.

This is, I guess, a subset of that 20% population, which bears an even greater percentage of

cost.

Let me ask you this, and then I want to go through what that profile of a person, what

would cause that person to be requiring so much care, but when we talk about the United

States as being much more expensive in terms of health care per capita, is it because that

five percent exists and doesn't exist in other situations?

Is it because the care for that five percent is that much more expensive?

Is it because we lack preventative care that would undercut that?

Does it, I guess, maybe speak to the success of this program as where the delta is between

that five percent in this country and that five percent in, say, Canada?

Yeah.

I would like to say in this case that five percent is usually defined by two or more

chronic health conditions.

Say they have heart disease, diabetes, and hypertension.

In the case of this program, they are also struggling with a serious mental illness.

That is even if it's depression, it's serious enough to require medication.

Washington Monthly Magazine specifically asked me to investigate and to look at programs

for that group.

That's how they're defined, but these people are, of course, all over the world.

The big answer is to why it costs too much, that is the billion dollar question.

I suspect that it is how we care for people in the United States, how we charge for care,

and the lack of a support network for people who really need it, which is what these programs

reveal, which is what the difference is in the health indicators and the health quality

and the life quality of these people when they do get that support.

But it's a very special kind of support.

It's very rare in the United States.

It might be that that support is more available in a country where there might be universal

care, or where social services might be easier to access.

In this particular case, all of these people are Medicaid patients.

It is a fascinating story.

You focused on a woman, Pat Powers, and her treatment at the Crider Health Center in St.

Charles, Missouri.

You really do get a sense of what can go wrong when you have a patient who is taking so many

different medications, but they're coming from so many different sources in some ways.

Yes.

She had a mental health provider who had no relationship with her primary care provider.

They didn't know each other.

She was initially trying to juggle several part time jobs to hold herself together, which

was making her anxiety disorder and depression and other psychological issues more difficult

to manage.

On top of that, she was trying to manage diabetes and weight and medication for her behavioral

disorders all on her own.

She eventually couldn't deal with the job, which meant not being able to keep up with

the insurance.

I think she might have briefly lost housing though she has housing now.

It just came apart.

It requires a great deal of attention.

One of the care providers that I interviewed, Nancy Gongaware, who was a nurse navigator

in this system said, 'Just think of how much it takes for a person whose life is completely

together to just manage diabetes.'

Right.

Not diabetes and hypertension and potentially obesity.

Which, by the way, weight gain is exacerbated by the kinds of medications that manage some

forms of mental illness.

It changes your body's ability to gain weight, and then you're managing anxiety, and then

you're managing depression, and you're trying to hold down a job.

I would imagine there are reactions between the medication, too.

If you don't have the same person who is prescribing them or isn't aware of what's being prescribed,

and you have a patient who is having a tough time maintaining the different medications

that they are supposed to be taking, or that they are taking, that I imagine these interactions

become a problem.

Yes.

This is a patient centered medical home demonstration, and they were carried out all over the country,

and hopefully they will continue to be.

One of the key aspects of a medical home is to make sure the providers caring for a person

interact with each other and share medication information, and share information about hospitalization

so that a behavioral health provider, for example, knows when someone ends up in an

ER.

So that they don't have this situation where a person is being overmedicated or having

interaction.

this pilot program and this grant that Missouri got, and I want to talk about why they're

getting a pilot program for Medicaid when they didn't accept the expansion of Medicaid

in that state.

Everybody hates Pippa Middleton. That�s basically what the new report from the New

York Times is saying. The Times doesn�t usually go after such major figures so blatantly,

so they must have gotten permission from some high up royal sources on this one. Scandalous.

The report goes in depth at exploring how Pippa was the belle of the ball during Kate

Middleton�s wedding day to Prince William, when nobody could stop talking about her dress

or ass. However, two years later, everybody hates her. Well, hate is a strong word, but

several prominent UK journalists go on the record stating that �people here just don�t

like her.� Fair enough. Since Pippa�s home entertainment

book sold very poorly last year, she�s since landed a job at Vanity Fair as well as landing

a �Friday Feasts� column at a major British magazine which people apparently don�t like.

According to these sources, �She is not a cook, not a writer and has not really done

anything but get this all off the back of her sister. Generally, there is a feeling

that she is simply capitalizing off the situation.� An editor for the Sunday express adds, �There

is some pretty sneering commentary over here that she has been cashing in on her sister.�

But she also adds, �She may have seemed like a shrinking violet around the time of

the wedding, but she is not. She has to make a living and this is her found fame.�

Obviously, the editor of Vanity Fair denied the rumors that they weren�t happy with

Pippa, saying, �We�re delighted to have Pippa. She�s a keen observer of classic

British pastimes.� Well, what else is he going to say? Oops, we made a big mistake

in hiring the sister-in-law to the future King of England, let�s fire her? No, he�s

not. Of course some of Pippa�s antics could have

adversely affected public opinion. Her drunken gun waving joy ride through Paris wasn�t

exactly welcomed by the Palace and painted Pippa as a spoiled dilettante, which of course

she is. Then there is her peevish reaction to being satirized for her inane home entertainment

book, Celebrate: A Year of Festivities for Families and Friends. The offending lampoon

in question is the book, When One is Expecting: A Posh Person�s Guide to Pregnancy and Parenting.

Really what can Pippa expect? Nobody likes to see the blatant nepotism of which Pippa

These damn little things have been everywhere lately!!

It's the new trend to sweep the nation.

I go to the gas station they are there!

I look online,there they are!

I go to steal money from my grandpa's wallet and he is playing with one.

So like I said, they are everywhere!!

I'm sure you are well aware of these little devices if you have spent any amount of time

online in the last couple of months.

They are small enough to fit into the palm of your hand.

Cheap enough to buy with the money you find in your couch And so simple it's kinda crazy.

You see they are typically plastic devices that come in a variety of shapes.

They have a rotatable component in the middle that usually uses metal bearings to allow

the user to spin the devices between their fingers.

Why?

Because America God Damnit!!!

Actually these little devices have become quite popular all around the world.

The devices primary use is to fidget with, which makes the name of it make a lot of sense

They were invented by A Florida woman named Catherine Hettinger who suffered from myasthenia

gravis.

Jesus I swear some of these medical terms are like somebody just took a bunch of letters

and pulled them out of a hat and whatever came out was the order of the word.

Basically she suffered from muscle problems and wanted a toy she could share with her

young daughter that didn't require much pressure or strain on her muscles.

So instead of playing video games like everybody else she decided to invent the fidget spinner

in the early 90's.

She eventually filed the paper work to protect her invention in 1997 and even had a deal

worked out with Hasbro!

So why does her granddaughter look pissed off in this picture?

She's rich!

She'll never have to work a day in her life again.

She can probably go sip champagne on a yacht with P diddy , Beyonce and that guy from monopoly

games.

Millions of the damn things have been sold!

She must have made a million bucks by now!

Well….Sadly….that is not the case.

The deal with hasbro fell through and she hasn't made a damn cent from her invention!

You see Catherine didn't have the money in 2005 to re-file the patent on her invention

meaning that after that point anybody who wanted to could take her idea and sell the

devices.

Which is what is currently happening, so…

Sorry Catherine!

Maybe think of a new invention.

Try like inventing a stuffed animal that you put beans in and call them bean babies or

beaner babies or something like that.

That's just an original idea that came off the top of my head.

I am full of them!

So why in the hell is this little thing so popular?

Well… for a few reasons actually.

It's cheap.

It's simple.

It's entertaining.

It's satisfying and believe it or not it actually serves a purpose.

It serves as a simple tool that helps relieve stress and anxiety.

Although I couldn't find information of studies being conducted, some people believe

that the devices can help those with too much energy or concentration problems.

Such as children with ADD or ADHD or ADDDDD. you know one of those medical problems.

The ones that doctors prescribe cool drugs for.

Although they may or may not help kids with certain problems the popularity of the device

has lead to their mass use on school grounds across the world, which in turn has led to

numerous bans and teachers confiscating the toys.

Back in my day it was Pokemon cards so history repeats itself in some way!

I'll never forgive you for taking my holographic chancy card Miss Mccandles.

Never!

But the biggest reason behind the popularity of the fidget spinner is the trend!

They have become popular which has caused more people to get them and use them which

caused more to do the same and before we know it everybody and their weird uncle has one

in their hands!

It's like the macarena all over again!

And when you have people making videos doing badass stuff like this with the fidget spinners….

how could you not want one for yourself?

The demand for the fidget spinner has reached such levels that companies can't even keep

up with their orders.

There are numerous reports of the item selling out and being on back order for extended periods

of time.

It wasn't long before the trend made it's way to youtube where Youtubers started to

exploit the trend for views!

Wait why are you looking at me like that?

I'm totally a legit fan of fidget spinners!

I'm not just making this video because it's popular.

No!

Not me!

na uh!

Some Youtubers even took the device and used it in very interesting and creative ways.

Like one of my favorite Youtubers Mr. John Hill.

"oh my gosh"

"do it again and ill get a close up of- how do you feel?"

"pretty freakin good!"

And finally like most things that reach a certain level of popularity, memes focused

on fidget spinners began to surface and make their rounds online amongst various meme communities.

Often focusing on the ridiculous nature of the trend!

So there you have it!

What was once an invention that was created in the 90's eventually became an idea that

everybody stole to start a strange trend that has swept the nation!

The lesson to be learned today is to never send naked pictures of yourself to your mailman

…I mean uhh…

To file your copyright papers so others don't make money off your invention….yeah, that's

what I meant!

But hey!

Have you heard about a Theodore Dreiser's novel "The financier"?

This is the first volume of "the Trilogy of Desire" which also includes "The Titan" and "The Stoic"

"An American tragedy" written by the same author is only of my favorite books

I've read it 5 or 6 times I think

It was inspired by the real story of Chester Gilette and someday I want to buy his diary

But back to the book

This is a story about a very ambitious and very enterprising self-made financier

who by his 30th becomes one of the financial powers in Phili

and manages to grow a pretty large fortune through stock speculations

But then he loses everything because of financial panic caused by the great Chicago fire

His legally questionable acts become known and his both financial and political allies abandon him

And by the end of the first book he is sent to serve the sentence for larceny and theft

Dreiser is a naturalist. His art is the art of facts and scrupulous description of events and things.

His characters are very persevering. They know what they want and they try to get it no matter what

I really have no idea how it is going to end, but I believe it's main idea is that

if you are good at something, stick with it and you will find success

And if you don't give up, failure is just a temporary state until you succeed

Yes, I do love inspiring stories and I absolutely love "The Pursuit of happiness"

I think such books and movies are able to make people better

and certainly they inspire us to fight for our future
